Current Progress of Structural Studies on β2 Adrenergic Receptor

ABSTRACT

Beta-2 adrenergic receptor is an important member among G protein-coupled receptors superfamily. As a membrane protein, it is difficult to crystalize and its three-dimensional structure is not known until recent years. Thanks to the development of the crystal engineering techniques and the breakthroughs in the free electron X-ray laser crystallography, many crystal structures of β2 adrenergic receptor at different states become available now. The discovery of the crystal structure β2 adrenergic receptor has enriched our understanding on many life phenomena and its impact to the structure-based drug design cannot be overestimated. We review the discovery history of β2 adrenergic receptor crystal structures, the structural characteristics of the receptor, the interactions between the receptor and different subtypes of the ligands (agonist, inverse agonist and antagonist) and clinical applications.

摘 要

β2肾上腺素受体(β2 adrenergic receptor, β2AR)是生命体中一类重要的G蛋白偶联受体。作为一种膜蛋白,由于结晶困难过去很难获得其晶体三维结构。近几年来,得益于蛋白质结晶技术和自由电子X射线晶体结构测定技术的进展,各种状态下的β2AR晶体结构获得解析。β2AR结构的发现丰富了人们对生命现象的解释,对于当今新药物设计具有重大意义。本文主要综述了β2肾上腺素受体晶体结构的发现历程、结构特征与不同类型配体(激动剂、反向激动剂和拮抗剂)的相互作用机制以及其在临床医学上的应用研究,并指出这一研究领域的未来发展方向。

1. 引言

G蛋白偶联受体(G protein coupled receptors,GPCRs)是迄今为止科学家们发现的最大的一类七段跨膜蛋白受体超级家族。人类大约有上千种GPCRs并且有约1000个基因来编码这些受体。GPCRs可以接受无数不同的细胞外信号如气味、光线和神经递质、激素等等,并将其转化为胞内信号,参与人体内几乎全部生命活动[1] [2] 。正因如此,GPCRs与许多疾病息息相关,如肿瘤、心脑血管疾病、自身免疫疾病等。目前全球临床药物中,GPCRs作为药物靶标的市场比例超过一半[3] 。2000年,第一个哺乳动物GPCRs—牛视紫红质(Rhodopsin)的晶体结构被成功解析[4] 。2007年,美国科学家 Brian K. Kobilka团队成功解析了第一个人类GPCR—β2肾上腺素受体的晶体结构,这使得G蛋白偶联受体晶体结构的研究成为了生物分子结构领域的探究热点。之后越来越多的G蛋白偶联受体或受体与小分子复合物结构得到解析[5] 。

2011年,Kobilka又成功获得了处于活性态下的β2肾上腺素受体晶体结构[6] ,这更是开启了GPCR结构和信号传导机制研究的新纪元。β2肾上腺素受体复合物的三维晶体结构的解析,让人们以崭新的视角来分析其受体结构、信号传导和调控机制等,从而给受体研究带来新的突破。通过对药物小分子结合的β2肾上腺素受体结构相互作用的深入研究,这将有助于人类深刻地认识其机体生命活动。从而使得β2肾上腺素受体配体成为新药物开发的研究热点,并且丰富该受体在药理学研究上的重要意义。

2. β2AR晶体结构的发现

追溯到1968年,美国科学家Robert J. Lefkowitz利用放射性同位素追踪细胞表面受体:将同位素125I引入到配体,并以此来追踪配体状态,这样发现与结合的受体,从而验证了细胞表面存在识别肾上腺素的物质[7] 。随后,Kobilka加入Lefkowitz团队,从仓鼠基因组中分离出编码β2肾上腺素受体基因。1986年,Kobilka成功地克隆出了人类第一个GPCR,即β2肾上腺素受体(β2AR)的完整的基因序列,该序列可以编码一类由413种氨基酸残基序列组成的蛋白质[8] ,该实验直接证实了受体的存在。这一里程碑式的重大成果更加推动了GPCRs的研究与进展。

