The Role of Aquaporins in Renal Fibrosis

ABSTRACT

Renal fibrosis is a common pathological characteristic that usually appears during the process of chronic kidney diseases (CKD), which is a kind of progressive damage and almost irreversible. Renal fibrosis induces kidney dysfunction and finally causes renal failure. Epithelial Mesenchymal Transition (EMT) is a pathological process that the epithelial cells lose their original characteristic and express the feature of fibroblast. Renal epithelial mesenchymal transition plays a critical role in renal fibrosis and regulates the process of renal fibrosis. Aquaporin (AQP) is a kind of water channel protein that highly expressed in kidney, which is involved in water transport and the formation of osmotic pressure in kidney. The expression of AQPs changes when renal fibrosis occurs. Many renal diseases cause imbalance of water transport, so AQPs in kidney may take part in the regulation of various renal diseases, especially renal fibrosis. This article reviews the function and mechanism between various kinds of AQPs and renal fibrosis. At last, we make an outlook on AQPs as the potential target of the EMT process in renal fibrosis.

水通道蛋白在肾脏纤维化中作用 研究进展

摘 要

肾脏纤维化是多种慢性肾病发展到一定阶段常见的病理改变,是一种进行性的几乎不可能的损伤过程,最终会引发肾脏功能损伤并引发肾衰竭。上皮间充质转分化(EMT)是一种在病理状态下上皮细胞改变表型,细胞间粘附因子表达减少,成纤维特征性细胞因子表达增加,细胞逐渐向成纤维细胞转变的进程。EMT过程在肾间质纤维化中起到重要调控作用。水通道蛋白是肾脏中高度表达的负责水分转运的膜蛋白,当肾脏纤维化发生时,水通道蛋白的表达将发生变化,对肾脏疾病的发生与调控起到重要作用。本文综述了多种水通道蛋白在肾脏纤维化疾病中的作用及机制,并对水通道蛋白作为潜在靶点调控纤维化中的EMT过程进行了一定展望。

1. 前言

慢性肾脏疾病一直是困扰人类健康的顽固疾病之一,多年以来患病人数都在不断增长 [1] ,虽然大多数患者在患病时都能够得到有效的诊断,但是因为其临床症状表现的出现存在一定的滞后,所以在纤维化早期许多人并没有去及时进行治疗。当慢性肾病进行性发展至纤维化形成期,则开始进入几乎不可逆的损伤过程。临床上暂时还没有有效的治疗方法将其完全治愈。肾脏纤维化是一种病理生理改变,是肾脏的功能由正常到损伤,直至功能丧失的渐进过程 [2] 。肾脏受到创伤、感染、炎症、免疫反应等多种因素刺激,其固有细胞受损,逐渐出现大量胶原异常沉积,造成肾实质逐渐损伤硬化,直至肾脏完全丧失功能。

肾脏内正常细胞纤维化、硬化的过程也就是肾脏纤维化的过程。肾脏纤维化是多种慢性肾脏疾病发展到一定阶段常见的病理改变 [3] [4] ,其严重程度不仅影响肾功能,而且与肾病的预后有着密切联系。肾脏纤维化将引发肾脏功能紊乱与损伤,严重的将最终引发肾衰竭 [5] 。目前认为肾脏纤维化形成的分子机制主要分四个阶段:第一阶段是炎症损伤引发上皮细胞的活化和受损。第二阶段是促纤维化因子的释放。包括细胞因子、生长因子、血管活性因子和趋化粘附因子等。第三阶段是纤维化的形成。主要表现在基质蛋白合成增多, 降解减少, 导致细胞外基质(ECM)在肾间质沉积。第四阶段是肾脏结构和功能受损。此阶段肾小管周围毛细血管堵塞、肾脏功能大幅降低 [6] 。

肾脏是人体中与水转运处理最密切最直接的器官,水通道蛋白是一种参与水转运和渗透压形成的膜蛋白 [7] 。在EMT过程中,细胞液体和离子转运能力下降,细胞间紧密连接和一些粘附因子表达下降,迁移能力得到上升 [8] ,这与AQP的表达变化有着紧密的联系。本文就水通道蛋白(AQP)在肾脏纤维化中的作用与机制做一综述。

