        <p>痛风性关节炎是一种因体内尿酸过高，导致尿酸盐结晶沉积于关节而引发的代谢性疾病，其主要病理特征为关节炎症反应和组织损伤。近年来，中医治疗因其疗效确切、安全性高、毒副作用小等优势，已成为临床关注的热点。文章旨在综述当前中医内治(包括中药复方、中成药及中药提取物)与外治法在痛风性关节炎治疗中的应用现状，以期为深入探究中医药防治该病的药物靶点与作用机制提供理论参考，并为优化临床治疗方案开拓新思路。</p>

        <p>Gouty arthritis is a metabolic disease caused by the deposition of urate crystals in joints due to hyperuricemia, primarily characterized by inflammatory responses and tissue damage. Currently, Traditional Chinese Medicine (TCM) has become a clinical focus due to its high efficacy, favorable safety profile, and low toxicity and side effects. This article reviews the current status of TCM internal therapies (including herbal formulas, Chinese patent medicines, and herbal extracts) and external treatments for gouty arthritis. The aim is to provide a theoretical basis for further exploration of the pharmacodynamics and mechanisms of TCM in preventing and treating gouty arthritis, and to offer new perspectives for clinical management.</p>

      <title>1. 引言</title>
      <p>痛风性关节炎(Gouty Arthritis, GA)属于中医学“痛风”、“痹症”范畴，临床以肢体筋骨、关节、肌肉的酸楚、麻木、肿痛，乃至后期出现关节屈伸不利、僵硬、畸形等为主要表现。近年来，GA的患病率呈上升趋势，且发病年龄日趋年轻化[<xref ref-type="bibr" rid="B1">1</xref>]。根据2020年中华中医药学会发布的《痛风和高尿酸血症病证结合诊疗指南》，别嘌醇、非布司他、苯溴马隆被推荐为痛风患者降尿酸治疗的一线药物；急性发作期则推荐尽早使用小剂量秋水仙碱或非甾体抗炎药。然而，上述药物的长期应用常伴随胃肠道反应、皮肤过敏、肝肾功能损害及心血管不良事件等副作用，尤其在以老年人群为主的GA患者中更为显著。因此，探寻更为安全、有效的治疗策略以减轻患者痛苦，是当前临床亟待解决的重要课题。中医学对痛风的认识源远流长，早在公元前8世纪的藏医典籍《四部医典》中即有相关记载[<xref ref-type="bibr" rid="B2">2</xref>]，数千年的临床实践积累了丰富的经验。深入挖掘和整理中医药防治GA的理论与方法，有望为临床提供更多元、更安全有效的治疗选择。本文拟对近年来中医药治疗GA的研究进展进行综述。</p>
    </sec>
    <sec id="sec2">
      <title>2. 中药内治法</title>
      <sec id="sec2dot1">
        <title>2.1. 中药复方</title>
        <p>四妙散出自《成方便读》，由苍术、黄柏、牛膝、薏苡仁组成，具清热利湿、舒筋壮骨之功，是治疗痛风的经典方剂。诸多临床有效复方均以其为基础化裁。例如，有学者应用加味四妙散(组成：苍术10 g，黄柏10 g，薏苡仁30 g，川牛膝10 g，茵陈15 g，土茯苓30 g，川萆薢15 g，野菊花10 g，威灵仙30 g，野木瓜15 g，茯苓15 g，独活15 g，甘草5 g)治疗GA，发现治疗组患者的急性炎症指标C反应蛋白(CRP)改善情况优于西药对照组(别嘌醇 + 尼美舒利)，且伴随CRP下降，血尿酸水平亦显著降低，表明该方兼具抑制急性炎症和降低尿酸的双重作用[<xref ref-type="bibr" rid="B3">3</xref>]。另一项研究在四妙散基础上加用黄芪、萆薢[<xref ref-type="bibr" rid="B4">4</xref>]，发现加味四妙汤能有效降低血清尿酸水平，其机制可能与促进尿酸排泄有关；同时，在急性痛风性关节炎模型大鼠中，观察到该方能降低关节肿胀度、改善步态，并显著降低关节浸出液中肿瘤坏死因子-<italic>α</italic> (TNF-<italic>α</italic>)和前列腺素E2 (PGE2)水平，提示其抗炎作用可能与抑制上述炎性介质的合成与释放相关。</p>
