Progress in the Diagnosis and Treatment of Prostate Cancer Specific Membrane Antigen for Prostate Cancer

ABSTRACT

Prostate specific membrane (PSMA) is a type 2 integral membrane glycoprotein with folate hydrolase and carboxypeptidase, which has high prostate tissue specificity and is high in poorly differentiated and metastatic prostate cancer. The expression is positively correlated with the degree of malignancy of prostate tumors. Based on this, the imaging method for detecting and treating prostate cancer with PSMA is continuously developed. The aim of this article is to review the progress of PSMA as a marker for imaging diagnosis and treatment of prostate cancer.

摘 要

前列腺特异性膜抗原(Prostate specific membrane, PSMA)是具有叶酸水解酶和羧肽酶的2型整合膜糖蛋白,具有较高的前列腺组织特异性,其在低分化及转移性前列腺癌中呈现高表达,与前列腺肿瘤恶性程度正相关,基于此,以PSMA为靶标检查及治疗前列腺癌的影像学方式不断发展。该文章目的在于回顾PSMA作为前列腺癌影像学诊断和治疗标志物的研究进展。

1. 引言

随着社会的发展,前列腺癌(prostate cancer, PCa)发病率逐年升高,现已成为严重威胁亚洲男性健康的第2大恶性肿瘤。自血清前列腺特异抗原(prostate specific antigen, PSA)发现以来,由于其较高的灵敏性,至今首选为前列腺癌筛查、术后随访的指标。但是,随之PSA的缺陷也不断显现,如PSA特异性差,在前列腺炎症、泌尿系感染时PSA亦可升高。同时,由于PSA的灵敏性过高,导致临床的过度治疗现象也普遍存在。虽然临床研究发现了如fPSA和tPSA等衍生物,一定程度上弥补了PSA特异性欠缺的局限,但是结果仍然不够理想。近几年的研究,也不断有新的前列腺癌标志物被发现,如PCA3、hK2、a-甲酰基辅酶A消旋酶、TMPRSS2-ERG融合基因等,这些标志物在一定程度上对于PSA在前列腺癌诊断中进行补充。

前列腺特异性膜抗原(prostate specific membrane, PSMA)是存在于前列腺腺上皮细胞胞膜的一种100 kD的II型固有膜蛋白。定位于染色体11p11~12。PSMA最早是采用单克隆抗体技术在LNCaP细胞系的细胞膜表面发现 [1]。组化研究结果显示,较正常前列腺组织,在高级前列腺上皮内瘤样病变和前列腺癌组织中其表达明显升高 [2]。PSMA由3个结构域组成,分别为含19个氨基酸(AA)的胞内N一末端区,含24个AA的跨膜区和含707个AA的胞外区。PSMA有明显的前列腺组织特异性,在前列腺癌和其它系统恶性肿瘤鉴别时特异性为94.5%,敏感度为65.9%。PSMA的表达与前列腺癌的生长和分化有直接相关性 [3]。在体外实验中可以发现,PSMA的表达与细胞中叶酸的浓度有关。叶酸浓度的增加会诱导肿瘤细胞进一步表达PASM [4] [5]。细胞实验研究结果可以看出,PSMA可以通过phospho-p38 (P-p38)反应通路刺激LNCaP细胞的分化、转移和存活 [6]。进一步研究,在LNCaP细胞系中,PSMA的敲除不仅与磷脂酰肌醇3-激酶/Akt信号通路的抑制有关,而且与细胞增殖、迁移和存活率的降低有关 [7]。PSMA同时在PCa的转移中也起到促进作用。已有研究对于成熟的前列腺癌细胞系(包括DU145、LNCap、PC-3、22RV1)的研究发现,CDH6,MMP3,和MTSS1可作为PSMA的调控基因。它们的表达与癌症的分期呈负相关,因此提示上述基因可能参与了PSMA对前列腺癌转移的抑制 [8]。

2. 前列腺癌的影像学检测技术及进展

目前,超声、CT、骨扫描和磁共振成像(MRI)等已被广泛用来检测原发性前列腺癌及其转移。然而,其局限性在于其对复发和转移性前列腺癌检测的敏感性较低。虽然PETCT和SPECT在前列腺癌的早期诊断、转移的诊断方面灵敏性较普通CT等更高,但仍然没有达到临床满意的要求。临床上急需一种更为灵敏的影响检测方式用于前列腺癌的诊断。近几年,由于PSMA在前列腺癌组织的特异性发现,基于PSMA作为前列腺癌影像诊断标志物技术研究备受瞩目 [9] [10]。

