Progress in Study on GnRH Agonist Protect Ovarian Function during Chemotherapy

ABSTRACT

With the increasing survival rate of cancer patients, they have higher requirements for their living quality. Chemotherapy treatment is the mainstay of tumour, but it induces ovarian damage. How to protect ovarian function during chemotherapy, protect fertility, prevent premature ovarian failure need to be solved urgently. GnRH-a has several advantages: it is widely available and inexpensive, and does not require ovarian stimulation or an invasive surgical procedure. However, the use of hormonal therapy to protect ovarian function from the effects of chemotherapy is the subject of many controversial debates and conflicting clinical reports with no clear consensus. The purpose of this review is to discuss the protective effect of GnRH analogues on ovarian function in chemotherapy of malignant tumors.

摘 要

随着癌症患者生存率的不断提高,癌症患者对自己的生活质量也有着更高的要求。化疗作为目前恶性肿瘤的主要治疗方式,其对卵巢的毒害作用越来越引起人们的重视。如何保护化疗患者的卵巢功能,保护其生育力,预防卵巢早衰成为临床医生亟需解决的难题。激素疗法因其使用便捷、经济、无创伤等优势迅速成为研究的热点,但其对化疗过程中的卵巢功能是否能起到保护作用,现医学界并未达成一致共识。本文旨在讨论GnRH类似物在恶性肿瘤化疗中对卵巢功能的保护作用。

1. GnRH与GnRH类似物

20世纪70年代人类首次从猪的下丘脑分离出了促性腺激素释放激素(Gonadotropin-releasing hormone, GnRH) [1] ,此后人们认识到GnRH是下丘脑弓状核神经内分泌细胞分泌的十肽激素,通过垂体门脉系统以脉冲方式释放,与垂体上的特异性受体结合后产生刺激垂体前叶细胞分泌卵泡刺激素(follicle-stimulating hormone, FSH)和黄体生成素(luteinizing hormone, LH)。GnRH高频释放会使LH的合成及分泌多于FSH,低频释放则会使FSH的合成及分泌多于LH。同时GnRH的释放和FSH、LH的水平也受到卵巢激素的反馈性调节 [2] 。由此形成的正负反馈在维持女性正常的生殖系统功能中起着重要的作用。研究发现,除下丘脑和垂体外,卵巢、胎盘、子宫内膜、子宫平滑肌和输卵管等外周组织及卵巢癌、子宫内膜癌、乳腺癌、前列腺癌等恶性肿瘤中都有GnRH和GnRH受体的表达 [3] ,这也为后来GnRH类似物广泛的临床作用奠定了基础。通过人为置换GnRH上的第6位、第10位氨基酸,就可得到具有更长半衰期、更耐降解酶的GnRH类似物:促性腺激素释放激素激动剂(gonadotropin releasing hormone agonis, GnRH-a)和促性腺激素释放激素拮抗剂(gonadotropin releasing hormone antagonist, GnRH-ant)。两者都作用于GnRH受体,产生一系列激素相关的病理生理作用作,但作用机制完全不同。

2. GnRH-a对化疗过程中卵巢功能的保护

GnRH-a保持有与GnRH相同的生物活性,但与GnRH受体具有更高的亲和力。研究发现GnRH-a刺激垂体前叶细胞生成FSH和LH的效应是GnRH高的50~100倍,其半衰期可达5 h以上 [4] 。数量丰富的GnRH-a在细胞外与GnRH受体结合形成的二聚体不容易解聚,导致内源性GnRH因无GnRH受体与之结合而无法发挥作用,从而抑制垂体促性腺激素的合成和释放;进而使得垂体释放FSH和LH水平降低,导致卵泡无法发育成熟,从而使其不受化疗药物的破坏 [4] 。首次应用GnRH-a时,机体会出现FSH和LH一过性上升,刺激卵巢激素的分泌和卵泡发育,此过程称为“点火效应” [5] ;GnRH-a继续作用后,GnRH受体被耗尽,血清中FSH和LH水平下降,从而产生抑制卵泡发育成熟的作用。目前临床上常用GnRH-a的代表药物有亮丙瑞林、曲普瑞林等。

