Noninvasive Selective Brain Cooling by Head and Neck Cooling Is Protective in Severe Traumatic Brain Injury

ABSTRACT

Therapeutic hypothermia is a promising treatment for patients with severe traumatic brain injury (TBI). We present here the results of a study in which noninvasive selective brain cooling (SBC) was achieved using a head cap and neckband. Ninety patients with severe TBI were divided into a normothermia control group (n = 45) and a SBC group (n = 45), whose brain temperature was maintained at 33 - 35 degrees Celsius for 3 days using a combination of head and neck cooling. At 24, 48 and 72 h after injury, the mean intracranial pressure (ICP) values of the patients who underwent SBC were lower than those of the normothermia controls (19.14 ± 2.33, 19.72 ± 1.73 and 17.29 ± 2.07 mmHg, versus 23.41 ± 2.51, 20.97 ± 1.86, and 20.13 ± 1.87 mmHg, respectively, P < 0.01). There was a significant difference in the neurological recovery of the two groups at the 6-month follow-up after TBI. Good neurological outcome (Glasgow Outcome Scale score of 4 to 5) rates 6 months after injury were 68.9% for the SBC group, and 46.7% for the control group (P < 0.05). There were no complications resulting in severe sequelae. In conclusion, the noninvasive SBC described here is a safe method of administering therapeutic hypothermia, which can reduce ICP and improve prognosis without severe complications in patients with severe TBI.

无创性头颈部降温法治疗重型颅脑损伤患者的神经保护作用

摘 要

研究无创性头颈部降温法作为无创性选择性脑亚低温方法在治疗重型颅脑损伤的临床效果。将90例重型颅脑损伤患者分成选择性脑亚低温组和常温组。利用亚低温治疗仪,对重型颅脑损伤患者采取头颈部密切敷贴方法取得无创性选择性脑亚低温(33˚C~35˚C)维持3天后自然复温。密切监测两组患者的颅内压等指标。结果显示,脑亚低温组、常温组在24、48、72 h的平均颅内压分别为19.14 ± 2.33 mmHg,19.72 ± 1.73 mmHg,17.29 ± 2.07 mmHg和23.41 ± 2.51 mmHg,20.97 ± 1.86 mmHg,20.13 ± 1.87 mmHg,前者较后者降低(P < 0.01);6个月后2组恢复良好率(GOS评分4~5)分别为68.9%和46.7% (P < 0.05),未见严重的并发症。本研究选用的头颈部降温法可取得选择性脑亚低温目标。该方法在治疗重型颅脑损伤患者中有降低颅内压,改善预后,而未见严重的并发症,是一种安全有效的治疗方法。

1. 引言

尽管医学不断发展,重型颅脑损伤的治疗依然棘手 [1] [2] [3] [4] [5] 。亚低温治疗对重型颅脑损伤是一种非常有前景的治疗方法 [2] 。临床报道全身亚低温治疗可引起全身并发症如感染,临床效果不佳 [1] [2] [4] [5] [6] 。选择性脑亚低温可减少全身亚低温治疗的并发症,并可尽早达到“脑部亚低温”的作用 [1] [2] [3] [4] 。为了进一步研究选择性脑亚低温对重型颅脑损伤的效果及安全性,我们开展了无创性头颈部降温法作为无创性选择性脑亚低温方法,现报告如下。

2. 研究方法

2.1. 一般资料

2002年1月~2010年12月,伤后24 h以内的重型颅脑损伤患者90例,其中男59例,女31例,年龄18~65岁,平均41.8岁,随机分成2组:选择性脑亚低温组45例,常温组(对照组) 45例。2组患者均符合以下的标准:颅脑外伤史、GCS:3~8分,伤后0.5~4.5 h (平均为3.7 h)头颅CT确诊颅脑外伤。同时合并其他脏器损伤所导致的严重休克及窒息等情况病例排除。2组主要临床资料无统计学差异,具有可比性(见表1)。所涉及的研究方法家属均知情同意,且经我院伦理委员会批准。

2.2. 治疗方法

确诊后患者即进行手术或即刻进入病房给予甘露醇脱水等处理。选择性脑亚低温组根据患者的情况

在入院时或手术后0~5.6 h (平均4.1 h)即给予亚低温治疗。具体方法见文献,保持有效降温3日后撤去降温物自然复温 [7] 。常温组除保持体温维持在 37.5 ℃ 以内外,其余治疗措施如甘露醇脱水、必要的置管、保持呼吸道通畅、防治低氧血症,敏感抗生素防治感染、维持内环境稳定以及一些支持性治疗如定期翻身和活动肢体等基础护理,防治深静脉血栓形成等措施与亚低温组患者相同。

2.3. 监测指标

2组患者在入院或手术后即行以下各项指标监测:(1) 脑温、直肠温度监测;(2) 颅内压力监测;(3) 心率、呼吸、血压、血氧饱和度监测;(4) 并发症:如肺炎、消化道出血、血小板减少、电解质紊乱等;(5) 出院后1年的GOS评分。