要对GPCRs作用机制有一个全面的理解,需要获得其三维结构,Kobilka继续以β2AR为研究对象,试图解析β2AR晶体结构。事实上,对于β2AR,结晶过程本身就是一个难题,因为膜蛋白的含量低、缺乏极性表面和非稳定性结构等特性,从而使它本身成为一类很难结晶的蛋白质[9] 。虽然在2000年,Palczewski等人解析了分辨率为2.8 Å牛视紫红质的晶体结构[4] ,但视紫红质不同于GPCRs其他成员的独特之处在于牛里面含量丰富并且结构也相对稳定[10] 。因此,在GPCRs研究领域,视紫红质不具有很好的说服力和代表性,从而导致GPCR结构研究也一度停滞不前。

带着对GPCRs晶体结构的极大兴趣和坚持不懈的决心与毅力,在2007年,以Kobilka为首的团队将一个抗体Fab5结合到β2AR第3个胞内环(Intracellular loop 3, ICL3)中,得到了稳定的受体晶体结构。Day等人揭示了该抗体结合到β2AR后,不会改变激动剂或拮抗剂的亲和性,也不会妨碍激动剂引起的受体结构变化[11] 。与此同时,结合反向激动剂卡拉洛尔(Carazolol) [9] 抑制受体的基础活性以及去掉了没有功能作用的羧基末端的部分氨基酸残基序列,最终获得分辨率为3.4 Å的配体–受体复合物晶体结构。另外,通过蛋白酶敏感性和分子内荧光共振转移光谱实验,Granier等人发现了β2AR未解析的最多结构区域之一是ICL3 [12] 。ICL3很难结晶,但是与信号分子如G蛋白能相互作用以及与G蛋白激活相关。如果截去ICL3,这能够减少跨膜螺旋的移动,但这也减少了有助于形成晶体的极性表面[13] 。由于T4溶菌酶是一种增加了极性面和限制螺旋移动的蛋白质,所以Kobilka团队利用蛋白质工程技术又将T4溶菌酶插入到β2AR的ICL3胞内环中,又获得了2.4 Å高分辨率的配体-受体复合物晶体结构[14] ,如图1所示。

β2AR的结构由核心区域7段跨膜α螺旋区(Transmembrane 1-7, TM1-TM7),以及膜外N端(N-termi- nus)、膜内C端(C-terminus)、3个膜外环(Extracellular loops, ECL1-3)、3个膜内环(Intracellular loops, ICL1-3)构成。ECL2氨基酸大部分形成α螺旋结构以及小部分无规则卷曲结构。其中ECL2有很重要的生理功能,会形成一个似盖子的结构罩住配体结合位点,这样很有可能对配体的进出造成阻碍,其保守的半胱氨酸Cys184和Cys190与TM3上的部分残基、配体相互作用[15] ,以及Cys190与Cys191,形成二硫键,这对稳定受体晶体结构以及结合配体具有重要意义。利用这些技术与方法,之后科学工作者们继续解析了很多GPCRs的晶体结构,这些对于GPCRs作用机制有了更详细的解释。Graaf和Bokoch等人分别用分子对接(Molecular Docking)技术来区分β2AR激动剂与部分激动剂,用核磁共振谱(Nuclear Magnetic Resonance, NMR)技术来研究配体结合到β2AR的构象变化。结果表明不同的配体小分子结合到受体,会引起G蛋白激活不同的效应,从而导致受体本身不同的构象变化[16] [17] 。但是激活后的受体更加不稳定,其活化晶体结构依然没有得到解析。那么受体激活后的晶体结构到底是怎么样呢?会产生哪些重大变化呢?

2011年,Kobilka研究组又取得了更大的突破:在之前的技术基础上,他们又结合纳米抗体技术,先后获得了结合激动剂的处于激活状态下的β2AR-T4l-Nb80复合物晶体结构[6] 和β2AR-Gs高分辨率复合物晶体结构。同时,β2AR-Gs晶体结构揭示了当配体激动剂BI-167107 [18] 结合到β2AR时,会引起受体的结构变化,使得TM5发生轻微的旋转并且远离配体结合区域和TM6向胞外产生很大的偏移。这捕捉到了β2AR如何被配体激活以及再激活下游G蛋白从而向细胞发送信号的过程。2012年,由于肾上腺素受体上的相关研究所做出的突出贡献,Robert J. Lefkowitz和Brian K. Kobilka被授予诺贝尔化学奖[19] [20] 。到目前为止,科学工作者们一共解析了16种β2AR复合物结构,如表1

