A IL-6 Receptor Blocker for Rheumatoid Arthritis—Sarilumab

ABSTRACT

Rheumatoid arthritis (RA) is a chronic systemic inflammatory disease predominantly affecting the peripheral diarthrodial joints. Sarilumab is a fully human IgG1monoclonal antibody that binds specifically to both soluble and membrane-bound interleukin (IL)-6 receptors and thereby inhibits IL-6-mediatedsignalling through these receptors. In Phase II and III clinical trials, Sarilumab in the background of methotrexate showed superior clinical efficacy over placebo in RA patients with inadequate response to methotrexate or inadequate response or intolerance to TNF inhibitors. Sarilumab monotherapy also showed superior efficacy compared with adalimumab monotherapy in RA patients with inadequate response or intolerance to methotrexate. Sarilumab is an alternative to biologic disease-modifying anti-rheumatic drugs (DMARDs) or targeted synthetic DMARDs in patients with moderate to severely active RA who have not responded adequately to prior conventional synthetic DMARDs or tumor necrosis factor-α inhibitors. In this review, mechanism of action, clinical efficacy and safety of Sarilumab in the treatment of RA will be summarized.

摘 要

类风湿性关节炎是一种慢性全身性炎症性疾病,主要影响外周关节。Sarilumab是一种全人源化单克隆抗体,能与可溶性和膜结合白细胞介素6受体(IL-6R)特异性结合,从而抑制白细胞介素6介导的信号转导。在II期和III期临床试验中,Sarilumab在对甲氨蝶呤反应不足或对肿瘤坏死因子抑制剂反应不足或不耐受的类风湿性关节炎患者中表现出优于安慰剂的临床效果。与阿达木单抗单药治疗相比,Sarilumab单药治疗对甲氨蝶呤疗效不佳或不耐受的类风湿性关节炎患者也表现出优异的效果。Sarilumab是对常规合成的生物疾病缓解型抗风湿药物或肿瘤坏死因子α抑制剂反应不足的中度至重度活动性类风湿性关节炎患者的生物疾病缓解型抗风湿药物或靶向合成疾病缓解型抗风湿药物的替代药物。本综述总结了Sarilumab在类风湿性关节炎治疗中的作用机制、临床疗效及安全性。

1. 引言

类风湿性关节炎(RA)是一种以进行性关节残疾、全身炎症和高发病率为特征的慢性自身免疫性疾病 [1] 。近年来,一些研究表明RA发病机制是由多种炎症细胞和细胞因子的复杂网络共同作用 [2] [3] [4] ,导致关节破坏和功能丧失,并产生疲劳,贫血,骨质疏松症和心血管疾病等全身现象,RA的病理生理学涉及许多不同的细胞类型,包括巨噬细胞,B细胞,T细胞等,这些细胞都可引起局部关节损伤。遗传和环境因素在激活免疫系统和最终产生异常和持续的炎症反应中发挥重要作用 [5] 。目前使用生物技术药物,针对某些促炎症参与自身免疫过程的分子,彻底革新了RA的治疗方法。根据基于治疗靶点方法的最新研究 [6] ,甲氨蝶呤(MTX)与生物疾病缓解型抗风湿药物(DMARDs)的联合应用是治疗难治性RA的有效方法。此外,阻断肿瘤坏死因子α (TNF-α)是为该类疾病提出的第一种生物技术作用机制,迄今为止,TNF抑制剂(TNFis)是用于治疗RA的最广泛的生物DMARDs。然而,在常规治疗中,约30%~40%的用TNFi治疗的患者未能达到临床目标,或产生不良事件,导致治疗方法被撤回 [7] [8] 。因此,越来越多的关于RA治疗途径的探索集中在涉及发病机理的其他潜在目标上,产生了许多不同作用机制的生物DMARDs,如B细胞消除,T细胞共刺激抑制和IL-6阻断。在体外研究中,IL-6通过促进T细胞和B细胞活化,诱导免疫球蛋白分泌,刺激定向造血干细胞前体细胞的增殖和分化等多种生理过程而在RA自身免疫途径中发挥关键作用 [9] 。因此,IL-6具有与关节炎和之前提到的该病的大多数关节外表现有关,如贫血、心血管疾病风险增加和骨质疏松 [10] 。根据这些研究结果,最终促成了第一个人源化抗IL-6受体(IL-6R)单克隆抗体托珠单抗(tocilizumab)的研发 [11] ,tocilizumab可与DMARDs联合使用 [12] [13] [14] [15] ,也可以作为单药治疗 [16] 。随机对照试验和临床观察取得了良好的结果 [17] ,特别是在需要生物DMARDs单药治疗时效果显著,国际指南推荐IL-6拮抗剂作为MTX治疗无效的一线生物DMARDs [18] [19] 。

