Investigation and Analysis of the Characteristics of College Students with Autistic Traits

ABSTRACT

In order to explore the status and characteristics of autistic traits in college students, the Chinese version of the Autism-Spectrum Quotient was used to investigate the status of autistic traits in 956 college students. The survey found that: (1) the status of autistic traits of college students was not optimistic, the positive rate was 4.7%, which was higher than that of the Center for Disease Control and Prevention (1.47%); (2) the autistic traits of college students were different in major, age, sex, family location and parents' socio-economic status; (3) taking high and low autistic traits as dependent variables, binary logistic regression analysis showed that major, age, sex and the socio-economic status of parents can explain 24.5% of the variation of autistic traits, but no variables in this study can significantly predict the high and low autistic traits. The current situation of college students’ autistic characteristics should arouse the attention of college students, parents, schools and all sectors of society, and future research should continue to explore the factors that affect the characteristics of autism, strengthen the research and intervention on the characteristics of autism, in order to improve the quality of life of college students.

大学生自闭特质的现状调查及特征分析

摘 要

为探索大学生自闭特质的现状及其特征,采用中文版自闭症谱系商数问卷对956名大学生进行自闭特质现状调查。调查发现:(1) 大学生自闭特质的状况不容乐观,阳性检出率为4.7%,高于美国疾病控制和预防中心的数据1.47%;(2) 大学生的自闭特质在专业、年龄、性别、家庭所在地、父母社会经济地位上存在不同程度的差异;(3) 以高低自闭特质为因变量,运用二元logistic回归分析发现,专业、年龄、性别、家庭所在地、父母社会经济地位可解释自闭特质24.5%的变异量,但本研究中没有变量能显著预测高低自闭特质。大学生自闭特质的现状应该引起大学生、家长、学校及社会各界的重视,未来的研究应该继续探讨影响自闭特质的因素,加强对自闭特质的研究和干预,以期提高大学生的生活质量。

1. 引言

自闭特质(autistic traits)也被称为更广泛的自闭症表现型(Broader Autism Phenotype,BAP),具有与自闭症谱系障碍(Autism spectrum disorders,ASD)类似的社会交流能力缺陷、共情反应缺陷以及刻板的认知风格 [1] [2]。在一般情况下,BAP的研究对象是ASD个体的家属 [3] [4],而自闭特质是在BAP研究对象的基础上,从ASD个体的家属推向更广泛的人群 [2]。自闭特质连续广泛地存在于典型发育(Typical Development,TD)个体和ASD个体中 [5] [6] [7],只是在严重程度上具有量化的差异 [2]。自闭特质可以采用自闭商数问卷(Autism-Spectrum Quotient,AQ) [8] 进行测量,TD个体处于AQ分数连续谱临界值以下的位置,而ASD个体则处于AQ分数连续谱临界值以上的极端位置 [8] [9] [10]。此外,精神分裂症、面容失认症、神经性厌食症和抑郁症等临床疾病都伴有不同程度的自闭特质 [11]。

双生子研究发现,自闭特质连续分布在每一个普通人的身上,呈中度至高度遗传 [5],受环境和性别的影响 [12]。自闭特质和ASD可能具有重叠的遗传基因和生物学病因 [10] [13] [14],两者都具有极大的稳定性 [12],会伴随个体终生 [15] [16]。因此,自闭特质被称为完全独立于大五人格的人格维度 [17]。目前,对中国大学生的调查发现,自闭特质水平越高的个体越容易出现抑郁、焦虑、自杀等精神问题 [18],这使得关于自闭特质的研究受到越来越多研究者的重视。本研究目的在于探讨大学生自闭特质的现状及其特征,从而提高大学生的生活质量,增加大众对ASD有更好的实质性理解以及推动ASD的干预研究。

2. 研究方法

2.1. 被试

采用方便取样,对重庆师范大学、信阳师范学院、乐山师范学院、遵义医科大学四所大学发放问卷,其中男生182名,女生774名,年龄范围分布在18岁~29岁(包含研究生),平均年龄为20.58岁。共发放问卷1250份,收回1137份,有效问卷956份,有效回收率为84.08%。详见表1

