摘要

目的:探讨首发精神分裂症患者偏瘦对认知功能的影响。方法:本研究纳入首发精神分裂症患者,按照BMI不同将其分为两组,对每一位患者的身高、体重、认知功能进行测评。结果:BMI与数字符号呈正相关,与连线测验A及连线测验B所用时间呈负相关;体重正常组数字符号、连线测验A及连线测验B的成绩好于体重偏瘦组。结论:BMI能够影响认知,体重偏瘦的首发精神分裂症患者认知功能低于体重正常组的患者,体重越瘦其认知功能越差。

ABSTRACT

Objective: The purpose of our study was to investigate the influence of underweight on cognitive function. Methods: The study recruited patients with first-episode schizophrenia, and they were divided into two groups according to BMI. The data of height, weight, and cognitive functions of the patients were collected. Results: There was a positive correlation between the BMI and Digit Symbol. There was a negative correlation between the BMI and Trail Making Test A and Trail Making Test B. Underweight patients with schizophrenia had significantly lower scores than the patients with normal weight on the Digit Symbol, Trail Making Test A and Trail Making Test B. Conclusion: Our study suggests that BMI can affect cognitive function; the cognitive function of the underweight patients is lower than the patients with normal weight; the lower the weight, the worse the cognitive function.

1. 引言

精神分裂症患者在注意力、执行能力及记忆力等方面存在中度至重度的认知功能缺陷(Krug, Stein, & Kircher, 2020),认知功能缺陷可能是该病的一个核心病理生理特征(Hill, Schuepbach, Herbener, Keshavan, & Sweeney, 2004; Snitz, Macdonald, & Carter, 2006)。近年来,体重指数与认知功能的相关性已经成为研究的热点问题,研究发现无论是健康人群(Gunstad, Lhotsky, Wendell, Ferrucci, & Zonderman, 2010; Gunstad et al., 2007)还是精神分裂症患者(李娜,刘善明,邓伟,张波,耿婷,张树森,黄明敏,李涛,邓红,2012;Guo et al., 2013),肥胖人群的认知功能均较体重正常者差。然而,精神分裂症患者的偏瘦率也较健康人群高(刘军军,程敏,邵阿林,曹慧,吴兵,龚有山,邹玉青,张向荣,2018)。偏瘦是否与认知功能相关,目前未见报道。因此本研究选取首发未服药精神分裂症患者为研究对象,探讨体重指数过低对认知功能的影响。

2. 对象与方法

2.1. 研究对象

为排除患者服药、利手及民族的差异,我们选取河北医科大学第一医院精神卫生科门诊及住院的首发、未服用抗精神病药物、右利手的汉族精神分裂症患者。入组标准:1) 符合DSM-IV精神分裂症的诊断标准;2) 征得患者或其监护人知情同意;3) 年龄16~45岁,性别不限;4) 既往未曾使用过抗精神病药物;5) 能够完成认知测验。排除标准:1) 伴有脑器质性疾病、严重的躯体疾病或有精神活性物质使用史者;2) 共患其他重性精神疾病者、头部外伤史昏迷大于2 h者、精神发育迟滞、痴呆、癫痫及严重的认知功能障碍者;3) 妊娠或哺乳期患者。

2.2. 方法

1) BMI值的获得

患者入组时,用同一衡器测量被试者的身高、体重,并由同一记录员负责记录。测量时,要求所有的被试穿轻薄衣服,并脱去鞋袜。BMI采用传统的计算方法,BMI = 体重/身高2

2) 临床评定

临床评定由精神科专科医生对患者进行一般情况调查,Annett利手问卷评定所有被试的利手情况。由经过一致性培训的两名精神科专科医生采用患者版的DSM-IV定式临床检查工具(SCID-P)进行临床评估,DSM-IV进行诊断,并用阳性和阴性症状量表(Positive and Negative Syndrome, PANSS)评估患者精神症状的严重程度。

3) 神经认知功能的评估

由经过一致性培训的精神科专科医生评定患者的如下认知功能:数字符号、数字广度、逻辑记忆及连线测验A、B。这几项认知测试可以测试4个领域,包括处理速度(数字符号,连线测验A);注意/工作记忆(数字广度)、执行功能(测验B)和记忆能力(逻辑记忆)。