2. 肾脏纤维化的发展及调控机制

肾脏纤维化是以在肾间质部位的成纤维细胞大量增殖、活化,细胞外基质(ECM)大量分泌合成,异常沉积为特征的 [9] 。肾间质的基质成分由成纤维细胞合成分泌而来,研究表明 [10] ,肾间质成纤维细胞主要有三个来源:1) 肾脏固有的间质成纤维细胞;2) 循环而来的间充质细胞;3) 肾小管上皮细胞间充质转分化(Epithelial Mesenchymal Transition, EMT)。其中EMT在肾小管间质纤维化中的作用正越来越受到重视。上皮间充质转分化(EMT)是肾间质纤维化中一个重要的环节 [11] [12] 。对肾间质纤维化的发生和恶化起到关键性作用。

上皮细胞间充质转分化(epithelial mesenchymal transition, EMT)是指在病理情况下上皮细胞外环境改变,维持生理功能的多种细胞因子功能失去平衡,保持上皮细胞形态的基因和蛋白表达受到抑制,肌成纤维细胞表型的基因和蛋白表达上调,细胞失去其原有上皮特征转变为间充质表型。细胞失去离子及液体转运功能转变成重塑基质的肌成纤维细胞。Strutz等 [13] 在抗肾小管基底膜疾病的小鼠模型中发现成纤维特异性蛋白1 (FSP-1)可以在肾小管上皮细胞表达,首次证实了EMT 参与了肾纤维化的发成与形成。EMT是一种有序的调节过程,包括四个关键步骤:1) 上皮细胞失去粘附能力;2) α-SMA的表达及肌动蛋白的重组;3) 肾小管基底膜破坏;4) 细胞迁移和侵袭能力增强 [14] 。

上皮细胞之间通过各种细胞粘附因子(如E-钙黏蛋白E-Cadherin)形成整齐的排列一致的片状结构,在上皮细胞层之下,基底膜将上皮细胞牢牢地与基质表面结合在一起,以维持上皮细胞层的顶端-基底端极性 [15] 。EMT发生时,E-钙黏蛋白的表达量减少,功能逐渐丧失,并伴随着细胞间紧密连接蛋白和细胞角蛋白的减少。与此同时,间充质细胞的多种标志物表达量增加,α-平滑肌肌动蛋白(α-SMA)等 [16] 。细胞间粘附连接的丧失引发了细胞骨架结构的重组,成为纺锤形态,这些新产生的成纤维细胞侵蚀原本基底膜上的细胞基质,并向邻近组织迁移,分泌出纤连蛋白等细胞外基质。能够激活并刺激EMT发生的有许多细胞通路,如生长转化因子β(TGF-β)、骨形态生成蛋白(BMP)、表皮生长因子(EGF)、成纤维细胞生长因子(FGF)等 [17] [18] 。许多信号通路是具有组织或细胞特异性的,在某个特定组织中起到关键作用,这可能与细胞状态、细胞微环境以及其敏感性有关 [19] 。

3. 水通道蛋白与肾脏纤维化

3.1. 水通道蛋白

水通道蛋白(aquaporin),是一种能够转运水分子的蛋白孔道,存在于各种组织细胞膜表面,由Agre及其团队首先发现 [20] ,因为其分子量和结构特点,最初被称为CHIP28 (channel-like integral protein of 28 kDa),后被正式命名为水通道蛋白1 (AQP1) [21] 。Agre也因发现水通道蛋白而获得2003年诺贝尔化学奖。

水通道蛋白家族具有相似的结构,水通道蛋白的单个亚基是一条单肽链,具有六个跨膜结构域,在第二、三以及第五、六跨膜结构域之间存在着疏水的环状结构,在此结构上存在Asn-Pro-Ala(NPA)重复串联序列,此重复序列是水通道蛋白家族所具有的特有的高度同源的特征序列。在肽链经过折叠后两个疏水环状结构嵌入细胞磷脂层中,在膜上形成供水分子通过的孔道 [22] 。水通道蛋白四级结构中具有四个上述构成的亚基,在其中一个亚基上链接有一个长链多聚糖基 [23] 。此四聚体中每个亚基都能够独立实现水分子的转运功能,但四聚体的整体结构对维持单个亚基的功能与位置起着重要的作用 [24] [25] 。多种AQP活性能够被氯化汞HgCl2或其他汞剂所抑制 [20] 。