为探讨GnRH-a是否能对化疗过程中的卵巢功能起到保护作用,Ilhae Park等 [6] 通过小鼠进行了实验研究,此研究选取了40只6~8周体重为16~18 g的雌性B6小鼠随机分为4组,每组10只,分别为生理盐水组,生理盐水+亮丙瑞林组,多西他赛组,多西他赛+亮丙瑞林组。在给予小鼠化疗药物两周前,给予相应小组小鼠皮下注射亮丙瑞林0.3 mg,6周后解剖其卵巢组织,观察卵泡数量。结果显示:单纯多西他赛组卵泡数(26.63 ± 5.40)与单纯生理盐水组(44.4 ± 12.16)相比明显减少,差异有统计学意义(p = 0.007)。多西他赛+亮丙瑞林组(40.14 ± 9.03)卵泡数明显多于单纯多西他赛组(26.63 ± 5.40),差异有统计学意义(p = 0.028)。同时行免疫组化染色,发现在1、2、4组中大量的卵泡被Ki~67着色,但在单纯多西他赛组中只有少量的卵泡被着色。在1、2、4组中没有组织被c-H2AX着色,但在3组中大量的原始初级卵巢被c-H2AX着色。这个试验充分说明了化疗前应用GnRH-a可减少86.8%由化疗引起的卵泡损害。另外还发现应用GnRH-a可使得卵巢处于抑制状态,从而避免了化疗药物对原始卵泡NDA损伤。但以动物试验推断GnRH-a的临床价值难免受到限制。

最新的临床试验也都证明了GnRH-a对化疗导致的卵巢损伤有着一定的保护作用。Moore H等 [7] 一项关于GnRH-a预防化疗导致卵巢早衰的研究,随访了218例年轻乳腺癌患者,结果显示肌注GnRH-a的患者卵巢早衰的发生率明显降低,且有着更高的妊娠率及新生儿出生率。虽然这个研究并没有阐明GnRH-a保护卵巢的机制,它显示了GnRH-a对早期雌激素受体阴性的乳腺癌患者的卵巢功能具有保护作用。研究发现同时发现GnRH-a在雌激素受体阴性的乳腺癌患者中的应用是安全的,且提高了患者的生存率。Hulsbosch S等 [8] 在2017年发表了一项关于长时间随访59例在化疗时采取生育力保护的癌症患者治疗后实际的妊娠愿望和结局的报道,此报道结果肯定了GnRH-a对年轻化疗患者生育力的保护作用。Leonard R等 [9] 的一项随机临床试验也证实给予肌注戈舍瑞林保护化疗患者的卵巢功能,在化疗后12~24月内肌注戈舍瑞林组闭经率明显小于单纯化疗组(22%:38%)。卵巢早衰的发生率也明显低于单纯化疗组(18.5%:34.8%),且在小于40岁的患者中差异更为明显(2.6%:20%)。Luminita Nicoleta Cima等 [10] 同样认为随着医学科学的发展对于血液系统恶性肿瘤患者,干细胞移植技术的发展使得患者术后生存率已不再是问题,更多需要关注的患者术后的生活质量问题和随着而来的生育问题,以及化疗导致卵巢早衰所出现的潮热、性功能障碍等影响患者生活质量的问题。Phelan等 [11] 研究发现在需行化疗的血液系统疾病患者中,联合应用GnRH-a也对预防化疗导致的卵巢损伤有保护作用。Schmidt KT的临床试验研究同样发现GnRH-a在霍奇金淋巴瘤化疗患者中的应用,在生育力保持方面有着积极的作用。且应用GnRH-a联合化疗更好的控制患者的月经周期 [12] 。

尽管证实GnRH-a对化疗过程中的卵巢功能有保护作用的临床证据有很多,但是仍有很多研究表明,GnRH-a对恶性肿瘤化疗患者没有很好的卵巢功能保护作用。2013发表的1项长达5年的对照研究显示:对于以新辅助化疗治疗的早期卵巢癌患者,如果以恢复月经作为标志,GnRH-a(曲普瑞林)治疗并未能够提供良好的卵巢功能保护作用 [13] 。且目前并没有完整的基础试验来证明GnRH-a对化疗过程中卵巢功能保护的确切机制。这都使得GnRH-a对卵巢功能的保护扑朔迷离,无法在临床作为保护方法普遍应用。

