The Present Status and Prospect of Brain Imaging in Conduct Disorder

ABSTRACT

Conduct disorder (CD) is one of the most common psychogeny in children and adolescents. In this review, we mainly focused on the studies about structural magnetic resonance imaging (sMRI) and diffusion tensor imaging (DTI) in conduct disorder, and we also suggested the possible directions of development in these fields. We hoped that this review would give help and guidance to further studies on the brain imaging markers of CD.

品行障碍脑影像学研究现状与展望

摘 要

品行障碍(conduct disorder, CD)是儿童青少年期最常见的一种精神障碍之一。本文主要对CD的结构性核磁共振成像和弥散张量成像研究进行综述和分析,并在此基础指出了未来CD脑成像研究可能的发展方向,以期为进一步研究CD的特征性脑影像学标记提供帮助和指导。

1. 引言

品行障碍(conduct disorder, CD)是儿童青少年期最常见的一种精神障碍,表现为反复、持续出现的攻击性和反社会性行为模式,如攻击他人、偷窃、破坏行为、离家出走、逃学等品行问题(American Psychiatric Association, 2013)。CD在不同性别人群中的发病率不同,如国外学者报道在男性青少年中CD的发生率为6%~16%,是女性发生率的2~3倍(Berkout, Young, & Gross, 2011)。CD还常常会伴随其他类型的精神障碍,如注意缺陷多动障碍(attention-deficit hyperactivity disorder, ADHD)、对立违抗障碍(oppositional defiant disorder, ODD)、物质滥用以及焦虑、抑郁障碍等(American Psychiatric Association, 2013)。目前,流行病学调查结果显示CD不仅引发了严重的经济负担和社会负担,而且预后极差,大多数CD青少年成年后发展为反社会人格障碍(antisocial personality disorder, ASPD) (Blair, Leibenluft, & Pine, 2014)。因此,探讨CD的临床特征(如冲动、攻击行为等)以及其神经生物学机制,寻找其特征性脑影像学标记,为其诊断和干预提供客观的生物学依据,已成为该领域前沿研究的热点之一。

2. 品行障碍的脑影像学研究现状

由于研究方法的限制和患者发病年龄均在青少年早期的原因,10年前关于CD的病理机制,尤其是其神经机制的研究并不多。近年来,随着实验方法的改进,对CD的脑成像研究逐渐增多(Hyde, Shaw, & Hariri, 2013)。从广义上讲,脑功能成像研究方法包括结构性核磁共振成像(structural magnetic resonance imaging, sMRI)、基于血氧水平依赖的功能性核磁共振成像(blood oxygenation level dependent functional magnetic resonance imaging, BOLD-fMRI)和弥散张量成像(diffusion tensor imaging, DTI)以及事件相关电位(event-related potentials, ERPs)等。本课题组在前期已对CD的脑功能成像研究成果做了相应的总结陈述,因此本文主要将分别对其他脑影像学研究方法的在CD中的主要研究结果进行概述。

2.1. 结构性核磁共振成像(sMRI)研究

本结构磁共振成像(sMRI)是一种拥有高空间分辨率和无创性的对大脑结构(灰质)精确定位分析的技术手段,目前已被应用到各种精神障碍和神经疾病的病理生理机制的研究中。Kruesi等人首次采用基于感兴趣区(regions of interest,ROI)的分析方法研究了CD患者的脑结构改变情况,结果发现患者的右侧颞叶体积显著低于正常对照组被试(Kruesi, Casanova, Mannheim, & Johnson-Bilder, 2004)。紧接着,Sterzer等人和Fairchild等人采用基于体素的形态测量学分析(voxel-based morphometry, VBM)的方法发现,与正常对照组相比,CD组的双侧杏仁核和岛叶的灰质体积下降(Sterzer, Stadler, Poustka, & Kleinschmidt, 2007; Huebner et al., 2008; Fairchild et al., 2011),而上述灰质体积下降恰恰可能导致其产生攻击行为(Sterzer, Stadler, Poustka, & Kleinschmidt, 2007)。回归分析显示,CD患者的品行问题症状与右侧岛叶体积之间存在显著负相关(Fairchild et al., 2011)。岛叶灰质变薄和灰质密度下降会导致前额叶环路功能紊乱,削弱前额叶对愤怒、攻击和冲动等的抑制性,增加个体产生攻击行为的可能性(Fahim et al., 2011)。此外,还有研究相继发现了品行障碍患者的前额叶(包括眶额皮层)和颞叶(Huebner et al., 2008; Dalwani et al., 2011; Stevens & Haney-Caron, 2012; Olvera et al., 2014)、顶叶(Stevens & Haney-Caron, 2012)、前扣带(Boes, Tranel, Anderson, & Nopoulos, 2008; Olvera et al., 2014)、纹状体(Fairchild et al., 2013; Wallace et al., 2014)、海马(Huebner et al., 2008)以及舌下回和小脑(Dalwani et al., 2011)等脑区体积下降。相关分析发现,冲动性与额顶叶和颞叶皮层的灰质体积下降有关,而品行障碍症状严重程度主要与边缘系统灰质体积下降有关(Huebner et al., 2008)。