4) 统计学分析

所得数据录入SPSS19.0软件,采用两样本t检验比较两组的年龄、受教育年限、PANSS总分及认知指标。以c2检验比较两组间男女比例的差异。用偏相关分析,以年龄、性别、受教育年限为斜变量,分析BMI与认知的相关性。

3. 结果

3.1. 基本情况

本研究共纳入101名体重正常及56名偏瘦的首发精神分裂症患者。两组患者在年龄、病程及受教育年限等方面差异无统计学意义,在男女比例方面差异无统计学意义。在精神病性症状的严重程度方面差异也无统计学意义,见表1

3.2. BMI与认知功能的相关性

研究显示BMI与数字符号呈正相关(r = 0.28, p < 0.01),与连线测验A (r = −0.27, p < 0.01)及连线测验B (r = −0.23, p = 0.02)所用时间呈负相关,而与数字广度及逻辑记忆能力无相关性,见表2

3.3. 两组间认知功能比较

体重正常组与偏瘦组的认知功能比较结果显示,体重正常者与偏瘦者在数字符号方面差异有统计学意义,体重正常者优于偏瘦者(t = 2.67, p < 0.01)。两组之间在数字连线A (t = −2.41, p = 0.02)及B (t = −3.37, p < 0.01)所用时间上差异均有统计学意义,偏瘦组所用时间更长。两组之间在数字广度及逻辑记忆方面差异无统计学意义,见表3

4. 讨论

本研究结果显示,体重指数对患者的认知功能有影响。体重指数与患者的执行功能及运动速度呈负相关,也就是说患者的体重越轻,其执行能力及运动速度越差。消瘦的患者在执行功能及运动速度方面比体重正常者差。未发现体重指数对患者注意/工作记忆及逻辑记忆的影响。

体重指数对认知功能的影响已经被近年来的研究所证实(Gustafson, Lissner, Bengtsson, Bjorkelund, & Skoog, 2004; Gustafson, Rothenberg, Blennow, Steen, & Skoog, 2003; Kivipelto et al., 2005),但是这些研究几乎均在关注肥胖对于认知功能的影响,认为体重指数增加会使患者的认知功能下降。仅有极少数的报道纳入了偏瘦的人群,Salokangas等(Salokangas, Honkonen, Stengard, & Hietala, 2007)研究老年阿尔茨海默病患者发现,体重指数偏低是老年患者阿尔茨海默病的危险因素。Salokangas等(Salokangas et al., 2007)运用功能大体评定量表(Global Assessment Scale, GAS)对722例精神分裂症患者进行了3年的随访研究,发现低体重的男性精神分裂症患者整体功能更差。Guo等(Guo et al., 2013)的研究虽然关注肥胖对认知功能的影响,但其也纳入了偏瘦的患者,其研究结果中,未曾发现连线测验与BMI有相关性,究其原因可能是其既纳入了偏瘦的患者也纳入了超重与肥胖的患者,偏瘦患者BMI与连线测验时间的负相关抵消了超重与肥胖患者的正相关,其结果也发现在连线测验A中偏瘦甚至比肥胖患者的用时更长。

既往也有关注过急性起病精神分裂症患者体重变化与预后的研究,结果发现其预后与BMI之间有相关性,BMI增加少者预后较差(Basson et al., 2001; Kinon, Basson, Gilmore, & Tollefson, 2001; Meltzer, Perry, & Jayathilake, 2003)。关于偏瘦对认知影响的机制目前还不是十分清楚,Horrobin等(Horrobin, 1998)提出精神分裂症患者从神经膜磷脂中去除必需脂肪酸,尤其是花生四烯酸和二十二碳六烯酸的速度增加,并且会降低这些脂肪酸渗入膜磷脂中。而偏瘦的患者可能消耗必需脂肪酸更快,导致其神经膜磷脂受损,从而导致其认知功能下降。

本研究的不足之处在于,仅采用BMI作为评定体重的指标,尽管目前这项指标是运用最为广泛的评价指标,但其不能像腰围–臀围比例一样能够评定肥胖类型。而有研究显示不同的肥胖类型对认知功能影响不同,其中向心型肥胖的认知功能受损最为严重(Elias, Elias, Sullivan, Wolf, & D’Agostino, 2003)。在以后的研究中,需要测定患者的腰围与身高之比,腰围与臀围之比等指标,协助判断患者的肥胖类型。

我们的研究显示,BMI可以作为独立危险因素影响精神分裂症患者的认知功能,BMI越低于正常水平,其认知功能越差。
