中药足浴治疗糖尿病足的研究进展

摘要

糖尿病足是糖尿病周围神经病变、糖尿病周围血管病变及局部感染三大因素综合作用于下肢,导致足部异常负荷、骨代谢异常、溃疡,最后形成坏疽的足部疾病。糖尿病足治疗有一定难度,近年来治疗糖尿病足的方式也越来越多,中医外治法对糖尿病足治疗逐渐受到重视。中药足浴是治疗糖尿病足的中医外治疗法,具有简便、效果佳的特点,通过足浴时热力作用、药力作用的双重效果,促进血液循环,扩张血管,改善周围组织营养,激发机体自身调节功能,值得在临床推广使用。

ABSTRACT

Diabetic foot is a foot disease characterized by diabetic peripheral neuropathy, diabetic peripheral vascular disease and local infection acting on the lower extremities in a comprehensive way, leading to abnormal foot load, abnormal bone metabolism, ulcers, and finally gangrene. It is difficult to treat diabetic foot. In recent years, there are more and more ways to treat diabetic foot. Traditional Chinese medicine foot bath is a traditional Chinese medicine external therapy for the treatment of diabetic foot. It has the characteristics of simple and good effects. Through the double effect of thermal action and drug action during foot bath, it can promote blood circulation, dilate blood vessels, improve the nutrition of surrounding tissues and stimulate the body’s self-regulation function, and is worthy of clinical promotion and use.

1. 引言

糖尿病足(Diabetic Foot, DF)是一种常见的糖尿病并发症。多是因为糖尿病患者血糖控制不佳,引起下肢周围神经及血管病变。糖尿病足这一并发症在糖尿病患者中的发病率逐年上升,达40%~60% [1] ,其中有约34% [2] 的糖尿病患者发展为糖尿病足溃疡,其中以中老年人为主,以2型糖尿病患者居多,有大约高达28%的患者可能会导致部分下肢截肢 [3] 。西医治疗方法 [4] [5] 多是扩张血管、营养神经、抗炎等治疗,中药足浴是中医外治法的一种,疗效显著,对于扩展糖尿病足的治疗方法和疗效,具有重要的意义。本文就中药足浴治疗糖尿病足展开论述。

2. 中药足浴

中药足浴是一种中医外治法,属于药浴疗法的一种,以辨证论治和整体观念为基础,进行选方调药,留取汤药进行足部的浸泡。中药足浴最早记载于东晋时期葛洪所著的《肘后备急方》:“赤龙皮汤,洗诸败烂疮方”、“水煮棘根汁,洗之”、“水煮令极热,以故布裹肿处,取汤热洗之,即瘥”等等均是治疗痈疽肿毒的相关记载 [6] 。

糖尿病足属于中医的脱疽,指南《糖尿病足病中医病证结合诊疗指南》2021版 [7] 中,将脱疽未溃者分为气虚血瘀证、血虚寒凝证、湿热毒盛证,并结合症状表现细致分为气血不足、痰瘀阻络、阴虚血瘀、血虚寒凝、肝肾亏虚,并根据不同证型提供了相应的中药足浴汤方。中药足浴发挥作用一方面皮肤毛孔受到物理刺激张开,毛细血管扩张,另一方面皮肤受到化学刺激,药物有效成分在热力作用下渗透皮肤,并被毛细血管充分吸收进入血液循环,从而发挥药力作用 [8] ,血脉通畅则气血运行通畅,可以缓解糖尿病足患者的疼痛麻木。

足部是足三阴经和足三阳经交汇的场所,药物通过皮肤吸收,刺激足部的反射区,再由经络进行传导,作用于全身,有利于血液循环和脏腑功能的改善。

中药汤剂具有抗炎杀菌的功效,中药足浴可以提高糖尿病足真菌感染患者的临床疗效。有研究表明,在经过治疗以后,基础治疗组的空腹血糖值从(10.189 ± 1.871)降低到(7.012 ± 0.815),而加中药足浴的实验组空腹血糖值从(10.625 ± 2.189)降低到(6.412 ± 1.784),两组间对比P < 0.05,具有显著差异;血清超敏C反应蛋白水平基础治疗组数值从(27.589 ± 6.871)降低到(17.012 ± 3.815),而加中药足浴组的数值从(27.625 ± 5.189)降低到(11.412 ± 3.784)两组数据P < 0.05,具有显著差异;基础治疗组的白细胞数目从(14.015 ± 3.044)降低到(10.525 ± 2.274),中药足浴组的数据从(13.748 ± 2.412)降低到(8.027 ± 2.124)两组数据P < 0.05,具有显著差异,综上所述,中药足浴可以减少局部感染的发生 [9] 。