        <p>三妙丸出自《医学正传》，由二妙丸(苍术、黄柏)加牛膝而成，具清热燥湿、消肿止痛之效，主治湿热下注之痹症。研究表明[<xref ref-type="bibr" rid="B5">5</xref>]，牛膝与苍术、黄柏的协同作用能增强二妙丸抑制细胞因子的效应，且此作用随牛膝用量增加而增强。三妙丸类方及其君药川牛膝可通过下调核因子<italic>κ</italic>B (NF-<italic>κ</italic>B) P65蛋白表达，抑制TNF-<italic>α</italic>、白介素-6 (IL-6)、白介素-8 (IL-8)水平，从而减轻急性GA大鼠踝关节的炎症反应和滑膜组织病理损伤。</p>
        <p>桂枝芍药知母汤源自《金匮要略》，主治“诸肢节疼痛，身体魁羸，脚肿如脱”，具祛风除湿、温经散寒之功效，临床常用于GA治疗。研究证实[<xref ref-type="bibr" rid="B6">6</xref>]，桂枝芍药知母汤治疗GA的机制与下调NLRP3炎性体信号通路相关因子的表达有关。NLRP3炎性体已被证实[<xref ref-type="bibr" rid="B7">7</xref>]在GA患者的炎症反应中起核心作用。具体而言，该方能显著降低模型大鼠关节滑膜组织中NLRP3蛋白表达，减少其适配蛋白ASC及半胱氨酸蛋白酶-1 (Caspase-1)的表达，并下调下游炎性细胞因子IL-1<italic>β</italic>、TNF-<italic>α</italic>、IL-6的水平，提示其抗炎机制可能在于抑制NLRP3炎性体活化，进而阻断Caspase-1依赖的IL-1<italic>β</italic>成熟及NF-<italic>κ</italic>B介导的炎症级联反应。</p>
        <p>白虎加桂枝汤亦出自《金匮要略》，原治温疟，因其证候(身无寒但热，骨节疼烦)与湿热痹阻型GA相似，故临床常用于治疗GA。在高尿酸血症合并急性GA大鼠模型中[<xref ref-type="bibr" rid="B8">8</xref>]，白虎加桂枝汤可减轻关节肿胀度，改善步态及炎症评分，降低血清CRP水平。同时，能降低血清及踝关节组织中NLRP3、Caspase-1、NF-<italic>κ</italic>B、TNF-<italic>α</italic>、IL-6、IL-1<italic>β</italic>的水平或蛋白表达，并升高血清抗炎因子IL-10水平。这表明白虎加桂枝汤可能通过抑制NLRP3炎性体的激活，调控炎症反应而发挥治疗作用。</p>
      </sec>
      <sec id="sec2dot2">
        <title>2.2. 中成药</title>
        <p>奚九一教授认为痛风之痹由内湿所致，当以健脾护肾祛湿为法，创制痛风灵(由首乌、黄芪、苍术、土茯苓、车前子等组成)。经307例临床验证，其近期有效率高达95.77%，远期有效率达90.00%，且经5年随访未发现毒副反应。</p>
        <p>朱良春教授指出痛风性关节炎多因脏腑失调、痰湿阻滞经脉而成“浊瘀之痹”[<xref ref-type="bibr" rid="B9">9</xref>]，治以泄化浊瘀、调益脾肾。其自创痛风汤(由土茯苓、萆薢、威灵仙、秦艽、山慈菇、桃仁、红花、土鳖虫、地龙、泽兰、泽泻、车前子、苍术、薏苡仁等组成)，并强调临床可随证配伍化痰、养阴等药。</p>
        <p>芪桂痛风片由黄芪、五加皮、桂枝、浙贝母、刘寄奴、海风藤、秦皮、野菊花组成。通过分子相似性比较发现[<xref ref-type="bibr" rid="B10">10</xref>]，该药含较多黄酮类化合物(如木犀草素、异泽兰黄素等)，这类成分可通过促进尿酸排泄治疗高尿酸血症[<xref ref-type="bibr" rid="B11">11</xref>]，并能与膜结合型前列腺素E2合酶-1结合，该酶在炎症与疼痛过程中高表达，提示该药可能通过抑制炎性递质释放和炎性细胞因子分泌治疗GA。网络药理学研究亦提示，芪桂痛风片所含甾体类生物碱可能赋予其抗炎、免疫抑制、解热镇痛、祛风除湿、活血化瘀、修复关节等多重药理作用。</p>
