Advances in Research of Repetitive Transcranial Magnetic Stimulation for Post-Stroke Depression

ABSTRACT

Post Stroke Depression (PSD) is a long-term and significant depression caused by neurological deficits. It can be accompanied by a series of clinical manifestations such as sleep disorders, anxiety, mood changes, and cognitive disorders. It can directly affect the patient's neurological rehabilitation and mental and psychological stability. Depression changes severely increase the hospitalization rate, disability rate and mortality rate of patients after stroke. Routines are mainly anti-depressant medications, as well as psychotherapy and other ancillary means, but antidepressants sometimes benefit less or ineffective PSD patients, and even side effects such as side effects lead to disease progression. In recent years, Repetitive transcranial magnetic stimulation (TMS) has been popular in a state with few side effects and considerable curative effect. Many authors have applied this technique to the treatment of post-stroke depression in order to minimize the drawbacks that the patients are the biggest Benefit. After consulting the literature, it was found that this treatment has its own opinions. To this end, this article mainly reviews rTMS treatment of post-stroke depression and accompanying various clinical symptoms, analyzes the research progress of its treatment, and plays a role in suggesting clinical work.

重复经颅磁刺激治疗卒中后抑郁的研究进展

摘 要

卒中后抑郁(Post Stroke Depression, PSD)为神经功能缺损后所致的以持久且显著的心境低落为主,可伴有睡眠障碍、焦虑、心境改变、认知障碍等一系列临床表现的疾病,可直接影响患者神经康复和精神心理稳定,抑郁改变严重增加卒中后患者的住院率、致残率以及死亡率。常规主要为抗抑郁药物治疗,以及心理治疗等辅助手段,但抗抑郁药有时对PSD患者获益较小或无效,甚者副作用等不利因素导致疾病发展恶化。近年来,重复经颅磁刺激(Repetitive transcranial magnetic stimulation rTMS)以其副作用少、疗效可观备受欢迎,不少作者将此技术应用于卒中后抑郁的治疗,以求最低的弊端促使患者最大获益。通过查阅文献后发现此治疗手段众家见解各有千秋。为此,本文主要对rTMS治疗卒中后抑郁及伴随各临床症状展开综述,分析其治疗的研究进展,为临床工作起到可能性提示作用。

1. 引言

一项全球回顾性研究警示全球疾病负担并非乐观,于2016年脑卒中位居全球第二致死因素,总死人数约达550万,其高发病率以中国为主(每年354人/10万人)。标准化后发病率、死亡率虽有下降趋势,但随着人口增长、老龄化及卒中治疗手段的发展,急性期存活人数、慢性期管理数量以及卒中所致伤残减寿年数的绝对数量仍在不断增加 [1]。急性期治疗手段已有众多明确的证据指引,但是对于脑卒中并发症的治疗和管理仍旧是我们每个临床医生不该忽视的阶段,尽早的神经功能恢复、认知功能改善以及精神心理稳定能较好的提高脑卒中患者的生命质量、社会参与度,以及减轻家庭、社会、国家医疗资源的经济负担。卒中后抑郁作为脑卒中后一项高发临床症状,结合患者及家属缺乏相关疾病的认识和严重性,缺乏一定的就医愿望,进而影响患者进一步康复,甚者抑郁程度占据整个疾病恶化进程,导致患者自杀观念抢先,对家庭、社会带来不可忽视的影响和负担。重复经颅磁刺激(rTMS)是一种安全、无创伤、无疼痛的新型治疗技术,对治疗心境障碍、帕金森等疾病,取得较好成果,且经美国于2007年FDA批准认证为一种新型抗抑郁方法 [2]。

2. 重复经颅磁刺激治疗PSD的机制

2.1. 生理学机制

2.1.1. Ca2+依赖性调节

rTMS启动神经元中的动作电位和(或)改变神经元水平刺激期间的神经兴奋性,还可能通过对诱导膜的修饰表现出其对静息电位和阈值的作用,尤其对通道特性改变细胞门控组件的活动、突触连接和(或)时序动态。rTMS可对Ca2+动力学的变化和Ca2+依赖性酶的激活,调节谷氨酸AMPAr/NMDAr的表达,以及即刻早期基因的诱导 [3] [4]。

