Ehlers-Danlos Syndrome with B4GALT7 Mutation: A Case Report and Literature Review

ABSTRACT

Ehlers-Danlos syndrome is a complex hereditary connective tissue disease. It is caused by multiple gene mutations which result in the dysfunction of Mucopolysaccharide synthesis and affect the function of extracellular matrix. It is characterized by joint hypermobility, skin hyperextension and tissue fragility. Among all subtypes of Ehlers-Danlos syndrome, one type caused by B4GALT7 gene mutation can also manifest short stature, abnormalities of forearm bone and joint, curved limbs and so on. So far, there are only 8 cases reported worldwide on this type of Ehlers-Danlos syndrome. In this paper, we reported a new case of B4GALT7 gene mutation patient diagnosed in China and two new amino acid mutations p.(Arg141Gln) and p.(Arg234His) were found by high-throughput sequencing technology. Further, we do a literature review, compared all the clinical phenotype and genotype of previously reported cases and this case, clarify the common manifestations, diagnostic criteria and pathogenic genes of this type of Ehlers-Danlos syndrome. Also, we compared the different clinical manifestations caused by different sites’ mutations of the B4GALT7 gene, tried to explain the effects of different sites’ mutations on protein function. All above can help clinicians to improve their understanding on this rare disease, increase the diagnosis rate, and improve the prognosis of this kind of patients as much as possible.

B4GALT7基因变异型Ehlers-Danlos综合征一例及文献复习

摘 要

Ehlers-Danlos综合征是一类复杂的遗传性结缔组织疾病,是由多个基因突变引起的粘多糖合成障碍,影响细胞外基质的功能,以关节活动度过大、皮肤松弛和组织脆性为显著特征。其中B4GALT7基因突变引起的Ehlers-Danlos患者还可表现出身材矮小、前臂骨骼和关节的异常、弯曲的四肢等,这一型患者目前报道的仅有8例。本文报道一例国内确诊的B4GALT7基因突变患者,运用二代测序技术发现两个新的氨基酸突变p.(Arg141Gln)和p.(Arg234His)。同时进行文献回顾,与之前报道病例的临床表型和基因型进行比较,明确该型Ehlers-Danlos综合征常见的共同特征、诊断标准、致病基因,进一步比较不同突变位点引起的临床表现,尝试对B4GALT7基因不同位点突变对蛋白质功能的影响进行解释,帮助临床医生提高对这一罕见疾病的认识,提高诊断率,改善患者的预后。

1. 引言

Ehlers-Danlos综合征(Ehlers-Danlos syndrome, EDS)又称先天性结缔组织发育不全综合征,是由Ehlers和Danlos分别于1901年和1908年提出,是一类复杂的遗传性结缔组织疾病,以关节活动度过大、皮肤松弛和组织脆性为显著特征 [1] ,发病率尚不清楚。因其临床表现和遗传方式的差异很大,自1978年至今EDS经历了多种分型和分类方式 [2] [3] [4] ,B4GALT7基因突变型EDS曾被命名为早衰型EDS或EDS伴身材矮小和肢体异常 [5] [6] 。2017年新的国际分类将EDS分为13个亚型 [1] ,不再提出早衰型EDS,将B4GALT7基因变异型归入第10个亚型为脊椎变异型EDS (Spondylodysplastic EDS, spEDS),以spEDS-B4GALT7命名。至今仅有来自6篇文献的8个患者确诊为spEDS-B4GALT7,患者除了EDS的典型表现外均表现出身材矮小、肌张力降低、尺桡骨融合和中到重度智力缺陷 [5] - [10] 。现将2016年8月我院生长发育门诊确诊的一例spEDS-B4GALT7缺失型患儿报道如下,并进行相关文献复习,以期提高临床医生对该病的认识和诊断率,改善患者的预后。

2. 病历资料

患儿,女,第一胎第一产,孕周37+剖宫产出生,出身体重2.25 kg,出生身长42 cm。1岁20天时因“生长缓慢”至我院生长发育门诊就诊,体格检查:身高68.2 cm (相当于同年龄、同性别儿童身高的−2.88SD),体重7.0 kg (相当于同年龄、同性别儿童身高的−2.97SD),头围42.5 cm,双足轻度外翻,其余部位未见明显异常。患儿父亲身高171 cm,母亲155 cm,排除家族性身材矮小。给与营养干预治疗,观察生长速度,定期随访。患儿3岁时复诊,身高82.7 cm (−3.68SD),实验室检查:甲状腺功能、生长激素、类胰岛素样生长因子、生化全项均正常,排除明显的影响生长发育的疾病。