3. 中药足浴治疗糖尿病足的临床应用

3.1. 运用于糖尿病周围血管病变

Xu Chenghua [10] 等运用中药足浴联合推拿手法治疗早期的糖尿病足患者,治疗14天以后,患者ABI改善、临床症状改善。Yuan Zhang [11] 等运用中药足疗联合普通换药治疗与安慰剂联合普通换药治疗进行对比,评估中药足疗治疗糖尿病足溃疡的安全性和疗效。也有研究表明,中药足浴可以在患者的溃疡部位传播感染,并增加脚趾脱落率(53%)和截肢率(30%) [12] ,所以一般中药足浴是用于治疗糖尿病足未破溃者。Ping Chung Leung [13] 等研究抗炎、促进循环和组织愈合的中药外用对于肢体局部肿胀和组织愈合的作用,发现效果良好。潘幸 [14] 等将中药足浴运用于糖尿病足溃疡者,联合常规清创换药可提高糖尿病足患者的治疗效果,促进溃疡愈合,抑制炎症反应,改善下肢缺血症状。崔巍巍 [15] 等用虎符铜砭刮痧联合中药足浴治疗0级糖尿病足,患者在临床症状和体征上均有改善,有效率达到90%以上。

3.2. 运用于糖尿病周围神经病变

Song Tianqi [16] 等运用治疗糖尿病足周围神经病变的常规西药治疗,与麻通散联合中药足疗进行了对比,结果发现麻通散联合中药足疗的效果优于常规西药治疗。张凌锋 [17] 等运用艾灸联合中药足浴治疗糖尿病足,可加快症状消失,降低血液流变学指标水平,提高神经传导速度。谭志雄 [18] 的研究表明,中药足浴能够有效提高病变临床治疗有效率,促进患者足部周围神经症状缓解、恢复足部功能。张丹 [19] 提出名中医王美素的经验,运用中药足浴联合足三里穴位注射维生素B12治疗糖尿病足,发现比口服甲钴胺片的效果更好,能够明显改善下肢血管和神经功能。吴亚辉 [20] 运用中药足浴联合足部的按摩,能改善早期糖尿病足患者的足部血管及周围神经状态,起到了良好的干预效果。

目前运用中药足浴治疗糖尿病足,大多联合其他方法一同治疗糖尿病足。除了常规药物疗法,还有艾灸、足部按摩、穴位注射等治疗方法联合使用,更能有效改善糖尿病足患者的血管和神经情况。中药足浴需要注意到水温和水位的问题,有研究表明,最佳的中药足浴方案为,足浴时药液深度达足踝上20 cm,时间为30 min,足浴温度根据病人的不同情况选择36℃~40℃,时间安排根据病人具体情况选择19:00~21:00 [21] 。

4. 足浴汤剂的中药使用

查找中国知网中运用中药足浴治疗糖尿病足的相关文献,从建库初起到2023年2月10日一共有243个相关文献,外网web of science、PubMed中截止到2023年2月10日共有9个相关文献。并且根据文献的内容,将记载的中药足浴汤方进行总结,分析其中的中药使用规律。

4.1. 血瘀证

血瘀证是运用中药足浴治疗糖尿病足研究最多的一种证型,运用的药物主要是红花、乳香、没药、川芎、丹参、当归、透骨草等活血化瘀、通络止痛的药物,偶有配合温性药物附子、艾叶等进行治疗,或者清热药物如黄连、黄柏、紫花地丁等。如表1所示。

4.2. 湿热蕴结证

湿热证型多用紫花地丁、大黄、蒲公英、黄柏、苦参、黄连、冰片等清热解毒的药物治疗,其中可能配伍活血化瘀的药物,如红花、丹参、鸡血藤等等,在清热的同时需要兼顾到活血。如表2所示。