2.1.2. 神经递质释放性调节

急性rTMS显着增加大鼠中5-HT1A受体的表达前额叶和扣带皮层,但并不会导致5-HT量的显着变化 [5]。慢性rTMS,降低突触前5-HT1B和5-HT1A自身受体的敏感性有助于进一步增加突触间隙的5-HT水平。在人类中,脑TMS-SPECT病例对照研究表明r TMS治疗有影响5-羟色胺能系统,特别是在左背外侧前额叶皮质上的HF (10 Hz) rTMS [5]。

2.1.3. 参与神经营养因子水平性调节

神经营养因子(NTs)构成一个主动调节突触可塑性蛋白质的家族,促进神经元存活和分化,但也促进细胞凋亡。在AD大鼠模型中,低频rTMS通过上调海马NGF,BDNF和NMDAr的表达来逆转淀粉样蛋白β1-42诱导的认知缺陷。长期rTMS增加了BDNF水平和小鼠单克隆抗体的总体生存能力海马HT22细胞,从而确保对氧化应激物等的神经保护作用作为Aβ和谷氨酸 [6] [7]。

2.1.4. 对神经内分泌系统调节

健康和疾病中大脑功能的调节严格依赖于活动内分泌系统。对压力的生理反应需要激活交感神经肾上腺素和下丘脑-垂体-肾上腺(HPA)系统。HPA轴对神经功能的影响包括一组复杂的直接功能前额皮质(例如,DLPFC,亚系扣带,内侧PFC),下丘脑,垂体和肾上腺之间的负反馈类型,还与其他脑网络(如基底神经节)具有双向影响(例如,伏隔核,纹状体)和边缘系统(例如,海马,杏仁核)。接受MANUSCRIPT特定神经激素,神经介质和神经递质如ACTH、谷氨酸、GABA、多巴胺、5-羟色胺)。在急性应激条件下,下丘脑释放促肾上腺皮质激素释放因子(CRF)到垂体腺,激活ACTH释放然后刺激皮质醇从肾上腺释放 [8] [9]。皮质醇会影响其通过负反馈回路进行分泌产生以及大脑活动,尤其是在高反应区域糖皮质激素受体的密度,如额叶区和海马区。Herbert通过对动物研究发现皮质醇可调节神经元的生长及重塑,其持续升高对糖皮质激素受体皮层区域产生毒性作用,导致神经元细胞功能降低、变性、死亡 [10]。刘晓丹等对30例首发抑郁患者研究发现抑郁患者左左外侧前额叶眶回(lOFC)、左侧舌回(LC)、右侧前喙扣带皮层(rACC)、右侧颞下回(ITC)的皮层厚度与皮质醇负相关,证实这些区域可能含有一定数目的糖皮质激素受体较易受到高皮质醇细胞毒性作用的影响 [11]。

2.1.5. 对炎症,氧化应激和预防神经细胞死亡的调节

在中枢神经系统中,对外部应激源的神经炎症反应的特征在于激活小神经胶质细胞和星形胶质细胞,导致促炎症的级联释放介质(谷氨酸,ATP,IL-1β,TNFalpha)和自由基的产生,包括活性氧(ROS),反过来,其维持神经炎症反应 [12] ,这个过程被称为反应性胶质增生和随后的突触重塑,突触的破坏连接,神经递质稳态失衡和谷氨酸兴奋性毒性导致适应不良的突触可塑性 [13]。神经胶质细胞的激活是损伤后恢复的关键步骤,非侵入性刺激技术可以代表一种调节神经胶质激活的新策略,如a-tDCS和c-tDCS诱导大鼠短暂激活(5~10天)的驻留小胶质细胞,调节神经干细胞的募集和增殖,参与恢复的机制脑损伤后 [14]。关于细胞死亡,在血管性痴呆的大鼠模型研究表明低频率rTMS调节凋亡信号传导(Bcl-2的上调和Bax的下调表达),表明一种旨在保护突触的活动 [15]。