患儿4岁1个月时再次复诊,体格检查:身高88.2 cm (−4.03SD),体重11.2 kg (−3.88SD),三角脸、扁平脸、眼球突出,前额宽,鼻梁塌陷、小下颌,右前臂轻度弯曲、活动受限,双足外翻,肌张力轻度低下,皮肤柔软苍白(因涉及患者隐私未提供照片)。追问病史得知,患儿患有“先天性青光眼”,与生后16天行青光眼手术治疗,现恢复良好。右臂“先天性尺桡骨融合”未行矫正手术。患儿家长拒绝进行全身骨骼的X光检查。因患儿存在多部位的异常及明显的身材矮小,考虑存在基因异常综合征的可能,建议患者进行矮小症相关基因检测。二代测序结果检出患儿存在B4GALT7基因两个位点的突变,对其父母进行一代测序验证,患儿的两个突变位点分别来自父亲和母亲,详细信息见表1图1。基因检测结果显示该患儿符合EDS伴矮小和肢体异常的诊断,据2017年对EDS的最新国际分型,应诊断为spEDS-B4GALT7。

3. 文献检索

以“Ehlers-Danlos syndrome”和“B4GALT7”为关键词搜索Pubmed数据库,以“Ehlers-Danlos综合征”和“B4GALT7”搜索CNKI数据库和万方医学数据库,Pubmed数据库显示9篇相关文献,CNKI和万方数据库中未检出相关文献。进一步筛选9篇英文文献,其中共6篇文献中8个病例报道 [5] - [10] 为B4GALT7基因突变型的EDS患者,其余为动物或组织试验。8个国外病例及本文病例详细信息见表2,患者的诊断年龄在2岁~33岁之间,所有病例均表现出身材矮小(身高标准差分值在−3.0~−6.5之间)。临床表型见表3,9例患者均有身材矮小、关节活动度大、皮肤弹性高,8例有肢体弯曲和典型面部特征,

7例出现马蹄足、内翻足、尺桡骨融合、肌张力减退,6例伤口愈合延迟、发育延迟、牙齿发育不良和通贯掌,5例眼科检查异常,4例有骨密度降低,3例有脊柱侧凸、后凸、指尖膨隆,这些被认为是B4GALT7基因缺陷型EDS的常见特征 [11] ,不同患者临床表型的严重程度存在差异。按照最新的EDS分型,均应诊断为spEDS-B4GALT7型。患者均不存在完全性生长激素缺乏(生长激素激发试验峰值在5~10 ng/ml之间),仅Guo等人报道的男性病例在5.5岁至10岁之间接受了重组人生长激素治疗,生长速度和生长水平均没有明显改善 [9] 。

4. 讨论

4.1. B4GALT基因突变型EDS的临床归类和命名

B4GALT7突变型EDS在2017年之前被称为早衰变异型EDS,之所以被命名为早衰型是因为最先发现的3例ß4GalT7蛋白(由B4GALT7基因编码产生)缺陷病例存在轻度老化外观 [5] [6] ,与Hernandea等人定义的早衰变异型EDS表现相似 [2] [12] ,但在之后确诊的几例B4GALT基因突变患者中并未证实该蛋白缺陷与早衰有关 [9] [13] ,这一命名可能无法代表本病的特征。而且,近年来随着二代测序技术的发展,很多与EDS相关的新的基因变异被发现,需要更好地定义EDS的分类和病因,因此在2017年Malfait等人发表了新的EDS国际分类,根据临床表型将其分为13个亚型,每个亚型都给出了相关的致病基因及临床诊断的主要标准和次要标准 [1] 。其中取消了早衰型EDS的诊断,将B4GALT7突变型EDS归入spEDS型,命名为spEDS-B4GALT7。

4.2. SpEDS和spEDS-B4GALT7的诊断标准

SpEDS作为EDS的一个新的分型,也属一类罕见的常染色体隐性遗传结缔组织疾病,发病率尚不清楚,包含了3个致病基因B4GALT7、B3GALT6和SLC39A13突变相关的EDS,这三个基因型的EDS临床表型多有重叠,因此统一归类为spEDS [1] 。它们分别编码蛋白ß4GalT7、ß3GalT6和ZIP13,均与粘多糖合成和结缔组织的结构和功能相关,患者除了结缔组织损伤的EDS典型特征外还有身材矮小、肢体弯曲、尺桡骨融合等表现。spEDS的诊断标准见表3,诊断至少需要主要标准的身材矮小和肌张力减退外加至少3个次要标准和特征性的X片异常,分子诊断是确诊spEDS的必要条件。spEDS的分子基础是B4GALT7基因的双等位基因突变,或者B3GALT6和SLC39A13基因的双等位基因突变,需用特定基因panel或全外显子测定来确诊。