2.2. 研究工具

本研究采用中国台湾学者刘萌蓉 [19] 修订的中文版自闭症谱系商数问卷(the Autism-Spectrum Quotient,AQ),包含社会技能、注意转换、细节注意、交流、想象五个维度,每个维度包含10道题,其中25道正向计分题,25道反向计分题。被试回答“完全同意”或“稍微同意”得1分,回答“完全不同意”或“稍微不同意”得0分,得分范围是0~50分,得分越高表明自闭特质水平越高。Baron-Cohen将32分作为是否患有ASD的临界值 [8],而本研究采用30分作为中国群体的临界值 [19]。

2.3. 数据处理

数据采用SPSS 15.0统计软件进行统计分析。

3. 结果

3.1. 大学生自闭特质的总体特征

956名大学生自闭特质的总体情况不容乐观,虽然AQ总分的平均值低于临界值30分,其中最高得分为37分,最低得分为7分,但是有部分大学生的得分处于临界值之上。各维度得分的平均值都低于6分,但在各维度上也存在得分较高的大学生。详见表2

3.2. 大学生自闭特质的阳性检出率以及具体情况

从956名大学生中检测得出,有45名大学生的AQ总分高于临界值30分,检出率为4.7%,即这45名大学生可能存在ASD,但确切的结果还需要临床医生作进一步诊断,详见表3。对得分高于临界值的45名学生进一步统计分析发现,他们在性别(χ2 = 2.22,p = 0.13)、家庭所在地(χ2 = 2.69,p = 0.29)以及专业(χ2 = 1.13,p = 0.10)上不存在差异,而在年龄上存在显著差异(χ2 = 24.86,p < 0.001),表现为19岁及以下年龄的大学生数量最多,共22名。

3.3. 大学生自闭特质在人口学变量上的差异

大学生的AQ总分、细节注意、交流三个维度在性别上不存在差异,而在社会技能、注意转换、想象三个维度上存在性别差异,具体表现为男生在社会技能、注意转换两个维度上的得分低于女生,而在想象维度上的得分高于女生。家庭所在地为城市的大学生的AQ总分和社会技能维度上的得分显著高于家庭所在地为农村的大学生,而在其他四个维度上不存在显著差异。大学生自闭特质得分在专业上存在显著差异,具体表现为在AQ总分上,文史类的大学生的得分最高,其次是理工类的大学生,最后是其他专业的大学生;在社会技能和注意转换维度上,理工类和文史类的大学生显著高于其他专业的大学生;在交流维度上,文史类的大学生显著高于其他专业的大学生;在想象维度上的得分,文史类的大学生显著高于理工类和其他专业的大学生。详见表4

3.4. 大学生高低自闭特质的情况及影响因素的logistic回归分析

按照前后各10%的标准将大学生分为高自闭特质组和低自闭特质组 [20],将其父亲、母亲工作方向分为高社会经济地位和低社会经济地位两种类型。

父亲社会经济地位低的大学生在AQ总分、社会技能维度、交换维度、想象维度上的得分显著高于父亲社会经济地位高的大学生。母亲社会经济地位低的大学生在AQ总分、社会技能维度、交流维度上的得分显著高于母亲社会经济地位高的大学生。详见表5

以高低自闭特质为因变量,采用二元logistic回归分析发现,专业、年龄、性别、家庭所在地、父母社会经济地位可解释高低自闭特质24.5%的变异量,但本研究中没有变量能显著预测高低自闭特质。详见表6

4. 讨论

4.1. 大学生自闭特质的总体特征

大学生自闭特质的总体情况不容乐观,有部分大学生的得分处于临界值之上。这种情况应该引起大学生的注意。研究发现,运动 [21] [22] 、情绪调节、情绪归因能力可以降低自闭倾向的概率 [23] [24]。因此,大学生、家长、学校以及社会各界应该重视大学生自闭特质的状况,同时,应该鼓励大学生多运动,养成良好的心态。