在肾脏中至少存在7种水通道蛋白 [23] ,分别是AQP1-AQP4和AQP6-AQP8。它们的分布位置各不相同,发挥着不同的作用。

3.2. 水通道蛋白与肾脏纤维化

在肾脏中至少存在7种水通道蛋白 [23] ,分别是AQP1-4和AQP6-8。它们的分布位置各不相同,发挥着不同的作用。水通道蛋白由于其功能与水分吸收、转运有着密切的关系。在肾脏疾病的发展中,尤其是在纤维化损伤中,水平衡的维持发生紊乱,对疾病的发展有着不利的影响。这都与水通道蛋白表达量的变化存在着重要的联系。

水通道蛋白1分布在肾脏近曲小管和髓襻降支细段 [26] ,与水分重吸收有着至关重要的联系。免疫电镜分析显示水通道蛋白1在原生质膜的顶端和底端都有表达,与其需要在细胞各个表面发挥水的转运的功能需要相一致 [27] 。水通道蛋白1在髓襻降支细段上呈现出一种轴向异质性 [28] ,这与肾脏内部的渗透压梯度相关,水通道蛋白1的分布密度随着渗透压的改变而改变 [29] 。水通道蛋白1与肾脏纤维化中的关键——EMT过程之间的联系有一些报道,Sara Lovisa等人 [30] 在小鼠TEC细胞上建立慢性损伤模型后检验了EMT标志物,并验证了EMT过程的发生。在此损伤过程中发现水通道蛋白1的表达出现了显著的下降。Masao Nakasatomi等人 [31] 在大鼠上建立UUO模型,诱发大鼠产生肾脏纤维化损伤,取肾脏组织进行检测后同样发现水通道蛋白1的表达也出现了显著性下降。体内与体外实验都说明了水通道蛋白1在肾脏纤维化损伤中表达出现明显变化,并且在这些肾脏纤维化损伤中,都检测到了EMT过程的产生。说明水通道蛋白1在EMT过程中可能发挥了重要的调控作用。不但在慢性肾病上,在急性肾损伤造成的EMT过程中,水通道蛋白1的表达也出现了一定的变化。Shimada Mikio等人 [32] 使用低剂量乙酸双氧铀诱导局部肾近曲小管损伤,几天后成纤维细胞标志物表达逐渐增加,并且水通道蛋白1的表达发生显著性降低。

水通道蛋白2在集合管细胞的顶端质膜和顶端的囊泡中有着丰富的表达 [33] 。水通道蛋白2的最主要作用是介导水的跨膜运输。水通道蛋白2受到加压素(ADH)的调控,分为短期调节和长期调节。水通道蛋白2分布在顶端质膜和顶端的囊泡,在囊泡中的水通道蛋白2平时不发挥作用。当加压素作用于加压素II型受体(V2受体)后,含有水通道蛋白2的囊泡向顶端质膜移动,对水的转运能力也得到增强 [34] 。水通道蛋白2在ADH影响下,在质膜与囊泡中不断改变位置的调节过程称为“穿梭机制” [33] 。同时,加压素长期作用影响水通道蛋白2,还能够使水通道蛋白2的表达量增加,称为长期调节。水通道蛋白2表达发生改变后,集合管重吸收功能发生改变,将会出现尿崩症、肾积水等临床病症。尤其在肾积水病症表现中,肾素血管紧张素系统高度激活,炎性介质高度表达,肾小管间质会产生纤维化病变。肾积水多因尿路梗阻引起,无论是在双侧输尿管梗阻(BUO)模型中,还是在单侧输尿管梗阻(UUO)模型中,水通道蛋白2的表达都会产生显著性降低。DANILOVIC等人 [35] 在BUO大鼠模型中发现水通道蛋白2的表达量明显下调,纤维化相关蛋白的表达上调。而WANG等 [36] 人在小鼠UUO模型中发现水通道蛋白2表达量显著降低,伴随着纤维化和炎症相关因子表达的升高。多囊肾病,一种先天性遗传疾病,在发病过程中会出现纤维化指标的升高,提示其疾病末期将导致肾脏纤维化。近年来,Tamma等人 [37] 发现发生在多囊肾病发生过程中,水通道蛋白2的表达异常,可能在疾病过程中起到关键调节作用。

