Abstract:
Contraceptive self

efficacy is an important factor
that
affects individual contraceptive behavior. Many re-
searchers indicated that the higher ones
’
contraceptive self
-
efficacy
is
, the more positive ones
’ contraceptive behaviour
will be. This article reviewed the research status of contraceptive self
-
efficacy, ai ming to show the research hotspots of

and increase the

摘
要:
避孕自我效能是影响个体避孕行为的重要因素,大量研究显示,避孕自我效能水平越高,个体的避孕
行为越积极。文章对避孕自我效能的研究现状进行综述,旨在展示避孕自我效能的研究热点,提高医务人员的
认识,同时分析避孕自我效能的研究空白及不足,预测避孕自我效能的研究方向。

引言
育龄女性的生殖健康直接关系到出生人口素质
和国民整体健康水平

指出,到

年,中国孕产妇死亡率控制
在
20/10
万以下,保障妇女享有避孕节育知情选择权,
减少非计划妊娠,降低人工流产率。避孕自我效能是
影响避孕行为的重要因素。为进一步研究避孕自我效
能对避孕行为的作用及其影响因素,探究避孕自我效
能的研究热点及研究方向,现对避孕自我效能的综述
如下。

概念
自我效能指个体对自己是否有能力去实施某一
行为的期望,是人们对自我行为能力的认知与评价
[3]
。
自我效能是个体对自身行为及环境实施控制能力的
信心,在个体健康行为的实施中发挥着重要作用
[4]
。

行为研究上,并将避孕自我效能定义为青少年的信念
强度,这里的信念指的是青少年能够在性生活和避孕
情境中实施控制,达到避孕保护目的的信心。

认为避孕自我效能是有性活动或潜在性活动的
个体控制性、避孕情境的坚定信念,避孕自我效能也
是性活动个体应用避孕措施的动机限制因素或增强
因素

。
概括地说,避孕自我效能指的是女性对自己能够
控制性和避孕情境达到避孕保护能力的信念强度
[5,7]
。
对自己应用避孕方法的知识和能力有信心的妇女能
够更容易坚持采用避孕措施,她们能够识别、摒弃错
误信息,并且以有根据的避孕观念作为避孕的依据
[8]
。
具有高自我效能的妇女更有信心拒绝没有避孕保护
的性交,尽管避孕有潜在的不良影响,但她们仍然倾
向于使用避孕措施
[9]
。

理论基础
根据自我效能理论的框架,个体对其能够实施某一
行为的期望将决定她在达到目 标时能否行动 和坚持
[ 10]
。
自我效能又被称为自我效能感
(Perceived Self
-
efficacy)
、
自我信念
(Self
-
efficacy Beliefs)
、自我效能期待
(Self
-
efficacy Expectancy)
。该理论是由
Bandura
[10]
于
1977
年
在其社会认知理论
(Social Cognitive Theory)
中提出。
自我效能强调人是行动的动因,即人的能动性。
班杜拉认为,个体的动机水平、情绪状态和行动更多
依赖于他们对自己的信念
[3]
,个体达到以自我为主导
的行为改变不仅仅需要改变危险行为的理由,而且要
有对自己能力的信心,即自我效能,从而进行自我控
制
[6]
。社会认知理论认为知识、态度、自我效能等个
人因素可以增强或减弱完成某一行为的可能性
[11]
,自
我效能是促使行为改变的重要先决条件
[ 11]
,病人对行
为改变的自我效能能够促进其健康结果

及健康行

,低自我效能与危害健康的行为有关

。
4.
避孕自我效能的测量工具
避孕自我效能量表是目前国际上使用最广泛的
测量避孕自我效能的量表。该量表是
Levinson
[5]
将自
我效能理论扩展应用到青少年女性避孕行为上,为了
评价有性行为的青少年女性的避孕应用障碍而研制。
避孕自我效能量表测量年轻女性控制性和避孕情境
达到避孕保护的信念强度
[15]
。量表由

过来的条目经当地的专家检查和评价,专家组由妇产
科专家、助产教师、两名在性健康方面经验丰富的心
理学家、两名临床助产师组成。由两名既精通汉语又
精通英语的护士将中文版的量表回译为英文。

。用翻译过来的中文量表对五名有
非计划妊娠史的妇女进行调查,五名妇女对量表条目
的意义能够理解。在其研究中,中文版避孕自我效能

受到个体的背景、性与避孕知识等个人特点的影响,
但是它主要是由个体对自己性活动的态度组成。

研究时指出,在对量表的得分进行统计分析时,应单
独分析每一个条目与避孕行为的关系,而不是单纯地
用总分来分析。
Heinrich

的研究证明避孕自我效能
与避孕应用之间有相关性,避孕自我效能量表是最重
要的避孕应用预测工具。这些研究表明,避孕自我效
能量表适用于测量不同群体青年女性的避孕行为
[15]
。
5.
避孕自我效能对避孕行为的影响
避孕自我效能是影响避孕行为的重要因素。
Bandura
指出,个体感知的对性和避孕的控制能力及自
我效能越低,发生危险性行为的可能性越大
[3]
。
Levinson
发现自我效能期望对女性避孕应用有重要的
作用
[5]
,自我效能能够促进个体的避孕行为变化向维
持阶段发展
[17]
。自我效能对青少年的危险性行为有直
接的影响作用,自我效能水平越高,发生危险性行为
的可能性越小
[20]
。
Heinrich
[18]
指出避孕自我效能是有性
生活的大学女生是否采取避孕行动的重要预测因素。