Tocilizumab治疗RA的显着成功激励了以IL-6或IL-6R为靶向的更高亲和力的新生物制剂的开发 [20] ,如Sarilumab、sirukumab [21] 、olokizumab [22] 和clazakizumab [23] 。Sarilumab是全人源IgG1单克隆抗体,由赛诺菲与再生元制药公司合作开发。2017年2月,获加拿大卫生部批准用于治疗对一种或多种生物或非生物疾病改善抗风湿药物反应不足的中度至重度活动性类风湿性关节炎的成人患者 [24] 。2017年5月22日,获得美国食品药品监督管理局批准治疗对改善DMARDs不耐受或应答不良的中、重度活动性RA [25] 。

2. Sarilumab的基本信息

Sarilumab是继tocilizumab后第二个上市的IL-6R拮抗剂。Sarilumab在设计产生人抗体的小鼠中对人IL-6R的亲和力比tocilizumab大20倍 [26] 。Sarilumab的基本信息如下表1

3. 作用机制

IL-6与IL-6受体结合并通过顺式和反式信号传导机制发挥作用。IL-6R由与IL-6结合的膜相关IL-6受体-α (mIL6R)和信号转导蛋白gp130分子组成 [27] 。在顺式信号级联反应中,IL-6与mIL6R结合 [28] ,并激活gp130分子。激活的gp130分子导致JAK1,JAK2和Tyk2的磷酸化,然后活化STAT1和STAT3。这个信号最终指导炎症分子的转录。在反式途径中,IL-6与可溶性IL-6R结合形成复合物并激活具有gp130分子的细胞。在正常生理条件下,可溶性的gp130分子充当缓冲液以阻断IL-6反式信号传导,而不阻断顺式信号转导 [29] 。

Sarilumab是一种重组人IgG1单克隆抗体,由2条人重链组成,每条重链通过二硫键与1条人 κ 轻链连接。Sarilumab与tocilizumab一样,特异性地以高亲和力与IL-6R上的特定表位结合,并且可以有效阻断IL-6信号转导的顺式和反式激活 [30] 。

4. 药效学

Sarilumab推荐剂量为每两周皮下注射200 mg或150 mg。Sarilumab可单独给药,也可与MTX或其他常规DMARDs联合使用。Sarilumab可使C反应蛋白(CRP)水平降低,单次给药两周内可将其降至正常水平。Sarilumab还能够降低纤维蛋白原和血清淀粉样蛋白A浓度,并提高血红蛋白和血清白蛋白浓度。Sarilumab的主要副作用之一是绝对嗜中性粒细胞数减少 [31] 。

5. 药代动力学

在1770例RA患者中,通过群体药代动力学(PPK)分析表征Sarilumab的药代动力学,患者每2周皮下注射Sarilumab 150 mg或200 mg,给药时间长达52周 [32] 。Sarilumab在2~4天达到最大浓度,在14~16周达到稳定状态。多剂量给药后Sarilumab的累积量是单剂量给药的2~3倍。稳态下的表观分布容积为7.3 L。

尚未表征Sarilumab的代谢途径。作为单克隆抗体Sarilumab预期通过分解代谢途径以与内源性IgG相同的方式降解成小肽和氨基酸。与其他单克隆抗体相似,Sarilumab不通过肾脏或肝脏通路消除,而是通过平行线性和非线性途径消除。这些平行消除途径的初始消除半衰期为8~10天,终浓度依赖性半衰期为2~4天。PPK数据显示在抗Sarilumab抗体存在的情况下,Sarilumab有较高的表观清除率升高的趋势;不建议剂量调整。