Yang等人对以往反社会行为文献进行的Meta分析结果显示,眶额叶、背外侧前额叶以及前扣带皮层等脑区的结构和功能异常与反社会行为密切相关(Yang & Raine, 2009)。Rubia的研究也发现CD主要与负责调节动机和情感的旁边缘系统异常有关,包括外侧眶额叶、腹内侧前额叶、上颞叶以及边缘系统结构中的杏仁核等(Rubia, 2011)。Aoki等人的Meta分析结果发现,与正常对照组被试相比,反社会行为组患者的右侧豆状核、左侧前岛叶和额上回等脑区的灰质体积下降,而右侧梭状回、顶下小叶、顶上小叶、扣带回以及中央后回等脑区的体积增加(Aoki, Inokuchi, Nakao, & Yamasue, 2014) 。最近,Rogers和De Brito所做的元分析结果发现,CD患者的岛叶、杏仁核以及额叶和颞叶等脑区灰质体积显著低于正常对照组,但这一结果受不同样本构成的影响(Rogers & De Brito, 2016)。

近年来,研究者们对CD患者的皮层厚度和皮层表面积等进行了相关研究。Hyatt等人首次对CD进行了皮层厚度的分析,结果发现,与正常对照组被试相比,CD患者大脑皮层厚度变薄和褶皱减少(Hyatt, Haney-Caron, & Stevens, 2012)。皮层变薄脑区主要集中于后部脑区,包括双侧上颞叶、顶叶、颞顶联合,左侧旁边缘叶以及右侧楔前叶;褶皱减少脑区主要集中在前部脑区,包括左侧岛叶、腹内侧和背内侧前额叶、前扣带、眶额皮层、颞叶以及右侧上额叶、顶叶和旁边缘叶。Wallace等人发现,与正常对照组被试相比,CD患者组上颞叶的皮层厚度显著降低;腹内侧前额叶的局部脑回指数下降;杏仁核和纹状体(包括壳部和苍白球部)的体积也显著降低;皮层表面积不存在显著性组间差异(Wallace et al., 2014)。最近,Sarkar等人的研究则发现,与正常对照组相比,CD组腹内侧和背外侧前额叶脑区的体积和表面积均显著下降,而皮层厚度上两者没有显著性差异(Sarkar et al., 2014)。本课题组Jiang等人研究发现,CD患者楔前叶-后扣带等部位的皮层厚度显著变薄,且这一脑区与行为学得分之间显著性负相关(Jiang et al., 2015)。

综上可见,关于CD的脑结构成像研究大多发现,患者的眶额叶、杏仁核、纹状体、岛叶、梭状回/枕叶以及颞叶等脑区的体积、皮层厚度或者皮层表面积等显著低于正常对照组被试。而上述脑区大多属于边缘系统和旁边缘系统,主要与情绪加工过程有关,因此上述脑区的异常可能表示反社会行为个体存在情绪加工异常等。如岛叶灰质密度降低会破坏前额叶回路,削弱前额叶对愤怒、攻击以及冲动性行为的抑制功能,从而导致个体产生攻击行为的可能性增加(Fahim et al., 2011)。

