Study on TSPO and Ligand in the Psychiatry and Neurology Disease

ABSTRACT

TSPO is the peripheral tissue binding site of benzodiazepine drugs, also known as peripheral benzodiazepine receptor (PBR). With the further research, the first subunit of PBR (i.e. the isoquinoline binding site of diazepam binding inhibitor located in the outer membrane of mitochondria) was renamed as 18kDa translocation protein. The important role of TSPO and its ligands in diseases has become a hotspot in recent years. Studies have shown that TSPO has a wide range of physiological regulation functions, and is involved in the occurrence and development of stroke, neurodegenerative diseases, neuroimmune diseases, neuropsychiatric diseases and other diseases. As biomarkers and treatment targets, its ligands play an important role in imaging and inflammation monitoring. However, the precise role of TSPO in neurological disorders remains a mystery. This article will review the latest research on the role of TSPO and its ligands in the central nervous system.

摘 要

转位蛋白(translocator protein, TSPO)是苯二氮卓类药物外周组织结合位点,又称外周型苯二氮卓受体(peripheral benzodiazepine receptor, PBR)。随着研究的深入,将PBR第一亚单位(即位于线粒体外膜的异喹啉结合点地西泮结合抑制物)重新命名为18kDa转位蛋白。TSPO及其配体在疾病中的重要作用成为近几年研究的热点。研究显示TSPO具有广泛的生理调节功能,参与了卒中、神经退行性病变、神经免疫病及神经精神疾病等多种疾病的发生发展,其配体作为生物标记物和治疗目标,在影像成像及炎症在体监测中扮演着重要的角色。然而,TSPO在神经系统疾病中精确作用仍然是个迷。本文将对TSPO及其配体在中枢神经系统中作用的最新研究进行综述。

1. 引言

TSPO是一种保守的蛋白质,主要存在于细胞线粒体外膜,它参与许多细胞调节过程,包括炎症反应、氧化应激和线粒体内稳态、胆固醇转运、类固醇激素合成、线粒体膜电位及呼吸功能的调控。除在周围组织中广泛的表达外,TSPO在激活的小神经胶质细胞中高度表达。TSPO配体作为生物标记物和治疗目标,在脑血管疾病、多发性硬化、肌萎缩侧索硬化症、阿尔茨海默症等神经系统疾病中扮演着重要的角色。然而,这种蛋白质的生理和神经病理的精确作用仍然是个迷。这篇综述概述了我们对这个分子多方面理解的最新进展,为临床进一步研究神经系统疾病的靶向药物提供新的思路。

2. TSPO结构与功能

2.1. TSPO分子结构及分布

TSPO是由5个异喹啉结合蛋白组成的疏水性蛋白质,包含169个氨基酸(富含色氨酸)和五个跨膜区域 [1] ,其广泛表达于全身各个系统,在睾丸、脂肪组织和肾上腺皮质等甾体合成组织中表达最多 [2] ,在中枢神经系统中,TSPO主要分布在胶质细胞线粒体外膜。位于线粒体外膜的电压依赖性阴离子通道(voltage-dependent anion channel, VDAC)和腺嘌呤核苷酸转移酶(adenine nucleotide transporter, ANT)共同组成线粒体多蛋白复合物,即线粒体通透转换孔(mitochondria l permeability transition pore, MPTP),TSPO参与其复合物组成 [3] ,并调控多种线粒体功能。

2.2. TSPO生理功能

TSPO生理功能与线粒体密切相关,包括炎症反应、氧化应激、甾体合成、细胞凋亡、线粒体膜电位及呼吸功能的调控等。研究显示,TSPO基因敲除能显著改变线粒体内稳态,并影响了线粒体多个功能 [4] 。线粒体膜电位改变是引起凋亡级联反应的启动因素,TSPO参与线粒体膜电位改变及呼吸功能的调控,进而可能引起细胞凋亡。最近研究显示,TSPO基因敲除后线粒体膜电位发生改变,促进了线粒体过氧化作用、引起活性氧生成增多,进一步导致细胞凋亡 [5] 。正常情况下,TSPO在神经免疫细胞中表达很低,当机体处于应激状态时,TSPO表达明显升高,其升高与胶质细胞激活呈正相关,能反映中枢神经系统炎症状态 [6] [7] 。因此,TSPO可被看作是神经炎症的分子标志物。研究显示,TSPO配体PK11195、Ro5-4846可降低促炎因子水平 [8] 。此外,TSPO是胆固醇转运的必要条件 [9] ,神经类固醇作为一种神经递质可维持大脑正常生理功能。目前认为,中枢神经系统抑制性神经递质受体γ-氨基丁酸A型(γ-aminobutyric acid A, GABAA)受体的调节与TSPO密切相关,具有镇静、催眠、抗焦虑和抗惊厥作用 [10] 。综上所述,TSPO已成为神经精神性疾病精准诊断和治疗的靶点蛋白。