近几年,自由电子X射线激光蛋白结构测定技术取得突破性进展。由于该技术无需大尺寸的蛋白晶体并能捕捉到快速动力学过程,所以已成为研究GPCRs等膜蛋白另一种重要手段[21] 。解析GPCR的非活化和活化晶体结构从分子水平上,为解释其作用机理提供了很重要的信息。这都受到了世界各地许多生物学家、化学家和药物开发等研究人员的青睐,为新药设计和研发,从而应用于临床治疗提高结构基础。

3. β2AR与配体的相互作用机制

β2AR和配体药物分子已经应用于许多疾病的治疗中。因此,在研究β2AR复合物结构中,深刻理解

受体和结合配体的特性,深入研究它们间的作用机制,对于药物开发具有很重要的作用。故以下系列问题有待解决:受体的结构和功能是怎么建立联系?那么配体到底是怎样结合到受体?又会是对于不同配体分子做出了哪些反应?

GPCR存在至少两种结构状态,即处于非活性态的受体R和活性态的受体R*。R被认为不能激活G蛋白;而R*能够耦合到G蛋白,产生相应的生命活动[29] 药理学上将配体对受体R和R*结合的亲和性不同,大致分为激动剂、拮抗剂、反向激动剂、部分激动剂或反向激动剂以及偏激动剂。激动剂是指结合并激活受体,使受体转向激活态而产生一系列生命效应。反向激动剂与非活性受体结合,产生于激动剂相反的效应。拮抗剂会阻碍的内源性激动剂到受体所产生的效应,不会引起生命体的物理化学反应。反向激动剂和拮抗剂选择性作用受体不同,拮抗剂还会阻碍激动剂和反向激动剂引起的效应。相对低亲和性的配体就是部分激动剂和反向激动剂。偏激动剂则指结合到受体并且引起G蛋白的独立信号通路[30] -[32] 。目前发现的与β2AR结合的配体典型骨架结构如图2所示。

最早探索肾上腺素受体的配体结构和功能关系的是Lands等人[33] 。研究人员利用不同的生物物理方法比如荧光标记亲和力研究和荧光探针分析等来探究β2AR与配体的结合模式,并得出了不同类型配体引起的受体结构变化是不同的结论[34] 。早期的科学工作者们利用荧光探针标识位于β2AR的ICL3的Cys265上,侦测4种配体结合受体的结构变化,发现了TM5和TM6的移动变化,激动剂异丙肾上腺素(Isoprotereno, ISO)结合的受体结构区域荧光强度减少了15%,部分激动剂沙丁胺醇(Salbutamol, SAL)和多巴酚丁胺(Dobutamine, DOB)更少的变化,拮抗剂阿普洛尔(Alprenolol, ALP)区域没有影响,没有构象变化但是减少了结合区域的适应性[35] [36] 。这对于β2AR的激活结构不同变化同样也可以应用到其他的GPCR研究上。

2007年在研究反向激动剂与抗体结合到非活性态β2AR的复合晶体结构中,Rasmussen等人分析到受体结构上的Asp113、Val114、Phe289、Phe290和Asn312残基与反向激动剂相互作用,反向激动剂则更是加强了TM6上的Leu272与其他残基作用,减少了受体基础活性,稳定其处于非活性态[8] 。之后,他们又用荧光探针附到TM6细胞质末端的Cys265上,观察到了激动剂和反向激动剂引起的受体构象不同。在配体4 Å内的残基侧链形成结合腔,结合腔位于疏水残基和一些极性残基处[23] ,并且配体能够穿透到受体15 Å深处[37] 。在激动剂的作用下,β2AR的TM3、TM4、TM5和TM7形成结合腔,残基Asp113、Ser204、Ser207与儿茶酚胺激动剂形成氢键相互作用,激活受体。在小分子配体BI167107结合受体结构中,配体与Asn312、Asp113、Ser203、Ser207形成氢键相互作用,TM5向内移动,导致Ile121、Phe282重新相互作用,进而TM6和TM7移动变化[17] 。有关学者做了20 ns的激动剂BI167107结合受体的分子动力学基础,发现前6 ns有6对氢键作用,后14 ns有9对氢键作用,表明激动剂BI167107很灵活地和受体结合相互作用,稳定受体处于活性态[38] 。在反向激动剂的作用下,保守残基Glu(Asp)ArgTyr三氨基酸“离子锁”破坏,使得受体处于非活性态。这些都揭示了激动剂和反向激动剂结合受体不同的方式导致结合位点的不同[12] [39] 。