2.2. 弥散张量成像(DTI)研究

DTI主要是通过无创性地测定纤维束轴突中的水分子的运动情况来评估个体白质结构的走向和完整性(如髓鞘化程度、胞膜完整性等)以及大脑结构和功能之间的关系。尽管DTI 技术在精神疾病中的应用已经较为广泛,但在CD甚至破坏性行为障碍(DBD) (包括CD、ODD等)中的研究依然处于起步阶段,相关文献报道也不够全面(见表1)。Kruesi等人2004年首次比较了10名儿童期起病型CD患者和10名正常对照组被试的脑白质体积差异,结果发现患者组的白质体积有小于正常对照组的趋势(Kruesi, Casanova, Mannheim, & Johnson-Bilder, 2004)。2005年Li等人首次采用DTI成像技术研究了DBD患者白质结构的变化情况,结果发现患者组双侧额叶以及左侧颞叶(尤其是弓状束)的FA值显著低于正常对照

组(Li, Mathews, Wang, Dunn, & Kronenberger, 2005)。钩束(uncinate fasciculus, UF)是连接大脑额叶与颞叶的主要白质纤维,Craig等人采用ROI纤维追踪分析对ASPD患者的研究发现,患者组UF的FA值显著低于正常对照组(Craig et al., 2009)。Motzkin等人对精神病态患者的研究也发现患者组UF的 FA值显著低于正常对照组被试,这就支持了Craig等人的发现(Motzkin, Newman, Kiehl, & Koenigs, 2011)。在此基础上,Sarkar等人采用同样的方法研究了CD患者UF的微结构改变的情况。结果发现,患者组被试的左侧UF的FA值显著高于正常对照组,同时RD值显著低于正常对照组被试(Sarkar et al., 2013)。同时,Passamonti等人采用基于体素的全脑分析(voxel-based analysis, VBA)以及纤维追踪方法对男性青少年型CD患者的研究也验证了Sarkar等人的发现(Passamonti et al., 2012; Zhang et al., 2014)。这说明,品行障碍患者可能确实存在钩束结构的异常,且该纤维束的损伤与其认知、社会情绪以及行为困难等密切相关。

Wang等人采用基于纤维束示踪的空间统计学方法(tract-based spatial statistics, TBSS)对 DBD患者的研究结果显示,DBD并伴有ADHD(DBD + ADHD)组被试FA值显著低于正常对照组,MD、RD以及AD值等显著高于正常对照组被试。上述有差异脑区主要集中在胼胝体,双侧内囊前肢,前、上、后放射冠,上纵束和下纵束,后丘脑辐射,右侧内囊后肢、外囊和穹窿以及左侧扣带等。同时DBD+ADHD组被试在胼胝体,双侧前、上放射冠以及右后放射冠等脑区的FA显著低于DBD但不伴有ADHD组(DBD-ADHD)患者,RD值也显著高于后者(Wang et al., 2012)。与此同时,Finger等人采用基于ROI的纤维追踪和TBSS方法对DBD患者的DTI研究却没有发现患者组与正常对照组被试之间存在白质微结构的显著性差异(Finger et al., 2012)。最近,Haney-Caron等人采用了TBSS的方法发现单纯性(pure)CD组在前/上放射冠以及下纵束和下额枕束等脑区的FA和AD值显著低于正常对照组被试(Haney-Caron, Caprihan, & Stevens, 2014)。由于AD值下降,所以该作者认为品行障碍患者可能是由于轴突完整性异常而非髓鞘化异常所引起。最近,Zhang和Sarkar以及 Lindner等人分别采用TBSS的方法探讨了CD患者整体脑结构的改变情况,结果发现了在胼胝体以及内囊等部位的白质微结构存在一定程度的改变(Zhang et al., 2014; Lindner et al., 2016; Sarkar et al., 2016)。