        <p>痛风消为临床经验方[<xref ref-type="bibr" rid="B12">12</xref>]，由萆薢、虎杖、当归、丹参、赤芍、乳香、没药、川牛膝、地龙、丝瓜络、清风藤、薏苡仁、海桐皮、连翘、郁金、鸡内金、醋延胡索、陈皮、甘草组成，临证加减。临床观察显示，其疗效及疼痛改善程度均显著优于秋水仙碱对照组，并能更有效地降低患者血清尿酸(UA)、血沉(ESR)、IL-1<italic>β</italic>、TNF-<italic>α</italic>及环氧合酶-2 (COX-2)水平，肯定了该方治疗急性GA的疗效，其机制可能与调控上述细胞因子水平以抑制炎症过程有关。</p>
      </sec>
      <sec id="sec2dot3">
        <title>2.3. 中药提取物</title>
        <p>秦皮总香豆素提取自中药秦皮。研究证实其对微晶型尿酸钠诱导的大鼠足肿胀及家兔急性GA均有治疗作用[<xref ref-type="bibr" rid="B13">13</xref>]，并能降低正常及高尿酸血症小鼠的血尿酸水平[<xref ref-type="bibr" rid="B14">14</xref>]。</p>
        <p>木犀草素广泛存在于金银花、菊花、夏枯草等中药中。研究表明，木犀草素能减轻急性GA模型大鼠的炎症反应[<xref ref-type="bibr" rid="B15">15</xref>]，其机制主要涉及下调TLR/MyD88/NF-<italic>κ</italic>B信号通路。此外，木犀草素还可通过减少细胞核内p65蛋白表达，抑制NF-<italic>κ</italic>B转录活性，并降低氧化应激水平，从而发挥抗炎和抗氧化双重作用[<xref ref-type="bibr" rid="B16">16</xref>]。</p>
        <p>槲皮素广泛存在于植物与水果中。在GA治疗方面，其可通过抑制IL-1<italic>β</italic>、TNF-<italic>α</italic>、COX-2及一氧化氮(NO)水平，减轻关节肿胀度、滑膜组织损伤及血管扩张充血[<xref ref-type="bibr" rid="B17">17</xref>]。同时，还能降低脂质过氧化终产物丙二醛(MDA)水平，提高抗氧化酶超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-Px)、过氧化氢酶(CAT)的活性。</p>
        <p>牛膝总皂苷为中药牛膝的主要活性成分之一。其可通过抑制NLRP3炎性体的装配及Caspase-1的激活[<xref ref-type="bibr" rid="B18">18</xref>]，调节下游炎性因子TNF-<italic>α</italic>及IL-1<italic>β</italic>的分泌，从而缓解痛风急性期症状。实验发现，萆薢总皂苷与牛膝总皂苷合用可增强抗炎作用[<xref ref-type="bibr" rid="B19">19</xref>]，显著降低模型大鼠关节浸出液中NO、PGE2、IL-1<italic>β</italic>、IL-6的水平，提示其协同抗炎机制与抑制炎性细胞因子合成有关。</p>
        <p>穿山龙皂苷提取自药材穿山龙。实验表明[<xref ref-type="bibr" rid="B20">20</xref>]，穿山龙皂苷能显著降低GA模型小鼠血清尿酸值，抑制黄嘌呤氧化酶和腺苷脱氨酶活性，并回调肾组织阴离子转运体1 (OAT1) mRNA的表达，提示其可能通过上调OAT1表达，促进尿酸排泄。其抗炎作用[<xref ref-type="bibr" rid="B21">21</xref>]可能与抑制IL-1<italic>β</italic>的产生，进而影响下游SDF-1/CXCR4、PI3K/PKC及NF-<italic>κ</italic>B信号转导通路有关。</p>
        <p>秦艽乙醇提取物可显著降低GA模型动物血清TNF-<italic>α</italic>、基质金属蛋白酶-3 (MMP-3)、IL-1<italic>β</italic>、IL-6水平[<xref ref-type="bibr" rid="B22">22</xref>]，提示其通过调控这些炎性因子的表达，发挥保护关节软骨、减轻炎症损伤的作用。</p>
