The Relationship between CD14 and Microglia in Peripheral Blood Mononuclear Cells in Patients with Alzheimer’s Disease

ABSTRACT

Alzheimer’s disease is a neurodegenerative disease common in older people. Alzheimer’s disease pathogenesis is not very clear; the research found that the inflammation of amyloid deposition caused by activation of glial cells is the core pathogenesis. Recent studies show that the CD14 signal transduction pathway and gene polymorphism in microglia activation plays a role in this paper research in recent years. The relationship between CD14 cells and MG in Alzheimer’s patients in peripheral blood mononuclear cells was reviewed.

摘 要

阿尔茨海默病(Alzheimer’s disease, AD)是老年人常见的一种神经退行性疾病。AD发病机制还不是很清楚,研究发现淀粉样蛋白沉积激活神经胶质细胞所导致的炎症反应是其核心病理机制。近期研究表明CD14信号传导通路及其基因多态性在小胶质细胞(Microglia, MG)激活过程中发挥着一定的作用。本文主要就近年来阿尔茨海默病人外周血单核细胞(peripheral blood mononuclear cell, PBMC)中CD14细胞与MG之间关系的研究进行综述。

1. 引言

随着老龄化人口进程的不断加速,阿尔茨海默病(Alzheimer disease)以情景记忆障碍为早期突出的表现,逐渐出现认知功能全面衰退、精神和行为症状的老年常见神经退行性疾病 [1] 。AD主要病理特征为神经炎性斑块和过度磷酸化tau蛋白构成的神经原纤维缠结。近年来愈来愈多的研究表明脑内的慢性炎症反应可能是其重要发病机制之一,参与炎性反应的细胞有星形胶质细胞、小胶质细胞、少突胶质细胞等,其中小胶质细胞(Microglia, MG)是其主要的炎症细胞。AD患者脑内β-淀粉样蛋白(β-amyloid, Aβ)的周围存在大量被激活的小胶质细胞,另外在其脑内还发现大量由MG产生的细胞因子和其它免疫介质,共同形成了AD病人脑内的慢性炎症环境,MG依存性脑炎症是AD发病机制的关键 [2] ,Aβ已被公认是MG依存性脑炎症的重要诱导物,被其激活的MG分泌促炎症因子和释放神经毒素,在AD的病程中引起大脑炎症及增强细胞毒性。

2. CD14在AD患者脑内MG激活中的作用

近期研究利用正电子发射断层扫描(position emission tomography, PET)技术可以使激活的小胶质细胞在人类活体大脑内成像,为研究MG以及何时被激活提供了一个新的视角 [3] 。Edison最近研究发现大脑皮层内被激活的MG数目的增多不仅与Aβ淀粉样斑块含量密切相关,而且与逐渐下降的认知能力有关 [4] 。Okello等研究认为在轻度认知缺损的病人认知能力的降低与MG的激活有关 [5] 。AD小鼠模型使用高灵敏度的放射性配体进行分析后显示MG的激活与Aβ的沉积有极大的相关性 [6] 。以上研究结果可能与实验中使用的放射性配体的灵敏度有关,灵敏度越高分析得到的结果可能就越准确 [7] 。

白细胞分化抗原14 (CD14)是一种特异性单核细胞和巨噬细胞表面标记物,在机体免疫、防御系统引起的一系列病理反应中起关键作用。CD14为革兰阴性细菌细胞壁成分脂多糖(LPS)的受体,是体内一种促炎细胞因子,属细胞表面糖蛋白家族成员之一,主要由成熟的单核巨噬细胞所产生,CD14的主要功能是在LPS结合蛋白的协助下结合LPS并引起细胞活化,活化的单核巨噬细胞可释放前炎症反应性细胞因子TNF-α,IL-1,IL-6等,参与炎症反应导致组织器官损伤。CD14基因位于第5号染色体5q23-31区,全长约为3900 bp,编码含有375个氨基酸的糖蛋白,分子质量为55000。在其基因启动子区上游-159 bp处存在C-159T基因多态性 [8] ,这种基因多态性可影响CD14基因转录和表达,进而影响到CD14的生物学功能,从而影响一些相关疾病的发生、发展和转归 [9] 。

目前研究表明CD14信号传导通路及其基因多态性在小胶质细胞(Microglia, MG)激活过程以及Aβ清除过程中发挥着一定的作用,CD14信号传导通路及其基因多态性成为AD的研究热点 [10] 。近年单核细胞CD14被认为参与了MG对fAβ (fibrillar Aβ, fAβ) (也可能是寡聚体)的反应。这个重要发现证实了fAβ确实是MG的免疫刺激物,可以通过Toll样受体(Toll-like receptor, TLR)激活小胶质细胞TLR在功能上可以与其它细胞表面受体相互作用,包括CD36、α6β1整联蛋白、CD47和清道夫受体A,fAβ与这些受体结合后激活了MG内信号转导途径,进而触发细胞内的多个并联的信号级联放大反应 [11] 。这是fAβ能够诱发MG产生和释放炎症(Th1-type)细胞因子以及活性氧和氮自由基的机制之一。近期研究发现TLR辅助受体CD14和Aβ之间来相互作用可以通过fAβ和CD14重组体共同孵育、实时表面等离子共振光谱法、荧光寿命显微成像技术基础上的荧光共振能量转移技术(FLIM-based FRET)以及抗体封闭等技术 [12] 。无论是AD动物模型还是AD患者,脑中CD14、TLR2和TLR4的表达都会增高 [12] 。结果提示TLR参与了AD脑内MG表型转化和反应的调节作用。