3. TSPO:未来神经精神疾病治疗的新靶点

3.1. TSPO与神经退行性疾病

既往研究显示,胶质细胞介导的慢性炎症反应是神经退行性疾病早起病理改变机制之一 [11] 。小胶质细胞与星形胶质细胞激活后促进炎症因子生成、释放,而TSPO可以调控该过程中炎症因子的释放 [12] ,提示TSPO可能参与神经炎症反应的发生。在肌萎缩侧索硬化症、阿尔茨海默症、帕金森病等神经退行性病变中均可以观察到TSPO表达升高,因此,减少TSPO表达,抑制神经炎症反应,有利于延缓疾病进程。Veiga等 [13] 研究表明,TSPO配体Ro5-4864能减少大鼠海马神经元损伤,抑制胶质细胞激活,可显著逆转阿尔茨海默症病理损害。在外源性神经毒性药物诱导神经损伤动物模型中,观察到小鼠海马炎症因子升高,并伴有不同程度的认知功能减退,使用TSPO配体DAP-713可改善这一现象 [14] 。最近一项研究显示,在MPTP诱导的帕金森动物模型中,TSPO合成配体Emapunil (AC-5216, XBD-173)可改善多巴胺神经元变性,并减少促炎因子生成,II期临床试验显示出Emapunil可穿过血脑屏障并且安全、耐受性良好。因此,TSPO配体Emapunil可能成为治疗帕金森病的一种有前途药物 [15] 。在肌萎缩侧索硬化模型中,TSPO配体Olesoxime可以通过改善运动神经元损伤,减缓病情加重,但临床试验显示病情没有缓解 [16] 。关于TSPO在神经退行性疾病中具体作用目前尚不清楚,仍需我们进一步探索。

TSPO与神经炎症反应密切相关,在多种中枢神经系统疾病中表达上调,TSPO作为神经炎症分子标志物用于疾病诊断是目前研究的热点。将标有放射性示踪剂的TSPO配体作为神经成像剂用于神经退行性疾病的诊断。通过正电子发射扫描(PET)或单光子发射计算机断层扫描(SPECT)可检测并量化大脑病变部位、范围及严重程度,从而能及时实施有效的干预手段 [17] 。

3.2. TSPO与焦虑抑郁症

TSPO是胆固醇从线粒体外膜转移到内膜的必要条件,也是类固醇生成的限速步骤,神经类固醇作为一种神经递质参与调节情绪及应激反应,具有抗焦虑抑郁样作用 [18] 。研究表明,TSPO可通过促进脑内四氢孕酮合成来改善焦虑状态,小鼠在阻断四氢孕酮合成后产生焦虑样行为,应用TSPO配体可显著逆转动物抑郁样行为。这表明TSPO配体具有显著的抗焦虑、抑郁作用 [19] 。TSPO配体依替福辛是一种临床批准治疗焦虑症的药物,有研究表明,其抗焦虑作用可能通过神经类固醇间接作用于GABA受体而产生 [20] ,而TSPO另一种配体XBD173也具有明显的抗焦虑作用,并且与苯二氮卓类药物相比,不产生镇静、肌松和戒断症状等不良反应 [21] 。神经类固醇结合位点与苯二氮卓类药物不同,这可能是避免产生一些不良反应的原因。同样在创伤后应激障碍(post traumatic stress disorder, PTSD)模型中,TSPO配体YL-IPA08表现出明显抗焦虑抑郁作用,且不引起镇静催眠、肌松及认知功能损害等不良反应 [22] 。关于TSPO配体是否直接作用于TSPO有待于我们进一步研究。