研究显示,真核膜蛋白里面的胆固醇有助于降低膜的流动性和弯曲性,所以胆固醇能够用于结晶学中加强稳定蛋白质结构的结晶提取,并且不会改变配体结合位点[22] [40] 。在之前的基础上,Cherezov和Rasmussen提取出胆固醇稳定的β2AR-T4l-carazolol复合物结构晶体,得到了新的发现。在受体结构上,二硫键Cys106-Cys191,Cys184-Cys190稳定了ECL2;TM2、TM5、TM6和TM7的扭曲与G蛋白激活有关。反向激动剂Carozolol与Phe208、Phe289、Phe290以及Trp286组成芳香环π-π相互作用[13] 。Costanzi做了基于视紫红质的同源建模和野生型β2AR结构比较研究,结果得出其配体的结合腔为TM3、TM5、TM6和TM7,形成极性和疏水作用,配体反向激动剂偏向于TM5,与丝氨酸Ser203、Ser204、Ser207相互作用[41] [42] 。另外,Bhattacharya做了不同配体效应调制的受体势能面(potential energy landscape)实验,结论表明反向激动剂、全激动剂和偏激动剂都有着不同的结合能量反应路径[43] 。但Dror等人以分子动力学模拟发现了阻滞剂(拮抗剂和反向激动剂)和激动剂结合到受体过程中,都有着相同的结合路径并且与胞外也有相互作用[14] 。这会为药物小分子和受体结合过程的阐述与优化提供了基本证据。

另一方面,在受体稳定性研究中发现,仅仅只有激动剂是远远不能稳定受体处于活性态的[44] 。2011年,Sprang得出G蛋白和激动剂结合到GPCR,才能稳定其活性态[45] 。为了探究一个激动剂是否使得受体稳定于活性态,Rosenbaum等人通过共价激动剂结合的β2AR晶体结构,做了长时期的30 μs的动力学模拟,最后的结果揭示了一个激动剂不能使β2AR结构平衡达到完全的活性态,而G蛋白或抗体能够一起去使得受体稳定于活性态。这与荧光寿命试验结果一致[24] 。另外,Roth和Hanson共同研究发现谷氨酸突变为色氨酸、络氨酸即Glu122Trp和Glu122Tyr能够增强受体的稳定性。在突变的分子模拟中显示色氨酸的环和络氨酸的羟基位置接近TM5螺旋断裂处,它们都与Val206的羧基氧相互作用,从而稳定了螺旋结构[46] 。另外,水分子在生命系统功能和结构起着一个必需的作用,是对配体和受体重要的结合位点,有助于受体筛选[47] 。

总之,氢键在蛋白质和配体相互作用中是一个很重要的元素,对于配体的结合特异性起着很关键的作用。由于受体的螺旋结构特征,氢键是由氨基酸侧链形成。基团NH在拮抗剂,NH或OH在激动剂,这些结构的氢键可以鉴别激动剂和拮抗剂。对于同一受体,其不同作用的配体之间有一些化学结构上的变化。近一步地,基于X射线晶体结构和同源建模,Levit等人通过统计分析序列、物理化学性质、结构等30种描述符的GPCR结合位点特性,发现了GPCR拮抗剂的化学结构变化与结合位点的氢键数量无关,而与结合位点的疏水性有很大关联。在GPCR胞外,受体结构形成二硫键作用的数量与拮抗剂的化学结构分子量变化无关,而与配体进入结合路径相关,这决定了GPCR拮抗剂的大小选择性[48] 。配体和蛋白质相互作用(静电作用、氢键、范德华力、疏水作用)的结构信息以及配体的形状与方向性对于药物设计有着指明意义[49] 。