成年RA患者的PPK分析显示年龄,性别,种族和体重对Sarilumab的药代动力学没有临床意义。目前还没有正式研究肝或肾损害对Sarilumab药代动力学的影响。PPK数据表明轻度或中度肾损伤不影响Sarilumab的药代动力学;尚未对严重肾功能不全的患者进行研究。

当与甲氨蝶呤联合给药时,Sarilumab表达不受影响。Sarilumab尚未与JAK抑制剂或TNF抑制剂联用进行研究。各种体外和有限的体内人体研究表明,细胞因子和细胞因子调节剂可以影响特定CYP酶的表达和活性,从而有可能改变作为这些酶底物的药物的药代动力学。与没有RA的患者相比,升高的IL-6浓度可能降低RA患者的CYP活性,从而提高药物水平。IL-6R拮抗剂可抑制IL-6信号传导逆转IL-6的抑制作用并恢复CYP活性,导致药物浓度改变。当Sarilumab与CYP3A4底物共同给药时,建议谨慎,因为可能会降低暴露量,从而降低CYP3A4底物的活性 [33] 。

6. 临床研究

关于Sarilumab在RA治疗中的疗效和安全性的研究主要有四项。其中第一项是MOBILITY A部分II期研究 [34] ,是一项多中心、随机、双盲、安慰剂对照、剂量变化的研究,在12周的研究中比较了5个剂量的Sarilumab及安慰剂在306例中度至重度RA患者中的应用情况。患者在服用MTX的同时,随机皮下注射安慰剂或Sarilumab,Sarilumab剂量分别为每2周100 mg、每2周150 mg、每2周200 mg、每周100 mg和每周150 mg。ACR20应答率分别为49%,67%,65%,62%和72%,而安慰剂组为46%。该研究显示Sarilumab可有效治疗活动性RA。四个最高Sarilumab剂量组具有相似的功效,并且每2周给药方案可以在整个给药间隔内有效抑制CRP水平。这项研究的时间很短,但为RA治疗中的生物DMARDs的剂量寻找提供初步依据。这项研究每组50至52名患者,其数量比其他研究少,但是样本量足以检测接受Sarilumab + MTX的患者与服用安慰剂 + MTX的患者之间的统计学显着性差异。整体研究设计良好,并为选择每2周150 mg或每2周200 mg皮下注射Sarilumab联合MTX作为治疗RA的批准剂量提供依据。

第二项研究是MOBILITY B部分III期试验 [35] 。这项多中心,随机,双盲,安慰剂对照研究在持续服用MTX的中度至重度RA患者中进行。共有1197名患者在接受MTX的同时,随机每2周1次接受Sarilumab 150 mg (n = 400)、200 mg (n = 399)或安慰剂(n = 398)。其共同主要终点包括在24周时达到ACR20应答的患者比例、在第16周时HAQ-DI从基线的变化以及在第52周时改良Sharp/van der Heijde评分从基线的变化。在共同主要终点中,两个Sarilumab剂量组均显示统计学显着改善。与安慰剂组相比,Sarilumab组的ACR20应答率显着提高。在150 mg和200 mg Sarilumab组中,HAQ-DI从基线变化显着高于安慰剂组。以改良Sharp/van der Heijde评分从基线变化进行评价,与安慰剂组相比,服用Sarilumab显着延缓了第52周时结构损伤的放射性进展。

第三项TARGET III期研究是一项多中心,多国,随机,双盲研究,用于中度至严重活动性RA患者以及对TNF治疗反应不足或不耐受的患者 [36] 。在这项研究中,546名患者被随机分配,在接受常规合成DMARDs的同时,每2周接受Sarilumab 150 mg (n = 181),每2周接受Sarilumab 200 mg (n = 184)或接受安慰剂(n = 181)。共同主要终点是在24周时达到ACR20应答的患者比例以及在12周时HAQ-DI从基线变化的患者比例。在24周时,Sarilumab组ACR20应答率明显更高(150 mg Sarilumab组为55.8%,200 mg Sarilumab组为60.9%,安慰剂组为33.7%,p < 0.0001),DAS28-CRP和HAQ-DI的变化结果也优于安慰剂组。