水通道蛋白3和4都分布在集合管上,与水通道蛋白2相对应,水通道蛋白3与4两者分布于基侧质膜上 [38] [39] 。水通道蛋白3也能够被ADH所调控,但水通道蛋白4对ADH不敏感。两者都与尿液的浓缩重吸收有关 [40] [41] 。水通道蛋白2将水从顶端质膜引入集合管细胞内后,水通道蛋白3与4协同作用,再将这些水分通过底端质膜引出集合管细胞,从而实现水分重吸收入血,尿液浓缩的功能。因为水通道蛋白3和4的分布位置及辅助尿液重吸收的功能,当水通道蛋白3和4的表达发生异常下调后,将引发肾积水病症。与水通道蛋白2类似,JENNIFER J.等 [42] 人发现当尿路上端发生梗阻后,纤维化相关基因和蛋白表达显著升高,肾脏出现一定的间质纤维化病变,在此过程中,水通道蛋白3和4表达的显著减少。李真珍 [43] 等人在对儿童轻度肾积水的进行研究过程中也得到了类似的结果。

水通道蛋白6分布在皮质、外髓质以及内髓质的集合管闰细胞上 [44] ,水通道蛋白6具有一个鲜明特点,只在细胞内的囊泡表达,而不在细胞外质膜表达。因此水通道蛋白6可能是一种特定的转运细胞内部水分的水通道蛋白。有研究 [44] 认为水通道蛋白6与肾小管的内吞、酸碱代谢有关。水通道蛋白7在近曲小管刷毛缘上表达,水通道蛋白8都在近曲小管和集合管表达,目前这三种水通道蛋白在肾脏的功能研究还不是很透彻,在肾脏纤维化中的相关研究也还不够完善。

由此可见,上述一些研究表明在急性肾损伤与慢性肾损伤过程中,多种水通道蛋白与肾脏纤维化或纤维化过程中的上皮间充质转分化进程存在着紧密的联系。水通道蛋白的表达状况在这些病症中发生上升或下调,有可能在一定程度上对肾脏纤维化过程起到调控作用。并且有研究 [45] 表明当水通道蛋白高表达时,肾脏对损伤的修复能力能够得到一定程度的提升。对于水通道蛋白能够调控肾脏纤维化损伤的推测是一个有力的支持。

4. 结语

肾脏纤维化,一个慢性肾病多会经历的病理过程,在全世界都引起的极大的关注,相关研究也在不断进行。虽然肾脏纤维化在临床中十分常见,但是暂时还没有特效的药物能够逆转纤维化过程。在理论研究领域中,抑制EMT过程可以作为一个治疗纤维化的思路,也有一些药物能够有效抑制纤维化过程,如促红细胞生成素(EPO) [46] 、骨形成蛋白-7(BMP-7) [47] 等。但是这些药物单体都是一些细胞因子,运用在临床中的成本较高,推广开来有一定难度。抑制氧化应激、炎症介质产生也是一个可以实行的方案,但是整体将其抑制,特异性不强,副作用较大。水通道蛋白在肾脏分布广,密度大,若作为药物靶点,能够针对于肾脏,发挥出一定的靶向作用,减少对其他器官组织的损伤。根据上述水通道蛋白与肾脏纤维化之间的实验研究,水通道蛋白很有可能在纤维化疾病过程中起到调节EMT进程的作用。若水通道蛋白能够调节纤维化疾病中的EMT过程,使EMT过程得到抑制,则水通道蛋白很有可能成为一个很有潜力,研究价值很高的治疗靶点。本文首次对肾脏内各种水通道蛋白与肾脏纤维化疾病之间的关系以及在调控纤维化过程中的作用机制进行了综述,并对水通道蛋白作为药物靶点调节纤维化病程做出了一定的展望,为今后肾脏纤维化的临床治疗提供了一个新的思路。