2.1.6. 参与神经胶质网络的调节

突触的可塑性导致星形胶质细胞,小胶质细胞,少突胶质细胞的动态神经胶质相互作用,细胞外基质与神经元和血管。神经胶质网络调节通过Ca2+依赖性同步神经元放电过程 [16] 以及与NMDAr依赖的LTP诱导有关 [17] ,释放神经元和神经胶质之间联系因子。在小鼠齿状回中,rTMS诱导胶质纤维酸性蛋白(GFAP)在星形胶质细胞中的转录,GFAP是参与细胞骨架的细胞骨架的中间丝(IF)蛋白形成IF网络 [18]。大多数离子通道和神经递质转运蛋白都是表达于星形细胞胞体和过程,并有助于突触体内平衡。

2.2. 病理可能性机制

本重复经颅磁刺激刺激大脑对应皮层区域,刺激神经元细胞动作电位兴奋状态,促进兴奋性神经递质以及神经营养因子释放,调节大脑兴奋状态以维持兴奋稳定,促进受损部位神经细胞自我修复、重塑以提高机体功能。可改善大脑对应区域血流,促进相互作用区域血供,促进病变区神经兴奋性 [19]。增加卒中患者脑对葡萄糖的摄取和利用,提高能量供应,改善缺血再灌注、自由基等损伤后的功能恢复 [20]。

3. 重复经颅磁刺激(rTMS)对卒中后抑郁的治疗

3.1. 对单纯抑郁的治疗

Robinson报道提示30%~35%脑卒中患者可在第一年内发生PSD,且在未予治疗下发病率、致残率更高 [21]。Xin等一项荟萃分析中,指出阳性结果表明rTMS具有对PSD的有益影响。但其异质性和潜在的偏见,应慎言其结果 [22]。另一项研究将重复经颅磁刺激(rTMS)作为卒中后抑郁(PSD)患者的附加疗法研究中显示效果客观。从张婧,刘超猛等不同meta分析中指出rTMS可较为显著的改善PSD前后抑郁症状,提高生活质量,且不良反应相对小,未见其严重特性 [23] [24]。对于rTMS治疗期间不同频率限定亦有不同看法,根据杨柳,杨琪各自meta分析指出,在治疗4周后,高频治疗相比低频治疗效果会更显组,且安全有效 [25] [26] ,但据Liu等研究发现虽高频治疗对抑郁有显著正效应,相比对照组头痛则更易出现 [27]。亦有报道rTMS联合高压氧、有氧运动等对PSD有较好的作用,且未发生不良反应,依从性较好,治疗相对安全、可行和耐受 [27] [28] [29]。

3.2. 对于PSD伴随睡眠障碍的治疗

文献报道约有34%~76%的卒中后患者可致不同程度的抑郁状态,而其可直接影响PSD患者睡眠质量 [30]。睡眠障碍已成为卒中后患者常见继发改变,可表现为入睡困难、睡眠不实或节律紊乱,相比非睡眠障碍患者,其明显影响患者日常生活能力,且抑郁与睡眠障碍互为影响,严重影响患者神经功能恢复 [31]。高频具有兴奋皮质的功能,低频则抑制兴奋,但可促进5-HT和GABA释放,调节觉醒周期,促进睡眠增加 [32]。通过查阅文献,并未阅及外文对于PSD伴随睡眠障碍的治疗报道,不排除数据库检索偏倚,中文数据库有此类报告。王韵喃等对120例患者随机试验中同样得出显著正效应临床意义,对于伴随睡眠障碍的治疗报道有限,结论应慎言而定,尚且仍需进一步多中心、大数据研究分析 [33]。

3.3. 对于PSD伴随运动障碍的的治疗

脑卒中后患者在一定意义会遗留不同程度肢体偏瘫、吞咽功能等情况,严重影响患者正常生活,而抑郁情绪改变患者社会交际以及降低生活兴趣,偏瘫肢体缺乏康复刺激训练导致病变应对肢体失用性萎缩,抑郁和运动障碍互为影响,增加康复难度,降低肢体功能。外文报道对患者双侧大脑M1区予以rTMS治疗有易于患者运动障碍肢体功能康复 [34] ,rTMS可促进皮层网络功能连接,刺激皮层功能重塑,提高功能恢复,不同频率亦有不同结果,Sasaki等团队研究指出健侧高频是优于患侧低频治疗近端肢体功能,而远端肢体如手功能恢复不如低频 [35]。外文研究将30例样本随机分配,以低频、中频、高频实验组与对照组观察结构发现,对于吞咽功能障碍低频能更显著改善症状 [36]。对于PSD后伴随肢体功能障碍、吞咽障碍等,rTMS提示有较好的效果,但是具体的治疗模式、参数指标、频段都尚有待进一步规范共识,以求更好的方式对患者提供有力治疗依据。