MG表面除含有CD14固有免疫受体外,还有晚期糖化终产物受体(receptor of advanced glycationendo production, RAGE)、清道夫受体(scavenger receptor, SR)和丝氨酸蛋白酶抑制剂酶复合物受(SEC-R),在体外及动物实验已经证实它们可通过Aβ42介导MG的活化 [13] [14] 。研究报道Aβ42诱导的大鼠MG的CD14激活途径是神经毒性产物产生并最终导致神经元死亡的重要原因 [15] 。近期,Liu等 [16] 用多种方法证实转基因鼠Aβ42诱导MG表达CD14,并对AD患者的额叶和枕叶皮质及海马进行免疫组化法检测,发现CD14在AD患者脑内的MG中过度表达。Fassbender等 [10] 发现在CD14受体功能缺失的APP转基因大鼠脑内Aβ42沉积物的增加,MG的CD14受体对Aβ42的吞噬作用明显减少。Reed-Geaghan等 [17] 研究认为CD14缺失的转基因小鼠在7个月大时小鼠脑内不溶性的散发的硫磺素-S染色阳性的蛋白斑的大小和数量均降低,这可能与活化的MG的数量总体下降有关。上述研究提示CD14在Aβ沉积导致小胶质细胞激活的过程中发挥了重要的作用。

3. CD14在MG清除Aβ中的作用

CD14与fAβ42在小胶质细胞表面有着共同的结构域,fAβ与小胶质细胞表面的CD14结合后进入到细胞内,然后被转运到溶酶体参与了小胶质细胞吞噬Aβ的过程。 [11] 许多研究采用AD动物模型来评价TLR2、TLR4和CD14在Aβ清除中的作用。APPswe/PSEN1dE9双转基因AD小鼠模型具有(8888TLR2, TLR4)基因的特点,它可以促进宿主细胞表面CD14和髓系分化的蛋白质(MD)-2相互结合 [16] ,而这对下游信号分子活化起着关键作用。Tahara等利用这种AD小鼠模型研究活体内TLR4在淀粉样病变中的作用 [18] ,发现与有完整TLR4基因的小鼠相比TLR4缺失的APPswe/PSEN1dE9小鼠大脑皮层和海马内Aβ含量要高。这些研究认为MG介导的Aβ清除机制有赖于TLR4功能的完整性。相反地,APPswe/P SEN1 dE9小鼠与TLR2-/-小鼠交配后出生的TLR2-/-转基因小鼠在出生后6个月内显示Aβ沉积延迟。这是否是由于TLR基因、CD14信号传导通路对Aβ的清除作用导致的还不是很清楚。Hickman等 [19] 发现8个月APPswe/PSEN1dE9小鼠各种不同Aβ结合受体的表达和各种降解酶都减少,同时炎症因子的基因表达明显增高。

在AD的发病及进展中,MG的作用有两面性。一方面Aβ激活的MG过量产生多种促炎因子如TNF-α,IL-1β,IL-6及增加Aβ生成导致神经元损伤、另一方面MG反应性产生部分抗炎因子参与抑制MG依存性脑炎症。AD早期MG能够有效地清除形成的Aβ,但在AD晚期,由于Aβ的不断累积MG参与Aβ清除机制的基因下调 [20] 。本组最新研究发现,慢性缺氧可上调MG线粒体内ROS及DNA损伤,诱发其产生IL-1β等促炎因子导致MG活化依存性脑炎症 [21] 。Herber等 [22] 的研究发现向AD动物模型的海马内注射TLR4的配体LPS,可以清除散发的而不是紧密分布的Aβ,而这个过程中伴随着CD14促进MG的激活。另外外周注射LPS可以激活大脑皮层的MG并且导致Aβ含量的降低,也为TLR、CD14有助于Aβ清除提供了依据 [23] 。近期研究表明AD发病是由于Aβ清除受损导致的,而与Aβ的生成增多无关 [24] 。导致Aβ清除减弱的原因是与年龄相关的MG功能障碍,这种功能障碍并不意味着细胞活性的降低,而是指活化后表型的转变 [25] 。有研究发现炎症细胞因子能够抑制MG的吞噬功能,但是否是因为CD14在MG吞噬作用被损坏仍有待于进一步研究 [26] 。

4. 结语

近年来神经病理学、临床流行病学及动物模型等多项研究证实神经炎症机制与AD的发生发展逐步成为本领域的研究热点。CD14在MG的激活和MG对Aβ的清除方面均发挥着重要作用,MG介导的炎性反应在AD发病病程中起双刃剑作用,活化的MG一方面会导致细胞因子和神经毒性介质的释放增加从而使疾病恶化,同时也可以通过增强对Aβ的吞噬及蛋白降解功能发挥神经保护作用。综上所述,通过CD14细胞与MG之间关系及其在AD发病中的作用的研究,为AD的诊治提供了新的思路。同时CD14单核细胞在感染和炎症中均起到重要作用,但其在其他疾病中的发挥何种作用以及如何更合理的论证并诠释与AD之间的关系,也是我们今后所要研究的方向。
