Clinical Observation on the Treatment of Early Osteonecrosis of the Femoral Head with Extracorporeal Shock Wave

ABSTRACT

Objective: To observe the clinical curative effect of extracorporeal shock wave in the treatment of early osteonecrosis of the femoral head. Methods: Totally 40 cases (48 hips) early osteonecrosis of the femoral head with extracorporeal shock wave treatment, who satisfied inclusion and exclusion criteria during March 2013 to March 2013 in Linyi City People’s Hospital of the femoral head specialist, twice a week, eight consecutive treatment for a period of treatment, a total of three courses. The visual analogue scale and the Harris score were observed and recorded before the treatment and 3, 6, 12, 18 and 24 months after treatment. Results: After treatment with extracorporeal shock wave, the VAS score of the hip joint and the Harris score of the hip joint were significantly improved compared with the previous treatment (p < 0.05). Conclusion: Extracorporeal shock wave is an effective treatment for early osteonecrosis of the femoral head, which can significantly reduce the pain of the hip joint and improve the function of the hip joint.

体外冲击波治疗早期股骨头坏死的临床疗效 观察

摘 要

目的:观察体外冲击波治疗早期股骨头坏死的临床疗效。方法:对2013年3月至2015年3月入住临沂市人民医院股骨头专科符合纳入和排除标准的40例(48髋)早期股骨头坏死患者给予体外冲击波治疗,每周2次,连续治疗8次为一个疗程,共3个疗程,观察并记录治疗前、治疗后第3、6、12、18、24个月时髋关节疼痛视觉模拟(VAS)评分、髋关节Harris评分。结果:体外冲击波治疗后,每次随访时髋关节VAS评分、髋关节Harris评分与治疗前相比有明显的改善(p < 0.05)。结论:体外冲击波是一种治疗早期股骨头坏死的有效方法,能够显著减轻髋关节疼痛、改善髋关节功能。

1. 引言

股骨头坏死(osteonecrosis of the femoral head, ONFH)是指由于不同病因破坏了股骨头的血供,导致骨细胞、骨髓造血细胞及脂肪细胞坏死,继发股骨头结构改变、塌陷,最终造成髋关节功能障碍,是一种骨科临床常见病及疑难病 [1] 。ONFH常见的类型有:激素性股骨头坏死、酒精性股骨头坏死、外伤性股骨头坏死。该病多见于30~50岁中青年人群,约有半数累及双侧股骨头。早期因症状和体征不明显,容易误诊、漏诊,如未经及时有效治疗,大约70%的患者在1~2年内可发展到股骨头塌陷 [2] 。股骨头一旦塌陷,病残率很高,最后需通过行全髋关节置换术(total hip replacement, THA)来改善关节功能,然而对于年轻ONFH患者THA的中长期疗效并不理想 [3] 。因此,对ONFH的患者早诊断,并进行及早的保髋治疗,是延缓或终止病程进展,避免股骨头进一步塌陷的主要预防措施,这也是当前骨科界必须重视的问题。2013年3月至2015年3月我科以体外冲击波治疗早期ONFH取得不错效果,具体报告如下。

2. 临床资料

2.1. 纳入标准和排除标准

纳入标准:1) 根据国际骨循环研究协会(Association Research Circulation Osseus, ARCO)的分类标准 [4] ,选取X线、MRI符合股骨头坏死I、II期的患者;2) 告知患者本试验的研究目的,并签署相关知情同意书。排除标准:1) 有严重认知障碍和精神疾患者;2) 有潜在血栓或出血疾患者;3) 骨骼发育不成熟者;4) III、IV期ONFH;5) 存在感染的患者;6) 已接受其他方法治疗的患者。

2.2. 一般资料

选取我科自2013年3月至2015年3月收治的符合上述标准的患者40例(48髋),平均病程为1.5年,左侧坏死15例,右侧坏死17例,双侧坏死8例;男性22例(27髋),女性18例(21髋);应用激素史例12(14髋),长期饮酒史20例(24髋),不明原因8例(10髋),明显创伤史0例;ARCOI期22髋,II期26髋;年龄21~48岁,平均35.2岁。本组40例(48髋)病人均获得超过24个月的随访。

2.3. 治疗方法

体外冲击波治疗用(苏州锡鑫医疗器械有限公司CS-2012A-9型骨科冲击波治疗仪)进行体外冲击波治疗。患者取仰卧位,根据X线片确定股骨头坏死区域,以ONFH区域及其周围骨质为冲击点,选3个冲击点,每个点冲击1000次,能量密度为0.2 mJ/mm2,每周2次,连续8次为一个疗程,共3个疗程,操作过程中注意避开重要的血管和神经。每次治疗后,在有经验的康复师指导下,嘱患者在床上行股四头肌等长收缩锻炼及髋关节屈曲、外展、内收、外旋、内旋活动。

辅助治疗:我们建议患者在治疗期间及治疗后辅以双拐行走,以减轻髋关节负担。患者分别在治疗前、治疗后3、6、12、18、24个月行髋关节正蛙位X线片检查,了解股骨头变化情况。

2.4. 观察指标

所有患者在治疗后均进行定期随访:1年以内每3个月进行1次,1年以上每半年进行1次。1) 根据随访内容(疼痛程度、关节活动度、有无畸形、行走时辅助、穿袜子、坐椅子、行走距离及爬楼梯等)及X线对所有患者进行髋关节Harris评分,>90分为优,80~90分为良,70~79为中,<70分为差。2) 应用疼痛视觉模拟评分VAS系统(范围0~10分,0分表示无疼痛,10分表示严重疼痛)对患者进行疼痛评分。