第四项MONARCH III期试验是一项多中心,多国,随机,双盲研究,在24周中对中度至严重活动性RA患者Sarilumab单药治疗与阿达木单抗单药治疗进行双盲双模拟比较 [37] ,这些患者均对MTX不耐受或反映不足。随机选择184名患者每2周接受Sarilumab 200 mg,185名患者每2周接受阿达木单抗40 mg。在第24周时,Sarilumab组主要终点DAS28-ESR从基线变化显着高于阿达木单抗组。Sarilumab组ACR20/50/70应答率和HAR-DI从基线变化也显着高于阿达木单抗组。在第24周时临床疾病活动指数(CDAI)的缓解方面,Sarilumab组也优于阿达木单抗组。这项研究具有临床意义,许多患者因对常规DMARDs不耐受等原因需要接受单药治疗,这项研究为此类患者的治疗提供新方法。这项研究的设计与ADACTA研究相似,通过CDAI的测量证实了Sarilumab的临床疗效与tocilizumab类似 [38] 。

7. 安全性

在Sarilumab的临床试验中发现的常见不良药物反应与其他生物免疫抑制剂常见药物不良反应一致,包括感染(例如鼻咽炎,上呼吸道感染和尿道感染),嗜中性粒细胞减少症,注射部位红斑,低密度脂蛋白(LDL)胆固醇增加和肝酶增加 [39] 。治疗药物不良事件(TEAE)在临床试验中有所不同。例如,在MOBILITY II期试验中,Sarilumab组中TEAE的发生率为43.1%至72.0%,安慰剂组为47.1%。导致终止Sarilumab的TEAE在150 mg剂量中为3.8%,在200 mg剂量中为7.8%。中断Sarilumab的主要原因是感染和中性粒细胞减少症。然而,在MOBILITY III期试验中,导致中断的不良事件百分比在150 mg剂量中为12.5%,在200 mg剂量时为13.9%。在MONARCH研究中,Sarilumab和阿达木单抗单药治疗的整体安全性和耐受性类似。两个治疗组之间的不良事件(AE)发生率(Sarilumab 64.1%和阿达木单抗63.6%),严重不良事件(SAE) (Sarilumab 4.9%和adalimumab 6.5%)和感染比例(Sarilumab 28.8%和adalimumab 27.7%)基本相当。在MOBILITY和TARGET试验的汇总探索性分析中,SAE主要发生在65岁以上的老年人群中 [40] 。

EMA和FDA都对LDL胆固醇增加后继发的主要不良心血管事件(MACEs)的临床影响进行了评估 [41] [42] 。在临床试验中,与安慰剂组和活性对照组的受试者相比,接受他汀治疗的Sarilumab组中的LDL胆固醇增加患者百分比较高;然而,在MACE和Sarilumab相关的LDL胆固醇增加之间进行确定性相关的试验中,MACE的数量很少。Sarilumab的处方信息包含黑箱警告感染风险,并建议患者在接受治疗前接受结核病检测 [43] 。此外,服用Sarilumab的患者必须避免接种活疫苗,因为存在感染风险。

8. 结论

临床研究的数据表明,Sarilumab (每2周150 mg或200 mg)与MTX联合服用比安慰剂加MTX更有效,并且每2周Sarilumab 200 mg单药治疗比阿达木单抗单药治疗更有效。Sarilumab、阿达木单抗(单药治疗)和安慰剂组之间的不良反应风险相似,但安慰剂组因不良反应引起的撤药次数较少。与历史数据相比,Sarilumab的总体疗效,安全性与tocilizumab和其他生物DMARDs类似。因此,Sarilumab是一种有效的生物DMARDs,可用于替代tocilizumab,其他生物DMARDs或托法替尼治疗对常规合成DMARDs或TNFis反应不足的中度至重度RA患者。