证据表明,个体对自我效能的感知和应用避孕套
的自我效能与避孕套的应用有很大的相关性

。

等的研究指出,青少年对自己与伴侣协商应
用避孕套的自我效能对他们的安全性行为有预测作
用

。

研究发现,在性生活中应用避孕措施的人
有高的自我效能

。较高的避孕自我效能能够促进青
少女在性生活中应用避孕套
[6]
。对自己的性行为有控
制信念的青少女即便在一些不利的环境中也能积极
地应用避孕措施,缺乏避孕信念的青少女则不会坚持
避孕行为

应用避孕套的结构方程模型,在该模型中,购买避孕
套的自我效能影响了避孕套的购买,劝说性伴侣应用

用路径分析的方法测试了避孕套应用知 识、
避孕套应用应对及自我效能之间的关系,研究发现,
避孕套应用的知识与自我效能、自我效能与避孕套应
用有直接正相关关系,避孕套应用的知识通过自我效
能的中介作用,间接影响避孕套应用行为。
6.
避孕自我效能的影响因素
6.1.
避孕知识
影响自我效能的因素有四个,分别是直接经验、
替代经验、言语劝说、生理情绪状态
[10]
。知识作为替
代经验,能够影响自我效能水平的高低。自我效能是
联系知识和行为的重要中介
[10]
,是从知识转变为行为
的一个关键因素
[25]
。但仅仅增加妇女的避孕知识,不
足以引起避孕行为的改变
[26]

认为预防的知
识必须与感知到的对性情境和技巧的控制能力

避孕
自我效能
)
相结合,才能引起行为的改变。
避孕知识与避孕自我效能的相关研究证明了两
者之间有正相关关系,避孕知识的增加能够提高避孕
自我效能。

等在研究中也发现,生殖避孕
知识与避孕自我效能之间有正相关关系,生殖避孕知
识与避孕自我效能的三个亚量表有明显的正相关关
系。
Sangi-Haghpeykar

在其对美国休斯敦

性生活、低收入、未怀孕的女性调查发现,西班牙女
性和白人女性比起来,在对口服避孕药认识上更少一
些,而且这个结果不受年龄、教育和婚姻状态的影响。
同时,研究者发现,西班牙女性比白人女性避孕自我
效能的水平低很多。

可以减少青少年怀孕的可能性。
然而,有些研究发现,知识的增加并不能提高自
我效能。

对香港非计划妊娠女性避孕知识与避孕
自我效能的研究中也指出两者的相关性没有那么明
显。作者分析,根据
Bandura
的自我效能理论,态度
和行为不是知识的直接结果,而人们结合知识、结果
期待、情感状态、社会影响、过去经历形成的对自己
应对能力的评价会影响态度和行为。此外,避孕知识
的量表不是针对具体避孕工具的知识,这也是该研究
中避孕知识与避孕自我效能没有显著相关性的原因。
6.2.
避孕态度
自我效能与态度具有密切相关,积极的态度也会
促进自我效能水平的提高。

研究发现,避孕
套应用的意愿与对
HIV
易感性的感知、使用避孕套自
我效能有相关性。与不应用避孕套的女性比起来,应
用避孕套的女性明显有高水平的态度和高自我效能。

发现,自我效能的期望对女性避孕工具的应
用有重要的促进作用

。对避孕有积极态度的妇女也
对紧急避孕药的使用有非常高的自我效能
[30]
。

等指出,购买避孕套的态度与劝说性伴侣应
用避孕套有显著相关性,即购买避孕套的态度越积极,
其劝说性伴侣应用避孕套的自我效能越高,对避孕套
的购买态度与避孕套购买的自我效能有正相关关系

因素,对避孕套的积极态度是女性决定应用避孕套的

着他们持续应用避孕套的行为。
6.3.
影响避孕自我效能的其他因素
研究表明,女性比男性有更高避孕自我效能

避孕自我效能随着年龄的增加而增加

,单身妇女的
避孕自我效能水平比同居、结婚、离异的妇女自我效
能水平低

。低教育水平与低自我效能相关

。社会
地位影响了个体对自我效能的感知,高自我效能与高
的社会地位有正相关

。但目前,对避孕自我效能的
影响因素研究还并不全面,各个影响因素作用大小方
面的研究也相对欠缺,因此避孕自我效能的影响因素
还需进一步探索。
7.
小结
避孕自我效能是影响女性避孕行为的重要因素。
大量研究证明,避孕自我效能反映了女性在性行为中
的控制避孕情景以达到避孕保护的信念强度,对避孕
行为有积极的促进作用。国外关于这方面的研究有大
量的证据,国内关于避孕自我效能的研究主要在香港
地区,内地几乎仍处于空白状态。笔者建议护理人员
及健康照顾者在对女性进行性健康宣教的过程中,应
该评估女性的避孕自我效能水平,采取相应的措施提
高其避孕自我效能水平,从而提高其在避孕中的信念
及控制能力。中国女性的避孕自我效能水平如何,不
同避孕行为水平的妇女其避孕自我效能有何差别,如
何采取措施提高女性的避孕自我效能水平,如何通过
护理干预提高女性的避孕自我效能,改善其避孕行为
是将来的研究重点。