3. GnRH-ant对化疗过程中卵巢功能的保护

与GnRH-a的作用机制不同,GnRH-ant则是主要通过快速竞争性结合GnRH受体来阻断GnRH与其受体结合,从而阻断二聚体复合物的形成和信号的传递,从而抑制LH及FSH的释放。GnRH-ant无类似GnRH-a的“点火效应”,但作用时间短,在停药后2~4 d垂体功能即可恢复 [14] 。GnRH-ant的代表药物有西曲瑞克、加尼瑞克。虽然GnRH-ant无“点火效应”,但其有剂量依赖性,临床上需皮下注射西曲瑞克0.25 mg/24 h才可持续抑制卵泡的发育。目前,我国尚未开展关于化疗期间联合GnRH-ant在卵巢保护方面的研究,国外也只有少量的研究可供参考。

对于GnRH-ant是否能对化疗过程中的卵巢功能起到保护作用,Drevs J等 [15] 同样建立了小鼠实验模型,用GnRH-ant和环磷酰胺共同处理SD大鼠,用免疫组化法检测BCL-2蛋白和caspases-3蛋白在卵巢颗粒细胞中的表达,用放射免疫方法测定大鼠FSH和雌二醇的水平。结果提示GnRHA-ant对初级卵泡具有保护作用,处于静止期的卵泡可避免受化疗药物损害,从而保护卵巢的储备功能。

目前关于GnRH-ant保护化疗过程中卵巢功能的临床试验并不多。Blumenfeld等 [16] 为评估GnRH-ant是否对化疗过程中的卵巢功能有保护,对8位患有严重系统性红斑狼疮的年轻女性化疗患者在化疗期间同时应用GnRH-ant,其结果是肯定的。Recchia等 [17] 对100例早期乳腺癌患者行化疗的同时使用GnRH-ant的患者进行了长达7年的随访,结果显示小于40岁的患者全部恢复了月经,年龄大于40岁的患者56%恢复正常月经,且有3例成功妊娠并分娩。Whiehead等 [18] 对18例恶性肿瘤患者在每次化疗开始前的1~4 d使用西曲瑞克(皮下注射0.75 mg,每4天1次,共3次)的研究结果显示,化疗结束后12个月,94%的患者恢复自发排卵和月经周期;78%的患者血清FSH水平在正常范围(<10U/L)内,平均水平为5.18 U/L;94%的患者在排卵期可测得孕激素(平均水平为55 nmol/L);87%的患者抑制素A和B的水平正常。以上学者都肯定了GnRH-ant对化疗过程中卵巢功能的保护作用,但缺乏严格的对照组进行比较。Digeni等 [19] 报道,24例年龄为(23 ± 6)岁的女性恶性血液病患者,在每次化疗时使用3 mg西曲瑞克隔天皮下注射(西曲瑞克组),与未使用西曲瑞克的31例年龄为(26 ± 6)岁的患者(对照组)进行对比,结果显示,化疗结束6个月后西曲瑞克组患者继发性闭经的发生率显著降低(分别为4%、27%,P = 0.036)。因此认为,GnRH-ant能有效降低继发性闭经的发生率,起到了保护卵巢功能的作用。

由于GnRH-ant不具有点火效应,能即刻发挥对性腺轴及性激素的抑制效应,不良反应也较GnRH-a更少,因此也有学者认为GnRH-ant较GnRH-a更适合用于对化疗患者卵巢功能的保护。

4. 总结

总之,目前对使用促性腺激素释放激素类似物来保护化疗过程中的卵巢功能尚存在争议,医学界也并未达成一致的共识。许多系统回顾试验和metal分析都有着不同的结果,造成这种现象的因素有很多,例如缺少一致性的随机临床对照试验,没有明确的指标来评估卵巢功能的恢复和卵巢功能的衰竭,而且目前只有极少数的临床试验进一步研究评估了使用GnRH类似物保护女性肿瘤患者卵巢的最后妊娠率。而且并不是所有患者在接受烷化剂为基础的化疗后都会出现卵巢早衰的症状,一大部分保留生育功能的化疗患者,最后成功妊娠 [20] 。这些临床的发现都显示了化疗引起性腺损害的复杂性。

保护化疗患者的卵巢功能,提高患者化疗后的生活质量,给予患者化疗前提供不同的保护卵巢功能的选择,且多种治疗方式共存是非常重要的。应用激素疗法并不是就要排除卵母细胞或胚胎冷冻的低温储存。总之,一些试验否定了GnRH类似物保护化疗过程中卵巢功能的作用,但更多的试验对其作用表示了肯定。综合最新的研究结果认为,对于年轻恶性肿瘤患者在接受化疗前及化疗过程中规范应用GnRH类似物对卵巢功能有一定程度地保护作用。