2.4. 统计方法

颅内压力以± s (均数 ± 标准差)表示。数据符合方差齐性后进行单因素方差分析,组间差异显著后各组均数比较采用t检验。2组死亡率和恢复良好(GOS:5~4分)率的比较采用卡方检验。与预后有关的主要临床的数据分析是采用多变量回归以及逻辑回归分析。利用SPSS 10.0统计软件,P < 0.05为有统计意义。

3. 结果

3.1. 温度

采用1: 1随机数字表入组90例患者,亚低温依据前述的方法进行治疗方法方便可行。选择性脑亚低温组患者脑温和直肠温度分别控制在33℃~35℃和 37.5 ℃ 左右;亚低温治疗结束后,脑温在8.4~20.6 h内恢复到治疗前。对照组脑温及直肠温度分别控制在 37 ℃ 38 ℃ 左右。

3.2. 颅内压

选择性脑亚低温组在治疗后24、48和72 h的颅内压较常温对照组明显降低(P < 0.01)见表2

3.3. 两组患者预后比较

亚低温与常温组相比较有显著差异(P < 0.05),预后较佳(GOS评分4~5分)率分别为60%和40%,明显好于常温组(P < 0.05) (表3)。

4. 讨论

我们可以通过以下两种方法来达到脑的亚低温:系统(全身)性的冷却以及局部(选择)性的冷却。许多临床技术可以用来完成全身的亚低温:降温毯、冰袋、气褥垫、冷却管、静脉输注致冷剂、腹腔灌洗冷水、心肺转流术等。在大多数的研究当中,都是通过降温毯合并冰袋来达到全身的亚低温的,相比于全身的亚低温存在的严重的并发症(如局部缺血和出血倾向等),选择性亚低温已经被临床实验证实具有治疗作用而且副作用相对较 [7] 。

对于恒温动物来说,有两种不同的选择性脑亚低温的方法,即通过动脉血或静脉 [7] 。选择性脑亚低温的治疗方法也可通过多种方法达到效果。鼻咽以及硬膜下腔的通过冷生理盐水灌洗,或者通过血管内(静脉或动脉)灌注冷晶体液。通过局部冷生理盐水冲洗冷却也能改善脑损伤。然而,在成人中,鼻咽以及硬膜下腔隙的冷却对于降低脑温的作用是非常有限的。血管内冷却方法比全身的表面降温效果应该更好,然而,该方法的技术要求相对较高,并且可导致血栓形成等严重并发症。

本研究显示,我们所选用的无创性选择性脑亚低温方法是有效和安全性的。通过使用一种新型的冷却剂,我们可以多次地使用这种方法。因为深部组织的温度主要取决于动脉血温度以及大脑的代谢热,而很少取决于血流量,降低了进入大脑血管的温度,故达到了有效的选择性脑亚低温的目标。而且,目标温度可以在2 h之内就达到,起到了快速降温的目的。在本研究中,选择性脑亚低温组患者的脑温被控制在33℃~35℃,直肠温度被控制在37.5℃左右,这样,因为亚低温而导致的由于血管收缩引起的其他器官的并发症就可以被避免了。研究也表明,在头部的冷却过程中,身体其他部位的温度可以保持不变。我们的研究也证实,选择性脑亚低温组的颅内压明显低于对照组,而颅内压的升高在重型脑外伤中

是影响预后的一个重要因素。另外,选择性脑亚低温组患者除了肺炎的发生率增高外,没有严重的副作用产生。

近年来认为,轻度亚低温(33℃~35℃)具有治疗作用而且副作用相对较少。研究表明,亚低温治疗主要通过减轻脑缺血及继发性脑损害,控制颅内高压,改善脑灌注压来保护脑组织,并且有减轻全身炎症反应等的作用 [1] [2] [3] [4] [6] 。本研究对重型颅脑损伤患者进行的亚低温治疗,具有无创、有效的特点。降温毯内的冷水温度可根据患者的情况进行调整,以保证脑温在亚低温治疗期间控制在33℃~35℃左右。本组治疗后未见与亚低温疗法相关的严重并发症。

当然,由于重型颅脑损伤的病理生理变化复杂,亚低温治疗并非适用于所有患者,部分研究还表明,亚低温治疗较常温治疗临床效果差 [1] [2] [4] [8] 。我们的初步研究表明,本研究中的选择性脑亚低温法为无创性,可以用于脑温的降低及维持,并具有降低颅内压和改善临床预后。虽然主要的并发症是肺炎,但是经过处理后未见严重的后遗症。因此,该无创性选择性脑亚低温在临床上治疗重型颅脑损伤时,是安全有效的。进一步的研究需明确选择性脑亚低温治疗的作用机理(包括副作用) [4] [5] [9] [10] [11] [12] ,选用适当的多参数监生理生物学指标进行有效的监测7,有助于确定颅脑损伤的诊疗,改善患者的预后 [5] [13] [14] [15] 。