4.3. 不同基因型与临床表型的关系

不同基因的突变引起的临床表型有差异,如严重的身材矮小、肌张力降低和肢体弯曲更多的出现在spEDS-B4GALT7这一亚型中,扁平椎骨只出现在spEDS-B3GALT6和spEDS-SLC39A13亚型中,在spEDS-B4GALT7中未见报道。即使同为spEDS-B4GALT7患者,突变位点不尽相同,其临床表型严重程度的变异范围也很大,如身材矮小程度从−3.0SD至−6.5SD、智力落后程度不同、有些症状仅在部分患者中出现等(详见表3)。2012年的一项动物试验看到ß4GalT7蛋白在生长板上的水平明显高于心脏、肾脏或肺等组织,而且在小鼠生长板的增生和肥厚区表达水平明显升高,表明B4galt7可能与软骨内骨化相关,解释了spEDS-B4GALT7患者身材矮小的可能原因 [14] 。

4.4. 患者不同突变位点的功能差异是临床表型变异大的主要原因

B4GALT7基因位于5q35.3,编码的ß4GalT7蛋白含有327个氨基酸,6个外显子区,是粘多糖合成过程中的一种关键酶,粘多糖是细胞外基质的组成部分,对结缔组织的结构和功能有重要作用。表2中列出了9例患者B4GALT7基因11个位点的氨基酸突变,其中p.(Arg270Cys)位点的突变最常见,出现了4次,本文患儿的两个突变位点p.(Arg141Gln)和p.(Arg234His)均为新发突变。图2中列出了这11个突变位点所处的外显子区域和不同的功能区,可以看到大多数突变位点位于该基因的催化功能区,仅例4中的p.(Leu41Pro)突变发生在跨膜区,这可能是该患者面部表型不明显的原因。1987年至2013年报道的4例患者的突变位点的功能已通过基础研究得到证实:p.(Leu206Pro)突变通过干扰蛋白质二级和三级结构的形成使酶活性丧失,从而抑制粘多糖的合成;p.(Arg270Cys)突变改变了受体结合位点的结构,减少了粘多糖的细胞外合成;p.(Ala186Asp)突变改变了供体结合位点的结构,导致酶活性减少,影响粘多糖的合成 [15] [16] 。不同基因位点的突变对粘多糖合成、细胞外基质的组成乃至结缔组织功能的影响程度不同,因此患者的临床表型也不同。2016年至今确诊患者的突变位点的功能,尚需更多的基础研究来证实。

4.5. SpEDS-B4GALT7相关疾病的鉴别诊断

Cartault等人在2015年将含有纯合的B4GALT7基因p.(Arg270Cys)错义突变的22个患者诊断为Larsen of Reunion Island综合征(LRS) [13] 。22例LRS患者的临床表现见表4,与9例spEDS-B4GALT7患者相比,也有严重的身材矮小,但LRS患者中没有肌张力减退的数据,肢体弯曲也仅出现在18%的LRS患者中,扁平椎骨在LRS中未见报道。考虑到LRS和spEDS-B4GALT7很多临床表型的重合,Ritelli等人认为不应该把两个疾病看为两个独立的个体,两个疾病都存在p.(Arg270Cys)突变,也证实了这个观点

[10] 。但仍需更多的研究来明确基因型和临床表型的关系,判断LRS是否应该被认为是spEDS表型谱中的一部分。另外,spEDS-B4GALT7还应与其他导致粘多糖合成异常的疾病鉴别。如B3GAT3基因突变导致的Larsen-like综合征,表现为心血管异常、身材矮小、多关节脱位和各种颅面畸形特征。均有先天性肘关节脱位和指端膨隆和关节松弛。但没有皮肤异常的表现,有典型的心脏异常 [17] 。其他基因如CHST3突变致脊柱骨骺发育不良和关节脱位、肘关节发育不良和半脱位、身材矮小。还有CHST14\CHSY1\EXT1等基因突变均有身材矮小的表现,这些基因突变可能分享共同的蛋白多糖合成缺陷的常见分子机制,可能有重叠的表型特征 [18] [19] [20] 。

本文首次报道一例国内的spEDS-B4GALT7患者的诊断经过,同时进行文献回顾,明确spEDS患者常见的共同特征、诊断标准、致病基因。并与之前确诊的8例患者的临床表现和遗传特征进行比较,分析不同基因型和突变位点引起的临床表型差异,尝试对B4GALT7基因不同位点突变对蛋白质功能的影响进行解释,帮助临床医生提高对这一罕见疾病的认识和理解,提高诊断率,改善患者的预后。但因该疾病的报道病例有限,对该疾病的进一步认识还需要更多的研究报道来总结B4GALT7相关的表型,明确表型和基因型的关系,收集更多的自然预后数据,为进一步治疗研究提供更多的循证医学证据。