3.4. 对于PSD伴随认知障碍的治式

脑卒中主要为血管功能障碍导致缺血或出血引发的一系列神经脑功能障碍,神经细胞坏死后不可恢复性,可降低发病后患者在记忆、语言、思维、计算、执行功能能方面的认知改变,甚者引发血管性痴呆,严重降低患者生命质量,影响其正常生活。左侧前额叶、背外侧区、右侧前额叶及其背外侧区区域兴奋性提高,可刺激神经递质释放,皮质间联系,促进认知改善。Sato报道予以左前额叶背外侧rTMS低频刺激,可改变皮层神经兴奋与抑制协同效应来改善认知 [37]。Galletly对63例样本采用计算机化认知测试评估治疗前后的认知改变,评估汉密尔顿抑郁评定量表评分从基线到治疗后有显着下降,然而,当抑郁症状的变化作为协变量被包括在内时,研究进一步支持了rTMS作为患者治疗选择的有效性和安全性。但是目前的研究表明,rTMS没有明显的认知障碍,认知功能的任何改善都与抑郁症状的减少有关 [38]。Rostami等回顾性研究显示rTMS对于试验样本在治疗后,其记忆、执行功能、注意力和抑郁状态出现明显变换,表明rTMS可能与之相关改善抑郁症患者的认知功能 [39]。但仍需进一步大数据验证以求最合理证据支撑。

4. 讨论

抑郁状态是缺血性卒中后最常见的精神并发症,对幸存者的功能康复和生活质量产生很大负面影响。其中约30%患者受卒中后抑郁的影响,使卒中后抑郁成为一个严重的社会和公共卫生问题 [20]。抑郁症和中风之间存在双向关联:中风增加了PSD的风险,但抑郁症又是中风的独立危险因素。PSD的病理生理学可能是多因素的,涉及在心理社会痛苦的背景下各种缺血诱导的神经生物学功能障碍的组合。几个证据表明,在急性缺血性卒中的神经炎症反应,下丘脑-垂体-肾上腺(HPA)轴的应激激活和能量代谢改变背景下的适应性反应(神经发生)受损(即线粒体功能障碍)之间存在关系。

就目前而言,特别是选择性5-羟色胺(5-羟基色胺,5-HT)再摄取抑制剂(SSRIs)主要被证明在预防和治疗PSD方面具有临床活性,但其作用机制有待进一步研究,且不排除增加颅内出血的可能 [40]。重复经颅磁刺激(rTMS)经常被用来增加功能恢复中风患者,rTMS可以改变刺激的大脑皮质部位的兴奋性以及沿着功能解剖学的远程结构连接 [39]。rTMS治疗是基于脑卒中后半球间平衡受损的概念 [41]。rTMS已被证明对治疗中风并发症是安全有效的。通过将兴奋性或抑制性电磁脉冲分别应用于病灶的同侧或对侧的半球,以及在调节半球间通信的经胼approach体通路水平,可以优化功能性大脑活动。León研究指明rTMS对卒中后导致运动障碍,失语症,构音障碍,口咽性吞咽困难,抑郁和感知-认知缺陷有明显改善 [42]。但Nam等一项长期研究中发现rTMS对运动功能没有显着影响或中风后6个月的残疾,还指出高频率rTMS不会产生持久的(中风后6个月)中风后运动功能和残疾的改善 [43]。当然进一步大样本量的随机对照临床试验仍是必然,其能以推荐更高水平的证据,在广泛的基础上正确实施rTMS在卒中受试者中的使用。

综上所述,对于目前rTMS对治疗PSD的机制尚未明确,其虽对于PSD以及各种伴随症状的疗效各有不同说话,但总体而言,疗效显著正效应结论占比高于低效应或无效可能。当然其定论尚需慎重,仍旧需进一步验证其结论。相信随着科学技术的发展对rTMS的进一步研究探索能较好的应用于PSD的治疗。