2.5. 统计学分析

所有数据均采用SPSS21.0进行统计学分析,计量资料采用均数 ± 标准差( x ¯ ± s)。以P < 0.05为差异具有统计学意义。

3. 结果

患者治疗前后髋关节疼痛视觉模拟(VAS)评分、髋关节Harris评分结果见表1,体外冲击波治疗早期股骨头坏死各个随访时间段均较治疗前有显著改善(P < 0.05)。

4. 讨论

本文通过对40例(48髋)接受体外冲击波治疗的早期股骨头坏死患者进行研究显示,所有患者随访时髋关节疼痛视觉模拟(VAS)评分、髋关节Harris评分均明显高于治疗前,差异具有统计学意义(P < 0.05)。因此,这种体外冲击波对早期股骨头坏死的治疗效果满意,且所有患者均未出现神经损伤等并发症。

股骨头坏死病因及发病机制至今尚未完全清楚,根据国内外目前研究结果,它是由多种病理机制引起的股骨头局部血运不良导致的骨细胞缺血与坏死 [5] 。同时,在大量新的骨细胞生成并代替坏死骨细胞的时候,由于新骨应力性尚低,加上长期患肢负重和股骨头受力不均,易出现头下微骨折,如果此时没有得到有效的治疗,最后将引起股骨头塌陷、关节间隙变窄,最终导致骨性关节炎的发生 [6] 。在股骨头坏死早期的水肿期进展到塌陷期的3~5年内病程是可逆的,因此,采用各种方法来预防塌陷学术界已经达成共识,THA只是一个终末期的治疗方法。对中青年患者更是如此,采取有效的手段,延缓或者阻止股骨头坏死的病程进展,最终达到延迟甚至避免行THA [7] 。近年来,髓芯减压植骨支撑术、钽棒植入术等保髋术式被广泛用于早期ONFH患者且取得了一定的效果,但它们属于有创操作,部分患者不愿接受这类治疗方法,因此,寻找一种无创且有效的治疗方法对早期股骨头坏死具有重要意义。

体外冲击波(extracorporeal shock wave, ESW)最早应用于泌尿系结石,随后,因其良好的组织渗透性和无创治疗,被广泛应用于网球肘、跟腱末端病、慢性肌腱炎、钙化性肌腱炎等腱骨结合处病变和腱病的治疗 [8] [9] 。近年来被广泛应用于骨科领域,且有研究证实,ESW可延缓或阻止早期ONFH的病程进展,对患者具有良好的疗效 [10] [11] 。体外冲击波是一种容易被早期ONFH患者接受的非手术疗法,ESW在作用于病变股骨头时压力呈递减性,测量其对股骨头内10 mm压力时,大约仍有50%的压力,从而可以到达坏死区实现对ONFH的有效治疗 [12] 。有基础实验研究证明,ESW对于ONFH的治疗主要有两个方面:其一是促进血管生成,血管内皮生长因子(vascular endothelial growth factor, VEGF)是一种特异的有丝分裂因子,能刺激内皮细胞增殖、促进新生血管形成及增加血管通透性 [13] 。ESW能增加血管生长因子(如内皮型一氧化氮合酶、VEGF及增殖细胞核抗原)的表达,从而促进血管形成。有研究显示,ESW或许通过VEGF的表达上调促进新生血管形成和改善股骨头血液供应 [14] 。其二是促进骨重建和再生,骨的代谢活动是一个动态平衡过程,通过成骨细胞和破骨细胞参与的骨形成和骨吸收来实现,此类细胞的分化及功能受多种生长因子调控,如骨形态发生蛋白(bone morphogenetic protein, BMP)、转化生长因子、成纤维细胞生长因子、胰岛素样生长因子、血小板衍化生长因子和白细胞介素等 [15] [16] 。研究显示,BMP能诱导未分化间充质细胞向成骨细胞和软骨细胞分化,在骨形成中起重要作用;ESW通过一氧化氮途径调节骨髓基质干细胞中VEGF、碱性磷酸酶、成骨细胞特异性转录因子和BMP-2的基因表达 [17] 。Ma等 [18] 发现,ESW能使家兔坏死的股骨头局部BMP-2表达上调,认为BMP-2在促进骨形成和修复中可能起重要调节作用。Wang等 [19] 研究表明,ESW组中新生血管形成和细胞增殖相关因子(血管性血友病因子、VEGF、血小板–内皮细胞黏附分子、可溶性血管细胞黏附因子)变化显著,骨重建和再生相关因子(增殖细胞核抗原、Winless 3a和Dickkopf-1)也明显改变。Wang等 [20] 研究显示,ESW在ONFH中的局部机械刺激可能影响血管生成、骨生成和抗炎生物指标的系统水平。二者共同作用于股骨头坏死的修复过程,可明显修复坏死区组织、促进新骨形成,从而达到治疗的目的。同时,ESW还可以通过神经轴突强刺激降低神经的敏感性,使疼痛向中枢神经的传导功能受阻,还能抑制疼痛物质的释放,共同达到缓解疼痛的作用 [21] 。

综上所述,体外冲击波能有效延缓或终止股骨头坏死的进程,改善患者的生存质量,是一种安全、有效的治疗方法,值得临床推广和应用。