以往研究表明,大约50%左右的品行障碍患者在成年期会发展为反社会人格障碍(ASPD)。对ASPD脑白质的研究早于CD患者的研究,如Raine等人在2003年首次对ASPD患者的白质结构进行了研究,结果发现其胼胝体的体积和长度均显著高于正常对照组被试,但厚度却低于后者(Raine et al., 2003)。Sundram等人采用VBA方法的研究发现,患者组胼胝体双侧膝部、右侧UF、下额枕束、前放射冠以及内囊前肢与膝部的FA值显著低于正常对照组被试,且与精神病态显著性负相关(Sundram et al., 2012)。上述脑区的MD值显著高于正常对照组,且与精神病态得分呈显著性正相关。精神病态(Psychopathy)与品行障碍也有紧密的关联,在DSM-V中也专门加入了一条诊断标准用于评估该特质(American Psychiatric Association, 2013)。最近,Hoppenbrouwers等人对伴有精神病态的罪犯的研究表明,患者组与杏仁核-前额叶网络和纹状体-丘脑-额叶网络环路有关的前丘脑辐射、下额枕束以及钩束的FA值显著低于正常对照组被试,且上述脑区与其冲动和攻击行为等密切相关(Hoppenbrouwers et al., 2013)。

需要注意的是,尽管已有一定数量的CD和DBD以及ASPD脑白质的研究,但上述研究却存在着一定的局限性,如分析方法与样本构成等方面,因此,导致其结果或结论不尽一致。首先从分析方法上来讲,采用基于感兴趣区的分析需要强有力的信息假设,即先验知识。Sarkar等人对CD患者的钩束的分析是完全建立在Craig等人对成年精神病态患者的研究基础之上。该分析方法仅仅能够说明单个ROI的情况,不能够代表整体上的改变,且该方法比较耗时。尽管Passamonti等人采用了VBA的分析方法,但该方法在实施过程中需要对被试数据进行平滑处理,这就有可能人为的去除一些个人特征,因此也经常受到方法学家们的批评,而这也是TBSS方法兴起的一个主要原因。分析方法的不同也会导致实验结果的差异,如Sarkar等人基于ROI的纤维追踪没有发现下额枕束以及下纵束的平均FA值差异,但在Haney-Caron等人进行的TBSS结果中,上述脑区的FA值以及AD值是显著降低的。其次从样本构成上来讲,上述采用TBSS分析的研究中并没有对DBD患者进行区分,也没有控制ADHD以及性别等因素的干扰作用。有研究表明,CD和ODD患者以及ADHD同样也存在脑结构与功能上的差异,应该单独进行相关研究(Stevens & Haney-Caron, 2012)。此外,不同性别被试之间脑白质结构也可能存在差异(Ruigrok et al., 2014)。最后,上述研究中患者组的样本量均比较少,这也可能影响实验结果的统计效力和结果的可重复性(Zuo & Xing, 2014)。

2.3. 事件相关电位(ERPs)研究

关于CD的ERPs研究主要集中于P300的实验范式上。已有研究结果发现,与正常对照组被试相比,CD患者的听觉和视觉P300的潜伏期明显延长,且其波幅明显下降(Cappadocia, Desrocher, Pepler, & Schroeder, 2009; Gao & Raine, 2009; Brazil et al., 2012),说明CD患者对靶刺激的注意加工能力相对较弱。Bertoletti等人的最近研究也发现,P300波幅与品行问题得分之间存在显著负相关(Bertoletti et al., 2014),而且不受实验刺激序列的影响(Gilmore, Malone, & Iacono, 2012)。因此,P300波幅下降被认为可以作为诊断品行问题的一个很有价值的指标。此外,Huang等人的研究结果发现,CD患者的在恐惧性音节条件下的MMN波幅显著高于悲伤音节条件下的波幅,且该波幅与患者的冲动性特征呈显著性正相关,说明其在处理情绪信息时存在前注意加工阶段的异常(Hung, Ahveninen, & Cheng, 2013)。最近,本课题组Gao等人发现,CD患者的反馈相关负波的波幅显著低于正常对照组,说明CD人群的反馈评估过程可能存在一定的异常改变(Gao et al., 2016)。

3. 小结及展望

通过上述文献分析可以看出,关于CD的脑成像研究(sMRI、fMRI和DTI等)基本上集中在执行功能、情绪调节与控制以及决策机制等三方面的研究上,发现了CD患者与情绪控制和调节有关的脑区结构或者功能的异常。这也支持了Davidson等人提出的个体产生暴力、攻击行为的主要原因是由于情绪调节回路受损的论断(Davidson, Putnam, & Larson, 2000)。然而,尽管在结构和功能成像上均发现了一些异常的脑区,但上述研究发现依然存在进一步补充和完善的空间,主要表现在一下几个方面。

