A Case Report of Diabetic Foot Infection Caused by Helcococcus kunzii

ABSTRACT

Background: Helcococcus kunzii is a facultative anaerobic bacteria, which was originally considered a normal flora of human lower limb skin. The cases of invasive infection caused by Helcococcus kunzii are extremely rare in domestic and foreign literature. In this paper, the patient with diabetes mellitus caused by Helcococcus kunzii is reported. The patient was 67 year old female with diabetes foot in our hospital surgery. In and after operation, the secretions were taken for bacterial culture. Helcococcus kunzii has been cultured two times. After the intravenous drip of levofloxacin and the combination of Chinese herbal medicine in our hospital, the condition of the patient improved significantly and discharged. Discussion: What has reported in this article is the first case of invasive trauma caused by the pathogen of diabetic foot disease cases in the country. Now with the use of a large number of antibiotics, Helcococcus kunzii, which was originally considered the normal bacteria of body can also cause the body invasive infection. And clinical workers should attach great importance to Helcococcus kunzii.

由孔氏创伤球菌引起糖尿病性足病感染 的个案报道

摘 要

背景:孔氏创伤球菌是兼性厌氧菌,最初被认为是一种人体下肢皮肤的正常菌群,因孔氏创伤球菌引起侵袭性感染的病例在国内外文献报道中极其罕见,现报道我院中医外科一例由此菌引起的糖尿病足的病历。病例:患者为67岁女性以糖尿病足收入我院中医外科,术中和术后取分泌物做细菌培养,两次结果均培养出孔氏创伤球菌,后经左氧氟沙星静脉滴注和我院自制中药联合用药后,病情明显好转后出院。讨论:本文报道的病例在国内是第一例由孔氏创伤球菌引起的糖尿病性足病感染的病例,现在随着大量抗生素的使用,原认为是人体正常菌群的孔氏创伤球菌,也会引起人体侵袭性感染,应引起广大临床工作者的高度重视。

1. 背景

孔氏创伤球菌(Helcococcus kunzii)是兼性厌氧菌,是由Collins等人1993年首次描述。最初被认为是一种人体皮肤的正常菌群 [1] [2] [3] ,尤其是下肢皮肤。因孔氏创伤球菌(Helcococcus kunzii)引起侵袭性感染的病例在国内外文献报道中极其罕见,现报道我院中医外科一例由此菌引起的糖尿病足的病历,报道如下:

2. 病例简介

患者为67岁女性、2016年8月9日以糖尿病足收入我院中医外科。

临床诊断:中医诊断:脱疽(脓疡期)湿淤壅盛,气阴两伤。

西医诊断:糖尿病性足病3级,糖尿病性周围血管炎,糖尿病性周围神经炎,糖尿病性骨病体格检查:

患者发热,体温38.5℃;脉搏105次/分;呼吸:20次/分;血压:160/90 mmHg。

实验室检查:空腹血糖:13.29 mmol/L;糖化血红蛋白:9.8%;总蛋白:50.2g/L白蛋白:29.3g/L;BUN:3.04 mmol/L;cr:40.3 umol/L;WBC:14.95 × 109 /L中性粒细胞百分比:83.24%淋巴细胞百分比:9.12%单核细胞百分比:5.7%中性粒细胞计数:12.44 × 109/L CRP:177 mg/L;PCT:0.16 g/L;D-dimer:4058 ng/L (N: < 500 ng/L);纤维蛋白原:6.10 g/L (N: 2 g/L~4 g/L);N-末端脑钠肽:2728 ng/L (N: < 300 ng/L)

专科检查:双下肢凹陷性水肿,左侧明显,全足潮红肿胀,第2趾明显,内侧破溃,创口直径约1 cm可探及骨质,脓腔深约5 cm,按压可见较多灰褐色脓液流出。

手术处理:即刻行切开引流术,减张治疗,给邪出路,周围中药箍围消肿微生物学资料:

术中和术后取分泌物做细菌培养,两次结果均培养出同一种菌:在血平皿生长为r-溶血,无色透明小菌落,镜下形为G+球菌,菌体较其他链球菌稍大,单个散在排列。见图1,触酶(-),氧化酶(-)上机VITEK 2-Compact鉴定结果为孔氏创伤球菌,生物编码:020112100000000符合率为99.0%同时请外院老师使用基质辅助激光解吸电离-飞行时间质谱仪(MALDI-TOF)进行细菌鉴定,其鉴定结果也为孔氏创伤球

菌,符合率为99.0%。

入院时:左足全足渐红肿。足趾肿胀疼痛加重,脓液增多。

出院时:经过20天左右的左氧氟沙星静脉滴注和我院自制中药联合用后,患者伤口分泌物明显减少,体温正常,WBC降至正常范围。

3. 讨论

创伤球菌属在表型上与绿色气球菌相似,是兼性厌氧菌,触酶阴性;无动力、无芽孢的革兰氏阳性球菌,创伤球菌属目前有5个菌种,分别是孔氏创伤球菌、化脓创伤球菌、绵羊创伤球菌、瑞典创伤球菌、西雅图创伤球菌,直到现在未见官方命名方法 [4] ,化脓创伤球菌只报道过一例从一个独特的人工关节感染病例中分离出,但其名称尚未验证 [5] ;据报道绵羊创伤球菌可以从不同种的动物体内分离,但是从未有从人体内分离的报道 [6] ;瑞典创伤球菌曾经报道过三例,一是菌血症的血液中和二例伤口分泌物 [7] ;西雅图创伤球菌有过一例报道,是从泌尿系感染患者的血液中分离出。孔氏创伤球菌是创伤球菌属中重要的一个菌种,孔氏创伤球菌(Helcococcus kunzii)是兼性厌氧菌,触酶阴性、无动力的革兰氏阳性球菌,镜下菌体呈对或簇状排列;经37℃24小时培养箱孵育后,细菌生长缓慢,菌落呈针尖样、浅灰色无溶血或轻微α溶血。在5% CO2或厌氧条件下与普通培养下培养,细菌生长状态并没有不同。孔氏创伤球菌之前被认为是人类皮肤的正常菌群,后来不断地从感染的伤口中分离出来,尤其是下肢皮肤伤口,特别是糖尿病患者蜂窝组织炎的足部溃疡,本文报道的此例患者为糖尿病3级并伴有高血压、心衰等症,患者身体极度虚弱,多次出入院,治疗起来非常困难,本例分离出孔氏创伤球菌,很可能与患者大量使用抗生素和免疫抑制剂,再加上糖尿病患者易出现感染,进一步加剧了这种菌感染的机会。现孔氏创伤球菌已被确定为脓肿、菌血症、蜂窝织炎、脓胸、植入式心脏设备感染 [8] 、心内膜炎 [9] 、骨髓炎、关节感染、皮脂腺囊肿感染的一种病原菌;细菌鉴定上采用法国梅里埃全自动细菌鉴定药敏仪VITEK-II Compact GP鉴定卡进行鉴定,后又请外院老师采用MALDI-TOF质谱仪进行细菌鉴定复核,细菌鉴定结果同为孔氏创伤球菌,药物敏感性试验结果显示此菌对氨基糖苷类、喹诺酮类和β-内酰胺类抗生素敏感性强一些。该患者经过20天左右的左氧氟沙星静脉滴注和我院自制中药联合用后,患者伤口分泌物明显减少,体温正常,WBC降至正常范围后出院。在这里描述的是第一例由孔氏创伤球菌引起的糖尿病足的患者,在国内报道非常罕见,国外文献中韩国偶有报道,在此提请这种罕见的细菌,随着免疫抑制剂大量使用和侵入性操作逐渐增多,这种细菌有可能成为一个新的病原体。