4. β2AR在临床医学上的应用

β2AR是一类作用于主要保证人体紧张状态时生理需要的交感神经系统的重要膜蛋白受体。通过介导体内儿茶酚胺类激动剂的生理效应,调节着心脏活动。β2AR首选作为临床上的药物靶标,对于如哮喘、心血管、高血压和心力衰竭等重大疾病有着广泛的治疗用途。在心血管系统中,β2AR可以调节心肌代谢、心率舒张和收缩功能,其拮抗剂用于治疗心血管疾病中;在慢性心力衰竭下,激动剂儿茶酚类的提高引起了肾上腺素的信号,这个增加的肾上腺素信号是心脏衰竭的重要的预兆指标[50] 。哮喘是一种常见的慢性呼吸道炎症性和平滑肌阻碍疾病[51] ,目前用于治疗哮喘的药物种类不胜枚举,β2AR激动剂如沙丁胺醇是治疗哮喘最常用的药物[50] [52] 。据有关研究显示,β2AR激动剂适合短期作用于心力衰竭者,而长期作用会导致死亡率增加;但阻滞剂(拮抗剂和反向激动剂)作用刚好相反。所以激动剂适合于急性疾病,而有害于慢性疾病,但是阻滞剂比较适合于慢性疾病,可能对急性疾病有害[53] -[55] 。

基于基本的受体活性和配体相互作用,毫无疑问β2AR在疾病治疗上越来越占据着重要的作用[56] 。Loza等人发现β2AR激动剂可以针对于T细胞上疾病调节作用[57] 。其中一种激动剂奇帕特罗(Zilpaterol)还可以提高牛的繁殖率[58] 。Larocca表示β2AR激动剂药物在调节内分泌和免疫系统、影响呼吸功能以及减少哮喘和肺部疾病有很明显的效果[59] 。学者们还找到了一种新型的治疗肥胖症的抑制剂[60] ,为广大肥胖病者带来了曙光。Pullar等人更加揭示了β2AR拮抗剂能够加速受伤皮肤的修复[61] ,给伤患者减少痛苦。在于近期新型药物TD-4306 [62] 研究中,改善了激动剂,让其也能用于慢性疾病中,极大地丰富了配体药物,更加推动了受体的应用研究。此外,一些研究发现,除了β2AR,很多其他的GPCR也成为临床上的靶标,用于治疗受伤、疼痛以及成瘾药物的设计[63] [64] 。

5. 总结与展望

本文简要地综述了典型的G蛋白偶联受体–β2肾上腺素受体三维晶体结构的发现历程,并且重点阐述了β2肾上腺素受体与配体之间的相互作用模式以及配体药物分子在疾病上的应用实例。从已知的三维晶体结构得到,激动剂和反向激动剂的结合腔是类似的,激动剂并没有引起结合点的很大变化,但是这些细微的变化不能由简单的计算机模拟建模技术捕捉到[65] 。这些更多细微的结构可能更加要结合结晶技术、NMR、电子顺磁共振(Electron paramagnetic resonance, EPR)和氢氘交换(Hydrogen/deuterium exchange, HDX)等实验手段 [23] [66] 。晶体结构的信息对于β2肾上腺素受体药物乃至G蛋白偶联受体药物设计有很重要的帮助,尽管很多晶体结构已经解析出来了,但是其结构信息对药物开发者来说依然不是很清晰、明确,所以这就需要科学工作者们继续开发各种新方法去获得受体和配体结合的晶体结构。已经解析的受体β2肾上腺素的活性和非活性结构,这对于拮抗剂和激动剂的药物发现有很重要的价值。Aristotelous等人用表面等离子体共振生物传感器(Surface plasmon resonance biosensor, SPR)方法发现了野生型β2肾上腺素受体的新颖的、高亲和性的拮抗剂[67] 。Wang等人结合分子动力学、分子对接和自由能计算来探究激动剂和受体之间的结合模式[68] ,这也为分子结构上受体药物发现与设计研究开辟了新途径。活性药物配体分子结合受体,阻止机体的一些病变,激发其免疫细胞的活性。因此,设计不同结构的配体药物分子,将β2肾上腺素受体靶标更加广泛地应用到人类临床疾病治疗、药理学研究领域,这必将是一条长期的、充满挑战但前景光明的道路。