4.3. 寒凝证

寒凝证型使用最多的药物是附子、桂枝,而后是红花、黄芪、独活、鸡血藤等行气活血通络的药物。寒邪凝滞,会导致血脉瘀阻,故寒凝多伴有血瘀,治疗时需要运用温经通络、活血化瘀方可奏效。如表3所示。

4.4. 未划分证型

未划分证型的用于治疗糖尿病足的中药足浴方剂中,红花、桂枝、当归、乳香、没药、透骨草、鸡血藤等等活血化瘀通络的药物运用最多,因此血瘀证是糖尿病足的一个重要的证型,治疗之中,需要兼顾到血瘀这个病理因素,如表4所示。

根据对中药足浴方剂用药的研究,大致可以推断出能运用的基础方主要由红花、桂枝、当归、乳香、没药、透骨草、鸡血藤组成。

红花的作用是活血通经、散瘀止痛,其中发挥药理作用主要的物质是红花黄色素 [22] ,红花能改善下肢的血管状态和减少炎症反应 [23] 。桂枝的作用是温通经脉止痛、温通阳气,其中发挥主要药理作用的物质是肉桂醛 [24] ,肉桂醛有抗菌消炎、提高免疫力的作用,能够有效治疗大肠杆菌、铜绿假单胞菌、金黄色葡萄球菌等 [25] 微生物引起的感染,而这也是引起糖尿病足溃疡感染的主要细菌,能够有效预防与治疗,还能够减缓周围神经的退化 [26] ,用以治疗糖尿病周围神经病变。当归的作用是补血活血,其发挥主要作用的成分是黄酮类物质 [27] ,能够改善血液循环,保护血管 [28] 。乳香和没药的作用相同,都是活血化瘀止痛,成分也都是挥发油类,通常作为药对出现,乳香的主要药理成分是乳香酸 [29] ,有促进血管生成和抗炎的作用 [30] ,没药的主要成分是萜类化合物 [31] ,能够有效抗炎、抗氧化、降低血脂等 [32] 。透骨草又称为急性子,作用是活血化瘀、透骨通经,其中棕桐酸乙酯和棕桐酸的含量最多,两者占29.03% [33] ,具有显著的神经保护活作用 [34] 。鸡血藤作用是活血补血通络,有效成分是黄酮类物质、酚酸酯和木脂素 [35] ,具有抗炎、抗菌、抗氧化等 [36] 作用。

5. 中药足浴现代研究

中药足浴相较于其他治疗方法所具有的特点是作用于足部的局部治疗、热力效应、皮肤局部药物吸收。糖尿病足主要的病理特点是患者的末梢循环障碍 [37] ,导致周围神经功能障碍 [38] ,所以很多治疗方案都是以改善糖尿病足患者末梢微循环为基础 [39] [40] [41] 。足浴可以改善糖尿病足患者的微循环状态 [42] ,从热力学方面扩张血管,而且中药足浴方式可以增加热余效应 [43] ;也可以从药物作用方面使微血管扩张 [44] ,改善糖尿病足患者的微循环。也有研究发现,CO2会使血管扩张 [45] ,所以在药液中加入CO2,对治疗糖尿病足或有促进作用。

从中医角度来说,足部是足三阴经与足三阳经的交汇场所,尤其肝在体为筋、脾在体为肉,两者对于经脉循行、下肢运动调节、皮肤状态均有影响。五腧穴大部分包括在足踝上20 cm内,所以水位控制在此,疗效更加。以补益心脾、养血安神等法调治,佐以中药足浴 [46] ,可以保持良好的睡眠和心情舒畅,有利于疾病恢复。

6. 结论

中药足浴作为一种糖尿病足的辅助治疗,配合其他疗法可以有效缓解糖尿病足患者临床症状。大多数运用于糖尿病足0级未破溃者,运用时要注意控制时间、温度和深度。运用的中药足浴方剂需要辨证进行配伍,根据用药分析得到一个常用的基础方:红花、桂枝、当归、乳香、没药、透骨草、鸡血藤,可以在此基础上进行加减化裁,寒凝重加桂枝、附子、艾叶等药物,气虚重加黄芪等药物、湿热重加黄连、黄柏、金银花、紫花地丁、冰片等药物。中药足浴可以改善末梢循环和神经状态,也能有效去除足部的常见细菌,减少糖尿病足感染的危险。