综上所述,TSPO可能通过调控类固醇合成、激活GABAA受体等作用改善焦虑抑郁状态,深入研究其具体机制将为焦虑抑郁的治疗提供新靶点。

3.3. TSPO与脑损伤

脑外伤后,TSPO表达明显升高,其升高程度与大脑损伤程度呈正相关,且与损伤后神经炎症反应及胶质细胞激活状态一致。实验性脑损伤动物模型中,随着损伤时间不同,TSPO表达量发生变化,表现为升高后降低趋势,在损伤后24小时达高峰。最新研究显示,与健康对照组相比,创伤性脑损伤患者血清TSPO浓度与GCS评分、血清IL-6、TNF-a、CRP浓度变化一致。血清TSPO被认为是上述预后变量的独立预测因子,其预测能力与GCS评分相似,且显著提高了GCS评分预后判断能力 [23] 。这表明TSPO可能与脑损伤后炎症反应、疾病进展和不良预后密切相关,TSPO配体也被作神经显像剂来评估脑损伤程度和预后。

在治疗方面,TSPO一些特殊配体具有损伤后神经修复功能。其具有机制可能与抑制免疫炎症反应、调控线粒体功能及细胞凋亡有关。在实验性脑损伤模型,给予Ro5-4864不仅能抑制胶质细胞激活,减少肿瘤坏死因子α、白介素1β等促炎因子水平,还能恢复线粒体膜内外电位水平,避免线粒体功能紊乱,抑制凋亡因子活化,明显增加神经元存活数目 [24] 。此外,在实验性脊髓损伤模型,TSPO另一种配体ZBD-2参与神经细胞修复过程,并减少脊髓损伤引起的疼痛症状及焦虑抑郁样行为,其可能的机制是抑制胶质细胞活化和修复突触可塑性相关信号系统 [25] 。

综上,TSPO及其配体不仅可以评估脑损伤程度及预后,还脑损伤功能修复过程,深入研究其具体机制将为脑保护作用提供新的靶点。

3.4. TSPO与胶质瘤

一些脑肿瘤具有高度侵袭性、破坏性极强,如胶质母细胞瘤,目前治疗手段有限,大多数患者易复发。脑肿瘤的检查与治疗仍有很大挑战性,需要寻找一种新的监测与治疗手段。TSPO作为肿瘤诊断的生物学标志物是目前研究的热点。在某些脑肿瘤中可以观察到TSPO高度表达,且表达量与肿瘤分级呈正相关。最新研究表明,应用TSPO配体[18F]DPA-714作为靶向示踪剂,通过PET成像可早期预测胶质瘤生长及浸润情况 [26] 。

TSPO作为胶质细胞线粒体外膜重要的组成部分,与VDAC关系密切,共同调控线粒体凋亡途径。有研究显示,使用TSPO配体PK 11195或敲除TSPO基因均能显著抑制由氯化钴诱导人胶质母细胞凋亡,其可能通过改变线粒体膜电位、增加ROS生成、促进MPTP开放来激活线粒体凋亡通路 [27] 。因此,TSPO不仅可作为胶质瘤病理分级、浸润范围及判断预后的指标,还为未来胶质瘤靶向治疗提供新的思路。

除上述疾病外,我们发现TSPO与其他中枢神经系统有关,如神经免疫病、卒中、神经病理性疼痛。Daughety等 [28] 利用实验性自身免疫性脑脊髓炎模型,发现Etifoxine不仅能减少外周免疫细胞入侵至脊髓,而且促进少突胶质细胞再生,从而改善疾病损伤。炎症是脑出血后继发性损伤的重要原因,直接、无创地监测脑出血后炎症反应,将有助于早期抗炎治疗。脑出血后,伴随白细胞浸润和小胶质细胞激活,TSPO表达迅速增加。Abid K.A. [29] 等研究发现,利用TSPO配体PK11195进行PET检查,并结合动态增强MRI可实现急性脑出血后炎症反应的在体监测。此外,TSPO配体可减轻脑出血后的脑损伤和炎症反应。近年有研究显示TSPO参与炎症疼痛的调节,认为TSPO会是治疗神经性疼痛的新靶点。钩吻素子在神经病理性疼痛模型中具有显著的抗炎镇痛作用,而TSPO拮抗剂PK11195抑制其镇痛作用 [7] 。

4. 总结

TSPO与中枢神经系统疾病密切相关,其配体神经保护机制迄今尚未完全阐明。除了作为标志物用于疾病诊断,在调控线粒体功能、细胞炎症及氧化反应方面发挥的作用需要更多研究证实。尽管某些治疗神经精神疾病的TSPO配体药物在I期临床试验显示出安全性和有效性,但其长期疗效和副作用仍需进一步研究。