此类研究主要致力于基于PSMA的早期前列腺癌的影像学诊断。Fendler等的研究评估了68Ga-PSMA-11 PET/CT方式诊断临床已活检确诊前列腺癌患者的准确性,该方式的诊断敏感性为67%、特异性92%、阳性预测值97%、准确度72% [11]。Woythal等的研究分别对前列腺癌手术前后的患者进行68GA-PSMA-11 PET/CT检测,也显示了较高的灵敏度和特异性,分别是87%和97% [12]。Uprimny等的通过采用GS和PSA水平不同进项分组研究发现。前列腺癌GS评分在6、7a(3 + 4)和7b(4 + 3)时,68Ga-PSMA-11摄取率低于GS大于7患者组。PSA在10.0 ng/mL及以上患者的68Ga-PSMA-11摄取率高于PSA在10.0 ng/mL以下的患者 [13]。多参数磁共振成像(mpMRI)和68Ga-PSMA-11 PET/CT检测结果较为一致。通过给mpMRI提供额外的分子成像信息,在检测高GS病例中,该方法可进一步避免假阴性结果。此外,PSMA PET/MRI可能有助于发现惰性肿瘤和大量中度、高度风险的肿瘤。

在前列腺癌分期的影像学检测中,根据目前的指南建议中高危肿瘤需进行腹部-盆腔联合MRI或CT,和骨扫描 [14]。在一项前瞻性研究中,30名患有中高风险前列腺癌的患者接受了术前68Ga-PSMA PET/CT检测,然后进行根治性前列腺癌切除术和盆腔淋巴结清扫。68Ga-PSMA PET/CT对淋巴结转移的诊断敏感性为64%,特异性为95%,阳性预测值为88%,阴性预测值为82% [15]。在另一项前瞻性研究中,Gorin等分析分析了PSMA靶向18F-DCFPyL PET/TC在术前分期中的诊断价值。其中25例尽管常规检测分期结果为阴性,被认为具有转移性前列腺癌的高风险的患者的术前分期中的诊断价值,检测敏感性和特异性为71%和88% [16]。Maurer等人进行的一项回顾性研究中,纳入130名中高风险前列腺癌患者,分别采用68Ga-PSMA-PET/磁共振断层扫描或PET/CT进行肿瘤分期。68Ga-PSMA-PET检测的敏感性,特异性和准确性分别为65.9%,98.9%和88.5%;磁共振断层扫描或PET/CT的敏感性,特异性和准确性分别为43.9%,85.4%和72.3%。因此,将68Ga-PSMA PET添加到传统方法中有可能取代目前的标准成像,从而实现更完整和准确的初级分期 [17]。

3. 前列腺癌的PSMA靶向治疗策略

除了可以用于前列腺癌检测的靶标,PSMA由于其组成型或诱导的内化特性以及其在前列腺癌中的高表达而被广泛用作靶抗原。包括肽类、单克隆抗体在内的几种方案已被用作纳米颗粒以提高前列腺癌细胞中的靶向效率。癌症标志物的发现和发展可能在前列腺癌分子成像中具有一定的作用。

适体是能够在体内呈现特定且稳定的三维形状的短DNA,RNA或肽寡聚体 [18]。它们的高亲和力和特异性与抗体相似,是通过与靶表面互补的三维构象实现的。在这方面,Lupold等人鉴定了两种RNA适体(A9和A10),其特征在于对PSMA具有高结合亲和力,因此可导致其NAALADase/谷氨酸羧肽酶II活性的抑制 [19]。随后另一研究组将A10适配体结合到胶束表面,在体外和体内研究中显示PSMA阳性的癌细胞中对药物高摄取 [20]。

PSMA可用作递送治疗剂的靶标,例如抗体-药物偶联(ADC)疗法。PSMA ADC全人型抗PSMA单克隆抗体是通过缬氨酸-瓜氨酸连接物与单甲基金霉素E结合。已有研究小组评估了PSMA ADC在体外前列腺癌细胞系和紫杉类难治性前列腺癌的新型体内模型中的抗肿瘤活性。他们观察到体外细胞毒活性对于具有PSMA表达增加的前列腺癌细胞是有效的(>105分子/细胞;IC50 0.022 nmol/L) [21]。此外,PSMA ADC在治疗异种移植肿瘤方面表现出较高的体内活性,这种肿瘤在之前的多西紫杉醇治疗中取得了进展。