1) CD样本的异质性可能会影响实验结果。在最新修订的DSM-V中,CU特征专门增加为CD的一个诊断分类和预后判断指标(American Psychiatric Association, 2013)。当前研究发现,不同水平CU特质以及不同亚型(儿童型或者青少年型)患者的脑区激活模式或者脑结构也不尽相同(Passamonti et al., 2010; Frick, Ray, Thornton, & Kahn, 2014)。上述研究中大多数样本都着重研究了伴有CU特质患者的脑机制的改变情况,但极少对品行障碍亚型进行区别研究。那么,不同CU特质在不同品行障碍类型患者之间是否也存在显著性激活差异以及有哪些差异就需要进一步研究。需要注意的是,品行障碍还常常会伴有ADHD、ODD、物质滥用以及焦虑和抑郁等其他精神障碍。在上述研究中,大部分研究样本中都包含了不同程度的混杂因素(Hyde, Shaw, & Hariri, 2013),如伴有ADHD和ODD甚至物质滥用等,而上述共病均与CD具有不同的神经机制(Rubia, 2011; Blair, Leibenluft, & Pine, 2014)。因此,在去除相关影响因素后上述研究结果会不会改变以及有哪些改变也都需要将来进一步的研究。此外,以上研究很少有专门探讨女性患者以及不同性别患者之间的脑结构或者脑功能异常改变的情况。在将来的研究中,研究者需要增加对女性CD的关注以及不同性别患者之间脑成像差异的研究。

2) 缺乏最新脑连接分析方法的应用。以往绝大多数关于CD的磁共振研究都是针对特定脑区的激活进行的,即都是对大脑功能分化原则的研究,发现了一些异常脑区,如杏仁核、眶额叶和纹状体等。但是,人脑本身是一个巨大而复杂的网络(大约包含1012个神经元以及多达1015个突触连接),其任何功能的实现都是需要多个神经元或者脑区的共同作用来完成。因此,在以往单个脑区激活研究的基础上进行脑连接分析可以更加全面、深入地认识大脑的内部组织形式,进而揭示其认知神经机制。目前,关于CD患者人群脑连接的研究才刚刚起步。如VBA、TBSS、纤维追踪发现患者的钩束和胼胝体等结构异常(Passamonti et al., 2012; Sarkar et al., 2013; Zhang et al., 2014; Zhang et al., 2014)、CD患者前额叶与杏仁核之间的功能连接显著低于正常对照组(Finger et al., 2012)。值得注意的是,结构网络连接和有效连接越来越受到广大研究者的青睐并被广泛应用于各种精神疾病的研究当中。尽管目前尚未见到有关CD脑结构连接网络变化的相关报道,但本课题组Zhang等人首次采用动态因果模型分析了CD患者的抑制控制网络的变化情况,结果发现患者组“Stop”环路之间的有效连接显著低于对照组被试(Zhang et al., 2015)。由于样本来源的局限性,这一研究结果有待于今后进一步的验证。

3) 缺少大样本和多模态的相关研究。纵观目前关于CD人群的研究,基本上都属于小样本(患者样本量大多不超过30例)。正如Zuo等人所指出的那样,目前大部分脑成像实验结果是难以重复的,而要克服这一实验缺陷,大样本数据是未来比较可行的解决办法(Zuo & Xing, 2014)。需要指出的是,已有研究虽然发现了某些脑区存在一定的异常,如激活增强或者减弱、灰质体积降低或增大以及白质微结构FA升高或降低等,但这些结果发现均是由单一模态得出,是由不同样本在不同时间点得出的,而同一组样本是否也存在类似的一致性改变尚不得而知。因此,在未来的研究中如何实现大样本、多模态的研究就成为了相关研究者的工作重点之一。模板可直接用于论文及其文字的编排,有的页边距、行距、字体都严格符合规定,请勿修改!尤其是页边距,由于期刊在后期制作过程中需要在页眉、页脚添加各种信息,所以所有论文务必确保现有的页边距不被修改,页面空白不被占用。