        <p>虎杖提取物可通过抑制大鼠滑膜组织中细胞间黏附分子-1 (ICAM-1)和NF-<italic>κ</italic>Bp65的异常表达与激活[<xref ref-type="bibr" rid="B23">23</xref>]，发挥对GA的防治作用。另有实验证实[<xref ref-type="bibr" rid="B24">24</xref>]，虎杖醇提物能显著降低模型小鼠踝关节肿胀度，并抑制关节滑膜中IL-1<italic>β</italic>、IL-6、TNF-<italic>α</italic>水平及NLRP3/ASC/Caspase-1轴相关蛋白和mRNA的表达，提示其作用机制可能与调控该炎性体轴有关。</p>
        <p>葛根提取物具有抗炎活性[<xref ref-type="bibr" rid="B25">25</xref>]，主要通过促进NO释放、降低外周血白细胞、中性粒细胞和淋巴细胞数量来实现，不仅能缓解关节肿胀，还能减轻炎症引起的脾肿大及肾损伤，同时兼具降低血清尿酸的作用。</p>
        <p>白藜芦醇可通过激活Sirt1抑制关节炎症并下调血清尿酸水平[<xref ref-type="bibr" rid="B26">26</xref>]，且其抗炎作用呈现剂量依赖性。Sirt1被认为是连接炎症抑制与尿酸代谢调节的关键分子[<xref ref-type="bibr" rid="B26">26</xref>]。同时，白藜芦醇还能下调尿酸转运体GLUT9的表达，促进尿酸排泄，进一步降低血清尿酸水平[<xref ref-type="bibr" rid="B27">27</xref>]。</p>
        <p>土茯苓总黄酮能显著降低小鼠踝关节肿胀度及滑膜组织中IL-1<italic>β</italic>、IL-6、TNF-<italic>α</italic>水平，以及ASC、Caspase-1蛋白和mRNA水平[<xref ref-type="bibr" rid="B28">28</xref>]，表明其抗GA机制与抑制NLRP3炎性体轴活化，进而减少下游炎性因子表达有关。</p>
        <p>近年来，从传统中药中筛选出的多种活性成分被证实可直接靶向NLRP3炎性体，为痛风治疗提供了新的策略。例如，源于中药白术内酯的木香烃内酯和源于青蒿的冬凌草甲素均可共价结合NLRP3的NACHT结构域，阻断NEK7-NLRP3相互作用，从而抑制NLRP3炎性体活化，减轻尿酸盐晶体诱导的炎症反应[<xref ref-type="bibr" rid="B29">29</xref>]。地胆草中的异土木香内酯也被发现能选择性抑制NLRP3，并在小鼠痛风模型中显示出良好的抗炎效果。这些发现不仅揭示了中药的现代科学内涵，也为开发新型NLRP3抑制剂提供了先导化合物[<xref ref-type="bibr" rid="B30">30</xref>]。</p>

    </sec>
    <sec id="sec3">
      <title>3. 外治法</title>
      <sec id="sec3dot1">
        <title>3.1. 针灸疗法</title>
        <p>针灸治疗GA具有操作便捷、起效迅速、毒副作用小等优点。临床研究[<xref ref-type="bibr" rid="B31">31</xref>]选取曲池、血海、三阴交、太溪等穴位进行针刺，发现治疗后患者血清尿酸水平显著下降，且降尿酸效果优于口服吲哚美辛或别嘌呤醇的对照组。围刺法在GA治疗中亦有确切疗效[<xref ref-type="bibr" rid="B32">32</xref>]，取阿是穴、足三里、阴陵泉、筑宾、支沟、内庭、陷谷、三阴交等穴行围刺[<xref ref-type="bibr" rid="B33">33</xref>]，能明显改善患者症状与体征，尤其对急性发作期的关节肿痛具有显著的消炎止痛作用。</p>
        <p>刺血疗法应用广泛，未见明显不良反应报道[<xref ref-type="bibr" rid="B34">34</xref>]。在大鼠急性GA模型中[<xref ref-type="bibr" rid="B35">35</xref>]，阿是穴刺血后，模型大鼠局部K<sup>+</sup>、多巴胺(DA)、5-羟色胺(5-HT)含量显著下降，表明其能有效抑制疼痛介质，发挥外周镇痛作用。此外，刺血疗法还具有抗炎效应[<xref ref-type="bibr" rid="B36">36</xref>]，其机制可能与增加抗炎细胞因子IL-10的表达有关。有学者[<xref ref-type="bibr" rid="B37">37</xref>]探讨了刺血量对疗效的影响，发现10 mL刺血量止痛效果更佳，且与5 mL刺血组相比，降尿酸作用更为显著。采用火针放血时，不同出血量疗效亦有差异，一项比较20 mL、40 mL、60 mL总出血量的研究发现[<xref ref-type="bibr" rid="B38">38</xref>]，疗效与出血量呈正相关(每穴出血量宜控制在30 mL以内)。刺血疗法联合温针灸或艾灸[<xref ref-type="bibr" rid="B39">39</xref>]，不仅可增强消炎镇痛作用(下调UA及炎症指标)，且相较于秋水仙碱，联合艾灸组未出现明显胃肠道反应、白细胞减少、皮疹等副作用。</p>