4.2. 大学生自闭特质的阳性检出率以及具体情况

本研究中,大学生患ASD风险的概率为4.7%,而美国疾病控制和预防中心的数据表明,每68名儿童中可能就有一人患ASD [25],检出率约为1.47%。本研究结果表明,大学生的阳性检出率情况比较严重,应该得到各界人士足够的重视。进一步的分析发现,存在ASD风险的大学生中,19岁及其以下年龄的大学生人数最多,这是因为大学生刚进入大学,存在角色转换困难、生活和学习适应困难等问题。这提醒老师和家长应该多关注大一新生的心理状态,同时学校也应该开展团辅类活动促进大一新生的生活及学习适应,帮助他们完成角色的转换。

4.3. 大学生自闭特质在人口学变量上的差异

4.3.1. 大学生自闭特质的性别差异

大学生的AQ总分、细节注意、交流在性别上不存在差异,而在社会技能、注意转换、想象三个维度上存在性别差异,具体表现为男生在社会技能、注意转换两个维度上的得分低于女生,而在想象维度上的得分高于女生。极端男性脑理论(Extreme Male Brain Theory) [26] 和共情—系统化理论(Empathising Systemising Theory) [27] 认为,男性相比于女性,他们的系统化能力更强,这也是社会技能、注意转换的得分低于女性的原因。研究者认为,女性具有右半球功能优势,在音乐、绘画、语言方面更有天赋,而男性具有左半球功能优势,在分析、推理、计算方面更有天赋 [28]。想象力主要是由右脑分管,这也就解释为什么女性的想象力比男性更好。

4.3.2. 大学生自闭特质的家庭所在地差异

家庭所在地为城市的大学生的AQ总分和社会技能维度上的得分显著高于家庭所在地为农村的大学生,而在其他四个维度上不存在显著差异。其中的原因之一是城市中的大学生从小生活的家庭环境、学校环境以及社会环境,都让他们面临着更大的压力、焦虑。研究者发现,焦虑、抑郁等负性情绪以及不良睡眠质量会增加自闭倾向的概率 [29] [30] [31]。城市里生活的环境相对局限,孩子的生活空间基本以自己家的房子为中心,与隔壁邻居的交流也不如农村生活的小孩。这也造成了家庭所在地为城市的大学生AQ总分和社会技能维度的得分高于家庭所在地为农村的大学生。

4.3.3. 大学生自闭特质的专业差异

大学生自闭特质得分存在专业差异,文史类专业的大学生的AQ总分、交流维度、想象维度得分最高,说明文史类专业大学生的自闭特质水平高于理工类专业和其他专业的大学生。以往研究发现,理工学生的AQ分数高于文史学生的AQ分数 [32],而本研究的研究对象多为师范院校的大学生,其中大部分学生为师范专业,然而师范院校对文理科的分类没有综合性大学那么明显,所以这造成本研究得到了相反的结果。

4.3.4. 大学生高低自闭特质的情况及影响因素的logistic回归分析

本调查发现,父亲、母亲社会经济地位低的大学生的AQ总分要高于父亲、母亲社会经济地位高的大学生。以往研究发现,相比社会经济地位高的人群,社会经济地位低的人群会表现出更高水平的自闭特质 [33],本调查与以往研究相一致。农民、工人或者家庭主妇的经济地位较低,这也能解释为什么父母工作为工人、农民和家庭主妇的大学生患ASD风险的人数最多。

二元logistic回归分析发现专业、年龄、性别、家庭所在地、父母社会经济地位可解释自闭特质24.5%的变异量,但本研究中没有变量能显著预测高低自闭特质,说明还有更多可以解释自闭特质的变量未被发现。自闭特质受遗传 [5] 、环境 [12] 的影响,这说明本研究的差异结果只是遗传、环境或者更多其他未被发现的因素在自闭特质上作用的结果,而不是影响自闭特质的因素,未来的研究还应该寻找更多的影响自闭特质的因素。

5. 结论

根据问卷调查和分析,可总结得出以下结论:(1) 大学生自闭特质的状况不容乐观,阳性检出率为4.7%,高于美国疾病控制和预防中心的数据1.47%;(2) 大学生的自闭特质在专业、年龄、性别、家庭所在地、父母社会经济地位上存在不同程度的差异;(3) 以高低自闭特质为因变量,运用二元logistic回归分析发现,专业、年龄、性别、家庭所在地、父母社会经济地位可解释自闭特质24.5%的变异量,但本研究中没有变量能显著预测高低自闭特质。
