Abstract:
Rheumatoid arthritis
processes are based on a sustained regulated communication network among different
cells types. This network comprises extracellular mediators such as cytokines, chemokines and matrix
degr ading prote-
ases, which orchestrate the participation of cells in the chronic inflammatory process.
Intracellular
transcription factor
pathways
are
corresponding with this outside communication world
, which
transfer
information about inflammatory
stimuli to the cell nucleus. This review examines the function of one key signal transduction pathway of

mitogen

MAPK signalling pathw ay is co nside red crucial
for the induction and maintenance of chronic inflammation, and
thus
its components emerge as interesting molecular
targets of small molecule inhibitors for controlling inflammation. This review not only summari

MAPK pathway but also examines the role of this pathway in
rheumatoid arthritis.

要:
类风湿性关节炎过程涉及多细胞间的信号网络持续的调控。该网络包括胞外介质如细胞因子、趋化因
子和基质金属蛋白酶,其协调各细胞参与慢性炎症过程。相对应的是胞内转录因子通路,其主要将炎症刺激信
息传递给细胞核。丝裂原活化蛋白激酶
(MAPKs)
信号通路中
p38
MAPK
信号通路对慢性炎症的诱导及维持起至
关重要的作用,因此针对该通路上的分子靶点研制出相应的小分子抑制剂用于控制炎症发展。本文通过总结
p38
MAPK
通路的多种功能作用和活性调节,对其在类风湿性关节炎中的作用做简要综述。

引言
细胞对外界刺激的响应部分需要细胞膜受体和
细胞核间信息传递。所以快速和完整的信息传导对细
胞适应和生存至关重要。胞内信使即充当了外界环境
和细胞核内基因之间的信息传递者。丝裂原活化蛋白

主要由胞外应激因子调节,
ERK
是丝裂原刺激物的作用靶点。之前的研究介绍了另两
条
MAPK
通路
(JNK
和
ERK)
在类
风湿性
关节炎
(rheumatoid arthritis)
中的作用

。本文着眼于

MAPK
信号转导通路及其功能作用。

MAPK
通路的诱导因素
p38
MAPK
分为四类亚型:
p38
MAP K

是唯一一个能够影响
p38
MAPK
的
MEKK
,其仅在滑膜细胞上有微弱的表
达。与之相反的是凋亡信号调节激酶
1(apoptosis signal
regulating k in ase 1, ASK1)
,一个由凋亡刺激剂诱导产
生的
MEKK
,不仅可以强烈诱导
MAPKKK
磷酸化

而且也能够在
RA
的滑膜细胞中检测到

MEKKs

的活化是通过

MAPK
活化尤为重要。

和
MKK6
共享
82
%
的氨基酸同源性但对与不同组织
和细胞类型的表达存在差异

的优先激活顺序:渗透应力通过优先

触发
p38
MAPK
活化,而成纤维样滑膜细胞和炎性细
胞因子接触后

的激活优先由

主要表达于滑膜衬里层,这也是

的活化位点。炎性关节组织中的成纤维样

的活化
p38

通路在
RA
滑膜被强烈激活,而在退
行性关节炎患者滑膜未检测到活化的
p38
MAPK
[4]
。
炎性滑膜组织中大约三分之一的细胞参与激活
p38
MAPK
通路。活化的
p38
MAPK
主要在滑膜微血管内
皮细胞及滑膜衬里层的细胞中表达。除了内皮细胞,
巨噬细胞和成纤维样滑膜细胞也是
p38
MAPK
活化发
生的主要细胞类型,而在淋巴细胞中却观察不到
[4]
。
同样,在关节炎动物模型的滑膜组织中
p38
MAPK
的
活化也与巨噬细胞,成纤维样滑膜细胞和血管内皮细
胞紧密相关。此外,破骨细胞也可在关节炎模型动物
滑膜侵袭位点表达激活的
p38
MAPK
[21]
。
TNF
、
IL
-1
和
IL
-6
是成纤维样滑膜细胞中
p38
MAP K
的强效诱导
物
[4]
。

MAPK
的亚型
到目前为止,共报道有
4
种不同的
p38
MAPK
亚
型,而这些亚型在疾病中表达和激活方式的差异还鲜
有报道
[22,23]
。因受到
LPS
刺激后络氨酸位点被迅速磷
酸化,从而发现第一个

成员

MAPK

,它主要参与炎性细胞因子如:

可被某些上游激酶如

细胞因子也

激活这四大类亚型
[5,24,26]
。因此
p38
MAPK
四个成员有着类似的关键功能。但是各亚型间
在激活方式、调节和抑制、底物特异性和某些功能方
面存在差异。

MAPK
四个异构体的表达和激活对
RA
滑膜
组织的
研究显示
p38
α
和
γ
-
亚型在
RA
患者炎症部位
-
滑膜组织中表达量最为丰富。滑膜组织代谢最活跃的
位点:内皮细胞和滑膜衬里层

中发现了,除
p38
δ
以外的,各磷酸化的
p38
MAPK
亚型
的表达。位于衬
里层和滑膜微血管之间的衬里下层未检测到
p38
MAPK
的四个亚型
,但
MAPK
家族其他成员:
JNK
和
ERK
均检测到不同程度的表达