Petrylak等 [22] 报告了基于PSMA-ADC的II期试验数据,该试验以2.5 mg/kg的紫杉烷难治性转移性去势抗性前列腺癌(CRPCa)患者为基础。39%的患者接受了卡巴他赛和多西紫杉醇治疗,58%的患者接受了恩杂鲁胺和阿比特龙治疗。以2.5 mg/kg开始给药并且以2.3 mg/kg调节耐受性。该研究表明,在36% (2.3 mg/kg)和16% (2.5 mg/kg)中观察到PSA下降30%或更多。循环肿瘤细胞(CTC)下降 ≥ 50%在74%和2.3 mg/kg的患者中均有所下降。治疗时间2.3 mg/kg远远超过2.5 mg/kg,以及严重不良事件发生率(37%对59%)。值得注意的是,PSA和CTC下降与较高的PSMA表达+CTC水平相关,而单独的PSA反应与较低的神经内分泌(NE)标记物表达相关,因此表明NE分化可能在此背景下起作用。在此类结果的基础上,本研究进一步扩展(见NCT02020135)。

噬菌体展示技术已被研究人员用于鉴定肽序列,其可与PSMA结合并同时抑制其酶活性。Denmeade等人将PSMA特异性肽与肌浆/内质网钙三磷酸腺苷(SERCA)泵的抑制剂(即Thapsigargin)偶联。这种泵与P型家族的离子动力ATP酶具有相同的催化性质。它将钙离子从细胞质转运到肌肉内质网中,所有类型的细胞都需要其活性。这种结合物保持无活性直至PSMA特异性肽被切割,从而开始SERCA抑制。在异种移植模型中,毒胡萝卜素在似乎毒性极低的剂量下可诱导肿瘤消退 [23]。基于这些发现,I期研究正在评估晚期前列腺癌和其他实体瘤患者中的毒胡萝卜素前药G-202(NCT01056029)。

PSMA可用于免疫疗法和放射疗法。“基于输注用嵌合抗原受体(CAR)改造的设计者T细胞的过继免疫疗法”来增强它们的抗肿瘤活性可以作为治疗癌症的高度特异性方式。因此PSMA + CD3双抗体能够重新靶向人CD4+和CD8+淋巴细胞以裂解表达PSMA的C4-2前列腺癌细胞。其他第一代和第二代抗PSMA设计的T细胞已在体外和体内研究中显示出它们的活性 [24]。最近,2C9 (KM2777)的小鼠–人嵌合IgG1已经可以与C–末端白细胞介素-2(IL-2)融合。在使用表达PSMA的前列腺癌细胞的异种移植肿瘤模型中,这种融合体KM2812显示出明显的抗肿瘤活性,在某些情况下肿瘤可完全消退 [25]。双效特异性抗体已用于人临床试验。I期试验研究了针对PSMA的自体T细胞过继转移的安全性,用于治疗去势转移性前列腺癌患者(NCT01140373)。疫苗是另一个非常重要的领域,它利用PSMA作为靶标来增加针对肿瘤细胞的细胞和体液免疫应答 [26]。

关于PSMA靶向抗血管放射治疗的潜在作用,Bandekar等。已经评估了负载有α–粒子发生器225Ac的脂质体以选择性方式杀死PSMA阳性PCa细胞。在这项研究中,抗PSMA靶向脂质体已经能够杀死PSMA阳性细胞,包括表达PSMA的内皮细胞,因此提示它们用于选择性抗血管放射治疗 [27]。关于PSMA靶向抗血管放射治疗的潜在作用,Bandekar等,已经评估了负载有α–粒子发生器225Ac的脂质体以选择性方式杀死PSMA阳性PCa细胞。在这项研究中,抗PSMA靶向脂质体已经能够杀死PSMA阳性细胞,包括表达PSMA的内皮细胞,从而表明它们用于选择性抗血管放射治疗。

4. 结论

PSMA的发现为前列腺癌的早期检测及治疗提供了新的发展方向。进一步采用免疫组化方法检测PSMA,可作为评估转移性PCa的预测标志物,以指导临床医生选择最适合个体患者的成像技术和治疗方法。但选择新出现的PSMA靶向示踪剂和治疗剂需要进一步研究,以确定疾病的不同部位和阶段的最特异性化合物。随着我们对PSMA在前列腺癌发生中的作用的理解和分子技术的进一步完善,PSMA将在PCa患者的诊断和治疗中发挥关键作用。