        <p>刺络拔罐放血疗法是临床常用方法。对于湿热蕴结型急性GA，该法能有效缓解疼痛[<xref ref-type="bibr" rid="B40">40</xref>]。选取关节红肿疼痛处行刺络拔罐放血[<xref ref-type="bibr" rid="B41">41</xref>]，比较治疗前后血尿酸、CRP、ESR及疼痛视觉模拟评分(VAS)的变化，结果显示该疗法能有效改善上述指标，且CRP、ESR及VAS评分的改善情况优于口服秋水仙碱的对照组，表明其安全性高、疗效确切。</p>
        <p>电针是现代科技与传统针灸的结合。研究证明[<xref ref-type="bibr" rid="B42">42</xref>]，电针结合穴位注射(复方当归注射液)能有效止痛并降低尿酸，疗效优于口服别嘌呤醇联合消炎痛的对照组。</p>
      </sec>
      <sec id="sec3dot2">
        <title>3.2. 外敷疗法</title>
        <p>四黄水蜜(由大黄、黄芩、黄连、黄柏加蜂蜜调制而成)外敷，联合加味四妙散内服[<xref ref-type="bibr" rid="B43">43</xref>]，可显著改善急性GA患者的症状体征，并降低血尿酸、白细胞计数及体温，起效迅速且未见明显毒副作用。金黄膏外敷治疗急性GA的研究[<xref ref-type="bibr" rid="B44">44</xref>]表明，其能有效降低血浆IL-1<italic>β</italic>和IL-8水平，且金黄膏联合戴芬(双氯芬酸钠)组疗效最优，单用金黄膏组次之，单用戴芬组再次之。</p>

    </sec>
    <sec id="sec4">
      <title>4. 结语与展望</title>
      <p>综上所述，中医药治疗痛风性关节炎具有悠久历史和确切的临床疗效。近年来，随着分子生物学技术的进步，中医药治疗GA的机制研究已从宏观症状观察深入到细胞因子、信号通路及基因表达调控层面，尤其是在调控NLRP3炎性体、NF-<italic>κ</italic>B通路及氧化应激等方面的研究取得了重要进展，为中医药治疗GA提供了现代科学内涵。</p>
      <p>然而，该领域研究仍存在以下亟待解决的问题：第一，研究深度不足。多数机制研究仍停留在检测若干炎症因子或单一通路的层面，缺乏对中药复方“多成分–多靶点–多通路”协同作用的系统性阐释，且鲜见采用基因敲除、RNA干扰等技术进行反向验证的研究。第二，缺乏毒理学评价与长期随访。目前临床研究多为短期疗效观察，对中药长期应用的肝肾功能影响、药物相互作用及远期安全性缺乏系统评估；多数RCT样本量小、设计欠严谨，缺乏双盲、安慰剂对照的高质量研究。第三，基础与临床脱节。现有机制研究多集中于动物模型，与临床患者实际情况存在差距，且缺乏将机制研究成果转化为临床生物标志物的探索。第四，“治未病”研究薄弱。针对高危人群及间歇期的预防性干预研究鲜有报道，未能充分发挥中医“未病先防”的优势。</p>
      <p>针对上述问题，未来研究可从以下方向突破：一是引入系统生物学方法，采用代谢组学、蛋白组学、网络药理学等多组学技术，构建中药复方“成分–靶点–通路–疾病”的相互作用网络，系统揭示其多靶点调控机制。二是加强高质量临床研究，开展多中心、随机、双盲、安慰剂对照的临床试验，并建立长期随访队列，评估中医药的远期疗效及安全性。三是推进机制验证研究，结合分子对接、表面等离子体共振等技术筛选中药活性成分的直接靶点，并利用基因编辑技术进行功能验证。四是深化“治未病”研究，针对高尿酸血症人群及痛风间歇期患者，开展饮食调理、运动干预及中药预防的随机对照试验，构建中医特色防治体系。五是加强毒理学研究，开展中药长期用药的肝肾毒性、生殖毒性等系统评价，为临床安全用药提供依据。</p>
      <p>随着现代科学技术与中医药理论的深度融合，相信中医药防治GA的机制将得到更全面细致的揭示，中医药的科学内涵将进一步彰显，为GA的临床治疗提供更多安全有效的创新方案。</p>