。内皮细胞主要表
达的
p38
MAPK
亚型是
p38
γ
,少量
p38
α
和
β
也检测
到参与表达,唯一未参与表达的
p38

检测到在滑膜
微血管壁的肌纤维细胞内表达。而在衬里层中,巨噬

在炎症滑膜中的作用
p38
MAPK
可影响多种细胞因子的产生
[5,6]
,其
通
过影响促炎和抗炎机制的平衡,最终导致生理自限性
炎症或慢性炎症发生。选择性阻滞
TNF
、
IL
-1
和
IL
-6
的表达能通过抑制关节炎症和减轻对炎性组织结构
的破坏来控制疾病,这也说明细胞因子的失衡是
RA
的关键诱因之一
[33]
。
p38
MAPK
对细胞因子的调节作
用及
p38
MAPKs
作为相同细胞因子的重要胞内信使
作用为开发合成抑制剂提供了理论基础。这些化合物
对于关节炎模型动物的确具有抗炎作用,但也面临药
物毒性等早期临床实 验的 难题
[30,34,35]
。除了调节炎性
因子表达外,
p38
MAPK
还可
控制细胞周期和凋亡,
诱导一氧化氮合成酶来调节氧化反应
[36]
以及嗜中性
粒细胞活化作用。研究者阻断炎症模型动物中
p38
p38 MAPK

通路后发现动物炎症症状减轻,
滑膜炎性组织
的形成减少,特定细胞成比例增加,与之相反的是滑
膜细胞数量明显减少。这些结果指出滑膜炎症的抑制

MAPK
的阻滞得以实现而不是因为
某些细
胞的特异性效应,从而得出结论:
p38
MAPK
通路是
决定滑膜炎症严重程度的重要因素。

p38
MAPK
在软骨损伤中的作用
软骨损伤是
RA
的标志之一。损伤发生是由于蛋
白聚糖损失加剧以及炎症组织附着并侵入软骨,最终
导致软骨结构分解。滑膜组织产生的基质金属酶
(matrix metalloproteinases, MMPs)
是炎症组织侵入和
破坏软骨的关键前提
[37]
。
MMPs
的合成受到多种蛋白
激酶家族的调控,其中也包括
p38
MAPK
。这也表明
阻断
p38
MAPK
对治疗关节炎有利
[38,39]
。的确,
p38
MAPK
抑制剂能够明显减轻炎症模型动物软骨分解
[30]
,但这种影响是直接通过
p38
MAPK
对
MMP
s
调
节作用还是间接由于炎性因子的低表达目前仍不清
楚。

p38
MAPK
在炎症性骨损失中的作用
炎性骨质破坏也是类风湿性关节炎的重要表征
之一。从局部骨侵蚀的形成到结构破坏最终导致骨关
节功能的丧失。破骨细胞的形成是关节炎骨损伤的成
因之一

,其形成主要受造血前体细胞的细胞因子
(RANKL
和
TNF-
α
)
刺激的调节,这些细胞因子激活
p38
MAPK
通路的同时亦可受其影响。因此
p38
MAPK
的活化对成骨细胞的形成尤为重要
[41]
。研究者
在阻断实验性关节炎模型动物
p38
MAPK
通路后,滑
膜组织中成熟破骨细胞和破骨细胞前体明显减少
[30]
。
因此使用
p38
MAPK
抑制剂可以有效阻断炎性组织侵
蚀邻关节骨。反之当
p3 8

信号异常
时,
MKP
-1
表达减少导致破骨细胞数量和大小的增加

。

MAPK
对内皮细胞功能的作用
活化
的
p38
MAPK
对内皮细胞的功能有以下几个
作用:

调控血管内皮细胞中黏附分子表达,

选择素、血管细胞黏附分子
(VCAM

加剧中性粒细胞和单核细胞与内皮细胞的粘附,诱发
炎症反应及血栓形成

信号通路亦可以
影响多种炎性因子的产生,还可介导中性粒细胞的活
化,中性粒细胞炎性聚集很大程度上依赖
p38
的激活。
p38
MAPK
活化后,促进钙内流,引起内皮细胞渗透
性增大,加速中性粒细胞游走,放大炎症反应
[

MAPK
信号通路激活后可增加细胞内
NO
的产生,使
细胞内诱导型一氧化氮合酶
(iNOS)
的浓度增加。
iNOS
可调控
NO
合成而且其本身是
p38
MAPK
的靶基因,
这就促进了血管舒张。
NO
诱导的血管舒张可增加炎性
组织的血流量并促进白细胞迁移至这些位点。

结论和展望
p38
MAPK
是
RA
的一个关键信息传递媒介。在
不同的刺激的作用下,信号通过磷酸化的级联反应传
递给
p38
MAPK
,而
p38
MAPK
可以通过磷酸化或者
非磷酸化的方式激活下游的蛋白,从而使细胞做出响
应。近年来对关节炎中
p38
MAPK
的表达、调控和病
理生理作用的研究大幅增加。目前对于
p38
的结构与
功能了解的已经比较清楚,一些新的观点也为抑制
RA
的炎症发展和组织损坏提供合理的治疗靶点。可
以预期随着各种研究的进展已知的
p38
MAPK
的各种
功能也将逐渐被运用到临床。
